Understanding the consequences of being RhD immunized during pregnancy 10 years after introduction of targeted routine antenatal anti-D prophylaxis: A retrospective nationwide cohort study.

INTRODUCTION: Targeted routine antenatal anti-D prophylaxis (RAADP) was introduced in Finland in 2014. The aim of this study was to assess the prevalence of anti-D immunizations among pregnant women and the severity of hemolytic disease of the fetus and newborn (HDFN) in affected pregnancies 10 years after RAADP was added to the national prevention program in the context of a rapidly declining birthrate in Finland. MATERIAL AND METHODS: A nationwide, retrospective cohort study included all RhD-negative pregnancies in Finland between 2014 and 2023. Information on antenatal screening was obtain

Predominance of Rh-mediated haemolytic disease driving exchange transfusions in rural north India: A five-year retrospective analysis revealing critical prevention gaps.

Exchange transfusion remains the definitive treatment for severe neonatal hyperbilirubinaemia; yet its aetiology and outcomes in rural tropical settings are poorly characterised. This five-year retrospective analysis of 58 neonates at a North Indian rural tertiary centre reveals that Rh incompatibility accounted for 76% of cases - a striking divergence from high income-country patterns where ABO incompatibility predominates - indicating critical gaps in antenatal Rh immuno-prophylaxis. The procedure achieved a mean bilirubin reduction of 50% (424 ± 106 to 212 ± 70&mu/L, p < 0.001) with no proc

Evaluating alloimmunization risk in patients with sickle cell disease and in prenatal care patients with RHCE variants.

BACKGROUND AND OBJECTIVES: Clinically significant alloantibodies complicate transfusion and prenatal care, especially in individuals with genetic variants affecting high-frequency antigens. Many patients from African descent carry RHCE*ceVS alleles, which alter the expression of c (RH4), e (RH5) and hrB (RH31). However, the risk and clinical impact of alloimmunization remain uncertain. We evaluated the alloimmunization in a cohort of patients with a RHCE*ceVS genotype. MATERIALS AND METHODS: We conducted a retrospective study on 48 patients with a RHCE*ceVS genotype divided into three categor

A novel and efficient feature extraction algorithm using kmer-derived mutation signal.

Effective algorithms for extracting genomic features are crucial for downstream bioinformatics research. Although kmer-based descriptive statistical features (frequency and position) offer a unique perspective for genomic feature extraction, their biological significance warrants further investigation. Therefore, uncovering the biological significance of kmers in genomes remains a significant challenge in current kmer-based genomic feature mining. In this work, based on reverse, complementary and reverse-complementary of kmers to simulate genomic accumulation mutation behavior, we proposed a n

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Frequency of maternal red blood cell alloantibodies and clinical utility of antibody screening in Abuja, Nigeria.

BACKGROUND: Pregnancy causes immunisation when the fetal red cells, possessing a paternal antigen foreign to the mother, enter the maternal circulation, resulting in the production of alloantibodies. Maternal allo-immunisation, also known as iso-immunization, occurs when a woman's immune system is sensitised to foreign erythrocyte surface antigens, stimulating the production of immunoglobulin G (IgG) antibodies. This study was carried out to determine the frequency of maternal Red blood cell alloantibodies in Gwarimpa General Hospital, FCT, Abuja, and to evaluate the clinical utility of antibo

Rhesus D Immune Globulin in Early Pregnancy: A Clinical Review.

This article reviews the actively evolving clinical practice of Rhesus D (RhD) testing and RhD immune globulin (RhIg) prophylaxis in the context of first trimester bleeding events, including abortion. It summarizes population-level data and indirect biologic studies, while comparing current guidelines for RhD testing and RhIg prophylaxis, highlighting existing discrepancies. Considerations such as cost, access, shortages, and fetal RhD status testing via noninvasive prenatal testing are addressed in this discussion. Although several organizations have updated their clinical guidelines to align

Human parvovirus B19 vertical infection and hydrops fetalis. A case report.

Non-immune hydrops fetalis represents a diagnostic challenge in high-risk pregnant women. Vertical infection with human parvovirus B19 (B19V) is a possible cause. National guidelines propose maternal serologic screening (IgG/IgM), which may be insufficient in some situations. We report a case of vertical B19V infection with difficulties in prenatal diagnosis. Preterm newborn, normal weight (2950 g), born to a 30-year-old mother with anemia and hydrops fetalis (week 17). Cardiac, chromosomal, isoimmunization-Rh, and usual infectious causes (TORCH) were ruled out. Maternal serology for B19V show

[Current role of the Kleihauer-Betke test in monitoring pregnant women].

