Uncomplicated monochorionic twins: neurodevelopmental insights and implications.

OBJECTIVE: Emerging evidence indicates that uncomplicated monochorionic diamniotic (MCDA) twins deviate physiologically from singletons, probably owing to adaptations to their unique intrauterine environment. However, their long-term neurodevelopmental outcomes remain poorly understood. Early identification of neurodevelopmental risks is critical to optimize neuroplasticity during key developmental periods. The aim of this study was to assess neurodevelopmental outcomes in uncomplicated MCDA twins up to 3 years of age and propose recommendations for future research and follow-up protocols. ME

Impact of chorionicity on birth weight discordance in twins.

BACKGROUND: Advanced maternal age (AMA, ≥35 years) is increasing, linked to delayed childbearing and infertility, with medically assisted reproductive (MAR) technology boosting twin pregnancy rates. Birth weight discordance in twins [BWDT, birth weight discordance (∆BW) >20%] raises neonatal morbidity. Chorionicity matters: monochorionic diamniotic (MCDA) twins face higher risks from placental anastomoses, while dichorionic diamniotic (DCDA) twins have better outcomes. This study explores their association with BWDT. The aim of this study was to characterize the association of chorionicity sta

Optimizing the prevention of mother-to-child transmission of hepatitis B through integrated care models: a pilot study in Jiangsu Province, China.

BACKGROUND: Preventing mother-to-child transmission of hepatitis B (PMTCT) is crucial for reducing the incidence of new hepatitis B virus (HBV) infections. However, the current PMTCT interventions including maternal screening, antiviral prophylaxis, infant immunoprophylaxis, and post-vaccination serological testing, fail to reach a broader population, especially those in rural areas. This pilot study aimed to optimize the PMTCT strategy by integrating existing medical resources, with the goal of achieving high coverage of the complex intervention services. METHODS: This real-world pilot study

A core outcome set for future male infertility research: development of an international consensus.

STUDY QUESTION: Can a core outcome set be developed through a global consensus to standardize outcome selection, collection, comparison, and reporting in future male infertility trials? SUMMARY ANSWER: A minimum dataset, known as a 'core outcome set', has been developed for randomized controlled trials (RCTs) and systematic reviews evaluating potential interventions for male infertility. WHAT IS KNOWN ALREADY: Numerous factors, including a failure to consider the perspectives of men with lived experiences of infertility or their partners when developing and conducting RCTs can limit their cl

Symmetrical Thalamic Lesions in Preterm Triplets: What Can We Learn From the Different Phenotypes?

BACKGROUND: Symmetrical thalamic lesions (STL) is a rare condition that affects term newborns who have suffered an antenatal hypoxic insult and present at birth with specific clinical features: feeding and swallowing difficulties, facial hypomimia, apnea, and frequently hypertonia. Recently STL have also been described in preterm infants. CASE DETAILS: We describe the first case series of STL in preterm triplets with a variable spectrum of clinical outcomes, following a clearly identified perinatal anoxic insult. Spontaneous male triplets, two monochorionic-diamniotic (T1 and T2) and one dich

Neonatal Cholestasis Associated With Transient Congenital Hyperinsulinism: A Case Report.

Congenital hyperinsulinism (CHI) and neonatal cholestasis (NC) are occasionally observed in neonatal settings; however, the causes have not been identified despite thorough investigation. Surgical evaluation is essential for patients with cholestasis to rule out biliary atresia because timely surgical intervention is critical. Few case reports have described the co-occurrence of CHI and NC. Herein, we report a case of a boy born as one of dichorionic-diamniotic twins via planned cesarean delivery. Apparent ischemic events were not present, with Apgar scores of 8 and 9 at one and five minutes,

Beneath a silent attack: Unraveling lung and brain injuries in a surviving monoamniotic twin.

Pregnancy with monochorionic monoamniotic twins is highly rare and frequently results in complications. Nevertheless, in monochorionic monoamniotic twin pregnancies, there are no reports of lung injury in the surviving fetus after single intrauterine fetal death. To the best of our knowledge, we present the first report of lung and brain injuries in a surviving fetus after single intrauterine fetal death in a monochorionic monoamniotic twin pregnancy. A male child was born with dyspnea. His healthy 39-year-old mother had a monochorionic monoamniotic twin pregnancy. After the cardiac arrest of

Schizencephaly in neonates and clinical importance. Cases report.

