Long term costs and effects of reducing the number of twin pregnancies in IVF by single embryo transfer: the TwinSing study.

BACKGROUND: Pregnancies induced by in vitro fertilisation (IVF) often result in twin gestations, which are associated with both maternal and perinatal complications. An effective way to reduce the number of IVF twin pregnancies is to decrease the number of embryos transferred from two to one. The interpretation of current studies is limited because they used live birth as outcome measure and because they applied limited time horizons. So far, research on long-term outcomes of IVF twins and singletons is scarce and inconclusive. The objective of this study is to investigate the short (1-year) a

Incidence of delivery of low birthweight infants in twin gestations.

BACKGROUND: Twin gestations are commonly associated with delivery of low birthweight infants. Despite the fact that Nigeria has the highest incidence of twinning in the world, there is paucity of information on the characteristics of LBW twin infants in Nigeria. OBJECTIVE: To determine the incidence and document some of the characteristics of low birth weight (LBW) twin infants. METHOD: A descriptive analysis of data on 119 LBW twin infants delivered between 1st January, 2000 and 31st December, 2003 in a Nigerian mission hospital in Benin City was carried out. The characteristics of the LBW

Developmental dysplasia of the hip in twins: the importance of mechanical factors in the etiology of DDH.

BACKGROUND: Developmental dysplasia of the hip (DDH) is a disease of multifactorial etiology. Genetic and intrauterine mechanical factors influence the development of the hip joint and the incidence of DDH. Breech position is a risk factor for DDH. The aim of this study is to analyze whether in twins, the intrauterine mechanical factors, such as a breech position, limitation of fetal mobility, limitation of hip abduction, and increase the incidence of DDH may, therefore, be considered as risk factors. METHODS: A pair of twins with either breech or cephalic presentation, and no family history

[Influence of sperm morphology on the outcomes and neonatal status in IVF-ET].

OBJECTIVE: To determine the influence of sperm morphology on the outcomes and status of the neonates in in vitro fertilization and embryo transfer (IVF-ET). METHODS: Strictly based on the WHO criteria, we divided semen samples into a moderately abnormal group (sperm of normal morphology 5% - 10%), a mildly abnormal group (10% < sperm of normal morphology < 15%) and a normal group (sperm of normal morphology > or = 15%) , and compared the rates of fertilization, cleavage, quality embryos, implantation, clinical pregnancy and live births among the three groups. RESULTS: There were not signific

First successful birth of twins in India following the transfer of vitrified oocytes.

We report the first twin pregnancy and birth in India after the transfer of embryos generated from vitrified and thawed oocytes to a 41-year-old woman who was in a donor program. Embryos were generated from the microinjection of pre-prepared sperms into vitrified oocytes. Twin male babies weighing 750 and 860 g were born by emergency cesarean section. Vitrification is one of the most promising options in cryopreservation and preservation of embryos and oocytes.

Examining autonomy's boundaries: a follow-up review of perinatal mortality cases in UK independent midwifery.

BACKGROUND: An earlier matched cohort study in the United Kingdom found a significantly higher perinatal mortality rate for births booked under an independent midwife compared with births in National Health Service units (1.7% [25/1,508] vs 0.6% [45/7,366]). This study examined independent midwives' management and decision making in the 15 instances of perinatal death that occurred at term. METHODS: Thematic analysis of independent midwives' case notes was performed in instances of perinatal mortality. Semi-structured interviews were conducted with the midwives concerned. RESULTS: Home birth

Trends in the obstetric features and management of twin pregnancies.

There are no reports outlining the trends in obstetric features and clinical management of twin pregnancies in an Irish obstetric population. The aim of this study was to investigate these factors for all twin pregnancies delivered during the 19 year period between 1989 and 2007, at Galway University Hospital (GUH). There were 52,199 infants delivered at GUH, of which 1594 infants (3.05% of births) were twins, related to 797 twin pregnancies. The overall incidence of twin pregnancies was 1.52%, increasing from 0.8%-1.0% in the early years of the study to 1.7-1.8% in the latter years of the stu

Radiofrequency ablation for selective reduction in complex monochorionic pregnancies.

Monochorionic pregnancies present unique challenges for selective fetal reduction, as vaso-occlusive procedures are required to ablate blood flow, usually in the umbilical cord, to achieve asystole in the selected fetus. We describe a case series of 35 monochorionic pregnancies (27 twins and eight triplets) undergoing selective fetal reduction using radiofrequency ablation. All procedures were performed under local anaesthesia. The procedure was technically successful in all cases. The live born rate was 88.6%. One (2.9%) woman miscarried within 2 weeks of the procedure, and two (5.7%) babies

Emergency cervical cerclage: predictors of success.

OBJECTIVE: To document predictors of success of emergency cervical cerclage. METHODS: This is a retrospective cohort study of 8 years at a university hospital. Emergency cerclage was defined as when the membranes were at or beyond the external os and was only performed where evidence of infection or labour were absent. Outcomes used were interval between cerclage and delivery, gestation at delivery; a 'good outcome' was defined as delivery after 32 weeks and healthy at discharge. Predictive factors for a successful pregnancy outcome were analysed using odds ratios (OR) with 95% confidence int

A preterm infant with semilobar holoprosencephaly and hydrocephalus: a case report.

INTRODUCTION: Holoprosencephaly (HPE) is a rare presentation in newborns. It refers to an incomplete or absent division of the prosencephalon or forebrain into distinct cerebral hemispheres. CASE PRESENTATION: A preterm baby girl, first of dizygotic twins, born at 26 weeks of gestation to a 45 year old mother, was prenatally diagnosed with ventriculomegaly on fetal ultrasonogram. At birth the baby had frontal bossing with sun setting eyes and a full anterior fontanel. Initial head ultra-sonography (HUS) showed ventriculomegaly and semilobar holoprosencephaly, which was confirmed by computed t

Fetal diagnosis of penoscrotal transposition associated with perineal lipoma in one twin.

Perineal lipoma is a rare condition which is commonly associated with scrotal anomalies and may be diagnosed as ambiguous genitalia. We report on a baby with penoscrotal transposition (PST) consisting of bifid scrotum and hypospadias with chordee associated with perineal lipoma. The affected baby was one twin of an IVF pregnancy referred to our center for ambiguous genitalia by fetal ultrasound. Examination of the uterus revealed a dichorionic, diamniotic twin pregnancy. Fetal ultrasound and MRI in our center revealed an exophytic perineal mass, which likely represented a lipoma, separate from