The Kleihauer test is a widely prescribed examination today. However, we propose revisiting certain indications beyond the prevention of maternal-fetal alloimmunization, particularly concerning the adjustment of anti-D immunoglobulin dosage. Indeed, some prescriptions seem irrelevant based on the literature and the experience of the reference center. These include chorionic villus sampling, systematic prescription in cases of reduced fetal movements or metrorrhagia. Other indications could be limited to specific situations, such as direct abdominal trauma, external cephalic version (ECV) in Rh

Guidance for Prenatal, Postnatal and Neonatal Immunohematology Testing in Canada: Consensus Recommendations from a Modified Delphi Process.

OBJECTIVES: Blood Group, antibody screen, fetal maternal hemorrhage tests and Rh(D) immunoglobulin (RhIG) administration are interventions during pregnancy that aid in the prevention of hemolytic disease of the fetus and newborn (HDFN). The timing, frequency, and nature of testing vary across centres due to limited data to inform standards development. Using Delphi methodology, this study aimed to establish guidance for Canadian practice related to prenatal, postnatal and neonatal immunohematologic testing, and RhIG administration, to reduce risk and improve diagnosis of HDFN. METHODS: A nati

Clinical Implementation of RHD NIPT in a Tertiary Obstetric Centre in Western Australia.

A clinically robust RHD non-invasive prenatal test (RHD NIPT) was developed to predict fetal RhD status and optimise RhD Ig use for RhD-negative pregnant women in Western Australia. Using rigorous methodology and stakeholder engagement we created clinical guidelines and educational support tools, such as reference guides and consumer brochures, to enhance test uptake. Six months post-implementation, 90% of healthcare professionals surveyed felt confident offering the test, with 87% having ordered it. A guideline review of 50 women confirmed they all received appropriate RhD Ig use. This succes

Management of Red Cell Alloimmunization in Pregnancy.

Rhesus immune globulin has resulted in a marked decrease in the prevalence of RhD alloimmunization in pregnancy; however, antibody formation to other red cell antigens continues to occur. Evaluation for the presence of anti-red cell antibodies should be routinely undertaken at the first prenatal visit. If anti-red cell antibodies are detected, consideration of a consultation or referral to a maternal-fetal medicine specialist with experience in the monitoring and treatment of these patients is warranted. Cell-free DNA can be used to determine fetal red cell antigen status to determine whether

The use of free DNA for fetal RHD genotyping in the Rh negative pregnant patient-the time has come.

Cell-free DNA to determine the fetal RHD genotype from the maternal circulation was first described in 1993. High throughput assays using polymerase chain reaction technology were introduced in Europe and gained widespread acceptance in the management of the Rhesus alloimmunized pregnancy. The specificity and sensitivity of these assays approached 99%. As confidence was gained with these results, Scandinavian countries began to employ cell-free DNA for fetal RHD typing as an integral component of their introduction of antenatal Rhesus immune globulin in non-alloimmunized pregnancies. Since 40%

Rho(D) immune globulin shortage and fetal Rh(D) screening with cell-free DNA.

PURPOSE OF REVIEW: Despite the availability of Rh(D) immune globulin (RhIg) to prevent alloimmunization in Rh(D)-negative pregnant patients, anti-Rh(D) alloimmunization remains a prevalent cause of hemolytic disease of the fetus and newborn (HDFN). Recent RhIg shortages have caused clinicians and professional societies to identify methods to prioritize RhIg administration. New cell-free DNA (cfDNA) tests to predict fetal red blood cell antigen genotypes have been proposed as an option to prioritize the administration of RhIg to Rh(D)-negative pregnant people. RECENT FINDINGS: Commercial labor

Clinical Characteristics and Outcomes of Intrauterine Blood Transfusion (IUT) for Infectious Etiologies.

OBJECTIVE: Congenital viral infection may result in fetal anemia and thrombocytopenia. While intrauterine blood transfusions (IUTs) are more commonly performed for Rh alloimmunization, reports using IUT for infection have varying success. Our primary objective was to characterize the outcomes of patients undergoing IUT for infectious etiologies at our center compared with Rh disease. STUDY DESIGN: This was a case series of patients undergoing IUT from 2012-2023. Infectious etiologies were identified by maternal serologies and confirmed by amniotic fluid polymerase chain reactions (PCR). Clini

Diastolic deceleration area in the fetal MCA: a new Doppler parameter.

Objective: Doppler velocimetry has been widely used throughout the years as a valuable tool in the follow-up and prognosis of various pregnancy complications. Numerous Doppler indices have been introduced to qualitatively describe fetal blood flow. Currently, the Pulsatility index (PI) is the most widely used index for this purpose. In current clinical practice, middle cerebral artery (MCA) PI measurement is commonly used to assess fetal well-being, especially in late-onset fetal growth restriction (FGR). However, existing evidence suggests that MCA PI alone is inferior to the ratio between MC
