Postpartum follow-up and Edinburgh Postnatal Depression Scale scores in prenatally suspected placenta accreta spectrum.

OBJECTIVE: To assess the impact of prenatal suspicion of placenta accreta spectrum (PAS) on attendance of postpartum care and Edinburgh Postnatal Depression Scale (EPDS) scores, with stratification by confirmed PAS based on clinical or pathologic PAS grading. STUDY DESIGN: Matched cohort of singleton pregnancies that delivered 2010-2024. Patients with prenatal suspicion of PAS were matched to patients without this concern by age (within 5 years), number of prior cesareans, term or preterm delivery, and hypertensive disorders of pregnancy. Exclusions were fetal anomalies, stillbirth, neonatal

Detection of hypertension and blood pressure phenotypes using ambulatory blood pressure monitoring in women with past hypertensive disorders of pregnancies.

OBJECTIVE: To evaluate the incidence of hypertension and blood pressure (BP) phenotypes using 24-hour ambulatory BP (24hr-ABP) in women with past pregnancy complications who were recruited into a Women's Heart Clinic (WHC). STUDY DESIGN: We recruited 156 women aged 30-55 years with past hypertensive disorders of pregnancy (HDP) and/or gestational diabetes (GDM) to a multidisciplinary six-month WHC that provided cardiovascular risk management in Melbourne, Australia. Women were referred for 24 hr-ABP monitoring if clinic BP was ≥130/80 mmHg. MAIN OUTCOME MEASURES: Primary outcome was incident

Evaluation of Rectus Abdominis Muscle Using High-Frequency Ultrasound and Shear Wave Elastography to Construct a Risk Prediction Model for Postpartum Low Back Pain.

RATIONALE AND OBJECTIVES: This study aimed to develop a risk prediction model for low back pain (LBP) using high-frequency ultrasound to measure rectus abdominis muscle activity in postpartum women. MATERIALS AND METHODS: This study included 197 pregnant women who delivered at our hospital and underwent routine postpartum follow-up. High-frequency ultrasound was used to assess the inter-rectus distance, thickness, and elasticity of the rectus abdominis muscle. Follow-up data were collected until six months after delivery, including information on LBP and exercise. Statistical analysis was per

Feasibility and impact of a mental health chatbot on postpartum mental health: a randomized controlled trial.

BACKGROUND: Perinatal mood disorders are common yet underdiagnosed and un- or undertreated. Barriers exist to accessing perinatal mental health services, including limited availability, time, and cost. Automated conversational agents (chatbots) can deliver evidence-based cognitive behavioral therapy content through text message-based conversations and reduce depression and anxiety symptoms in select populations. Such digital mental health technologies are poised to overcome barriers to mental health care access but need to be evaluated for efficacy, as well as for preliminary feasibility and a

Patient acceptability, continuation and complication rates with immediate postpartum levonorgestrel intrauterine device insertion at caesarean section and vaginal birth.

BACKGROUND: Immediate postpartum long-acting reversible contraception (LARC) has been shown to reduce unintended pregnancy but uptake of this type of contraception in Australia is low compared to European counterparts. AIMS: To assess self-reported continuation rates, complications and satisfaction in patients having immediate postpartum hormonal intrauterine device (IUD) inserted at caesarean section (CS) or after vaginal birth (VB). MATERIALS AND METHODS: Retrospective cohort study of all patients with immediate postpartum hormonal IUD insertion over three years at a tertiary maternity ser

Timing of Emergency Postpartum Hospital Visits in the Fourth Trimester.

OBJECTIVE: The concept of the "fourth trimester" emphasizes the importance of individualized postpartum follow-up. Women seek care for urgent issues during this critical time period. Our objective was to evaluate trends in presenting complaints and admissions in an emergency setting over the first 42 days following delivery. STUDY DESIGN: Postpartum hospital encounters within 42 days of delivery at our institution from 2015 to 2019 were studied. Demographic information, delivery route, and emergent hospital encounter details were obtained from the electronic medical record. The postpartum enc

Incidence of dehydration and hypernatremia in exclusively breast-fed infants.

OBJECTIVES: To verify in exclusively breast-fed, term infants the incidence of hypernatremic dehydration and identify possible maternal and/or infant factors that interfere with successful breast-feeding. STUDY DESIGN: We prospectively included all healthy breast-fed neonates referred to our Neonatology Unit between October 1999 and March 2000. All neonates with a weight loss > or = 10% of birth weight had a breast-feeding test and a determination of serum sodium, urea, and base excess. Student t test and chi-square test were used for statistical analysis of the data. RESULTS: Of 686 neonate
