Study of compliance with a new, targeted antenatal D immunization prevention programme in Denmark.

BACKGROUND AND OBJECTIVES: A targeted routine antenatal anti-D prophylaxis programme was implemented in Denmark where anti-D immunoglobulin is given based on the result from noninvasive antenatal screening for fetal RHD. Our objective was to evaluate compliance with this new programme right after its initiation. MATERIALS AND METHODS: We examined the treatment outcome of 239 D-negative pregnant women who gave birth at our hospital between June and September 2010. RESULTS: The majority of these women (90%) underwent antenatal RHD screening, 86% of the women who were recommended antenatal prop

Significance of serological monitoring in a Bombay Rh (D) negative phenotype pregnant woman: a case report.

A 32 year old Indian female was referred to our hospital at 32 weeks of gestation because of difficulty in blood group determination and further antenatal care. The results of cell and serum grouping of her blood sample were suggestive of Bombay (O(h)) Rh (D) negative phenotype. An indirect antiglobulin test (IAT) using a pool of red cells from two Bombay Rh (D) positive blood donors gave negative result using the tube as well as the gel technique (LISS-Coombs Card, BioRad, Switzerland), thus ruling out anti-D antibody in her serum. The anti-H titer was 16 (tube technique) and with dithiothrei

Prediction of the rate of decline in fetal hemoglobin levels between first and second transfusions in red cell alloimmune disease.

OBJECTIVE: To determine variables that predict the rate of decline in fetal hemoglobin levels in alloimmune disease. METHOD: Retrospective review of singleton pregnancies that underwent first and second intrauterine transfusions for treatment of fetal anemia because of maternal Rh alloimmunization in a tertiary referral center. RESULTS: Forty-one first intrauterine transfusions were performed at 26.1 weeks (standard deviation, SD, 4.6), mean volume of blood transfused was 44.4 mL (SD 23.5) and estimated feto-placental volume expansion was 51.3% (SD 14.5%). Between first and second transfusio

The prevalence of Rhesus negativity among pregnant women in Enugu, Southeast Nigeria.

BACKGROUND: Hemolytic disease of the newborn, secondary to rhesus D (Rh D) iso-immunization, contributes significantly to perinatal morbidity and mortality. Prevalence data in Nigeria, and Southeast Nigeria in particular, is very scanty. This study was carried out to provide our experience in this preventable clinical condition in Enugu, Southeast Nigeria. OBJECTIVE: To determine the prevalence and trends of Rh D negativity among pregnant women in Enugu, Southeast Nigeria. MATERIALS AND METHODS: A 5-year retrospective study of rhesus negative women was carried out at the University of Nigeri

CHAOS in the mirror: Ballantyne (mirror) syndrome related to congenital high upper airway obstruction syndrome.

In the mirror syndrome, maternal edema mirrors fetal edema. The pathogenesis is unknown. The most common etiologic associations are rhesus isoimmunization, twin-twin transfusion syndrome, and viral infections. Less than 10% of reported cases are associated with congenital anomalies. We report a case due to congenital laryngeal stenosis, which also caused congenital high airway obstruction syndrome (CHAOS), characterized by pulmonary hyperplasia and edema or anasarca, related to airway abnormality. The fetal manifestations of the mirror syndrome and CHAOS overlap, but occurrence of the two in t

Fetal RhD genotyping by real time quantitative PCR in maternal plasma of RhD-negative pregnant women from the Sahel of Tunisia.

OBJECTIVE: The aim of this study was to evaluate the diagnostic value of RhD fetal genotyping from the plasma of RhD-negative pregnant women. METHODS: We analysed the plasma samples of 65 pregnant women. DNA quantification was done using real time quantitative PCR (RQ-PCR) in multiplex targeting multiple RhD exons 5, 7 and 10, with a standardized pool of plasmid calibrators. Results were compared with serological analysis of cord blood after delivery. RESULTS: Fetal RhD status was predicted with 95.38% accuracy from maternal plasma of pregnant women in the 11(th) to 40(th) weeks of gestation

A new fetal RHD genotyping test: costs and benefits of mass testing to target antenatal anti-D prophylaxis in England and Wales.