BACKGROUND: Schizencephaly is a congenital alteration of the architecture of the cerebral cortex, and its diagnosis in the neonatal period will be determined by prenatal diagnosis or severity of the cases. CLINICAL CASES: We present three neonatal cases with variable clinical expression and electroencephalographic findings during clinical course. Case 1: term neonate, born by cesarean section due to pre-eclampsia and presenting early respiratory distress. On clinical examination, macrocranial was clear, so a cranial tomography (CT) scan was performed showing bilateral open-lip schizencephaly.

Abdominal Aortic Thrombosis in a Preterm Baby With Congenital Diaphragmatic Hernia.

Congenital diaphragmatic hernia (CDH) is a serious congenital anomaly often associated with pulmonary hypoplasia and persistent pulmonary hypertension of the newborn (PPHN). Central vascular access such as umbilical arterial catheters (UACs) is routinely used in neonatal intensive care but is associated with the risk of vascular complications, including thromboembolic events. We present a case of preterm dichorionic diamniotic (DCDA) twins born at 34 weeks of gestation with antenatally diagnosed CDH. The baby developed severe respiratory distress requiring high-frequency oscillatory ventilatio

Tattooing is Mainly Cultural: A Representative Twin Study of Tattooing Determinants.

Tattooing has become increasingly common in recent decades, yet little is known regarding factors that influence tattoo behavior. Sources of influence will be important, for instance in aiding studies of long-term health effects. From the population-based Danish Twin Tattoo Cohort established in 2021, the study included 9,173 randomly selected twins born 1920-2004. Among these were 4,790 (52%) responders to a questionnaire on tattooing and lifestyle factors. There were 55% females, 22% were monozygotic twins, and the median age was 51 years. Shared influence of tattooing over time was assessed

Emergency neurosurgical separation of pygopagus conjoined twins in a triplet pregnancy: illustrative case.

BACKGROUND: Conjoined twins are a rare type of congenital malformation. The point of attachment is the primary factor used to classify conjoined twins; typically, this is front to front, with thoracopagus and omphalopagus twins accounting for about 75% of cases and pygopagus twins only between 6% and 19%. OBSERVATIONS: This is a case report of 46-day-old female triplets born to a 37-year-old para 4 mother. Triplets A and B were conjoined at the lumbosacrococcygeal area dorsally. Triplet C was not conjoined. The conjoined triplets had a low birth weight; hence, it was decided to start nutritio

Dexamethasone dosage and course effects on respiratory outcomes in preterm twins: retrospective cohort study.

This retrospective cohort study evaluated the effects of complete, incomplete, and multiple courses of antenatal corticosteroids (ACS) on respiratory outcomes in twin preterm infants born between 28 + 0 and 36 + 6 weeks of gestation. A total of 1,115 neonates were analyzed. Primary outcomes included respiratory distress syndrome (RDS) and other adverse respiratory conditions. Infants exposed to more than one course of dexamethasone had higher requirements for CPAP support (aRR 1.400, 95% CI 1.098-1.785; P = 0.007), increased lung atelectasis (aRR 2.456, 95% CI 1.052-5.730; P = 0.038), and elev

Timing of Antenatal Corticosteroid Administration and Neonatal Outcomes.

IMPORTANCE: Antenatal corticosteroids (ACS) are accepted to be most effective in reducing prematurity-related neonatal mortality and morbidity when administered 1 to 7 days before birth. However, precise data on the optimal timing of administration are scarce. OBJECTIVE: To investigate the association between ACS administration to birth interval as a continuous variable and neonatal outcomes among preterm neonates. DESIGN, SETTING, AND PARTICIPANTS: This national retrospective cohort study was conducted from 2018 to 2021 at level III neonatal intensive care units participating in the Canadia

Customized growth charts for twin vs singleton pregnancies and their ability to identify small for gestational age-associated risk of adverse perinatal outcome.

BACKGROUND: Twin fetuses are born earlier and have slower growth than singletons. It is uncertain as to what degree this is pathological or a physiological adaptation. OBJECTIVE: We set out to develop a customized chart for twin pregnancy and compare it with the corresponding singleton chart in its ability to assess risk of adverse perinatal outcomes. STUDY DESIGN: The cohort consisted of 8457 twin pregnancies (16,914 fetuses) without information on chorionicity, recorded during routine care in 127 UK hospitals. We performed a mixed-effects linear regression analysis to calculate customized