BACKGROUND: Postnatal and antenatal anti-D prophylaxis have dramatically reduced maternal sensitisations and cases of rhesus disease in babies born to women with RhD negative blood group. Recent scientific advances mean that non-invasive prenatal diagnosis (NIPD), based on the presence of cell-free fetal DNA in maternal plasma, could be used to target prophylaxis on "at risk" pregnancies where the fetus is RhD positive. This paper provides the first assessment of cost-effectiveness of NIPD-targeted prophylaxis compared to current policies. METHODS: We conducted an economic analysis of NIPD im

The MCA Doppler and its role in the evaluation of fetal anemia and fetal growth restriction.

Doppler velocimetry of the middle cerebral artery (MCA) has played a major role in fetal medicine for the last 23 years, both in intrauterine growth-restricted (IUGR) and anemic fetuses. Its utility in the diagnosis and management of cases of fetal anemia was initially demonstrated in cases of red cell alloimmunization and later extended to other types of anemia. In addition, MCA Doppler studies are crucial in the evaluation of IUGR fetuses. This article is a review of the role of the MCA in these 2 conditions.

Development of a transparent interactive decision interrogator to facilitate the decision-making process in health care.

BACKGROUND: Decisions about the use of new technologies in health care are often based on complex economic models. Decision makers frequently make informal judgments about evidence, uncertainty, and the assumptions that underpin these models. OBJECTIVES: Transparent interactive decision interrogator (TIDI) facilitates more formal critique of decision models by decision makers such as members of appraisal committees of the National Institute for Health and Clinical Excellence in the UK. By allowing them to run advanced statistical models under different scenarios in real time, TIDI can make th

Routine antenatal anti-D prophylaxis--is the protection adequate?

BACKGROUND/AIMS: Prophylactic anti-D given during pregnancy can be detected in current indirect antiglobulin tests (IAT). Using this to measure the persistence of prophylactic anti-D, this study set out to determine whether there is an association between anti-D detectable at delivery and the RhD status of the foetus and/or the duration of the pregnancy post the standard 28 week dose of routine antenatal anti-D prophylaxis (RAADP). The study also investigated the detection rates of anti-D at delivery when given in a two dose regime or a one dose regime. METHOD: All IAT screening was undertake

[Management of feto-maternal red cell allo-immunizations].

Feto-maternal red cell alloimmunization is defined by the presence in a pregnant woman of alloantibodies directed against blood group antigens present on the red blood cells of the fetus and inherited from the father. It arises from the immune response to a first contact to these same antigens during a prior transfusion, transplant or pregnancy. The placental transfer and the fixation of the antibodies on the fetal red cells antigenic targets lead to a haemolysis in the fetus and the newborn. The resulting haemolytic disease can show different clinical forms, from a mild anaemia with neonatal

Report of the first nationally implemented clinical routine screening for fetal RHD in D- pregnant women to ascertain the requirement for antenatal RhD prophylaxis.

BACKGROUND: A combination of antenatal and postnatal RhD prophylaxis is more effective in reducing D immunization in pregnancy than postnatal RhD prophylaxis alone. Based on the result from antenatal screening for the fetal RHD gene, antenatal RhD prophylaxis in Denmark is given only to those D- women who carry a D+ fetus. We present an evaluation of the first national clinical application of antenatal RHD screening. STUDY DESIGN AND METHODS: In each of the five Danish health care regions, blood samples were drawn from D- women in Gestational Week 25. DNA was extracted from the maternal plasm

Predicting antepartum stillbirth.

Stillbirth affects approximately 1 in 200 pregnancies and is one of the most common serious complications of pregnancy. Stillbirth can occur as a consequence of diverse processes, including fetal abnormality, isoimmunization, infection, maternal preeclampsia, placental abruption, maternal medical conditions, complications of labor and delivery, growth restriction, and also often occurs in the absence of an apparent cause or predisposing factor. This review summarizes a number of factors which identify women at increased risk of stillbirth, including maternal characteristics, blood tests, and b

When anti-G and anti-C antibodies masquerade as anti-D antibody.

We describe a case of a pregnant woman with anti-C/anti-G antibodies masquerading as anti-D antibodies. Further, confirmation of anti-D antibody is recommended with adsorption-elution studies to confirm the true antibody status. This will avoid the consequence of withholding Rh immunoglobulin prophylaxis in cases when anti-D antibodies are not present.
