Efficacy and safety of EUS-guided radiofrequency ablation in pancreatic neuroendocrine tumors: A systematic review and meta-analysis.

EUS-guided radiofrequency ablation (EUS-RFA) has gained recognition in the last decade as a preferred alternative method for pancreatic neuroendocrine tumors (PNETS) where surgical resection is contraindicated. However, clinical outcomes of EUS-RFA for PNETS have not been well studied due to variability in procedure techniques and lack of data from good-quality studies. In this meta-analysis, we aim to study the pooled clinical outcomes of EUS-RFA in patients with PNETS. We searched multiple electronic databases and conference proceedings from inception through Jan 2024. The clinical outcomes

Ciprofloxacin prophylaxis in allogeneic haematopoietic cell transplantation reduces bacteraemia but increases antimicrobial resistance without improving patient outcomes.

BACKGROUND: Fluoroquinolone prophylaxis during allogeneic haematopoietic cell transplantation (allo-HCT) is debated due to concerns regarding toxicity and antimicrobial resistance (AMR). METHODS: We compared clinical and microbiological outcomes of all adults undergoing their first allo-HCT for haematological malignancy at two large UK transplantation centres between January 2020 and December 2022. The centres employed differing prophylactic strategies: Site 1 administered ciprofloxacin prophylaxis; Site 2 did not. RESULTS: Among 343 eligible patients, primary allo-HCT indications were acute

Recombinant TsP53 mitigates inflammation and blood-brain barrier disruption in CLP-induced septic mice.

BACKGROUND: Sepsis-associated encephalopathy (SAE), a neurological complication of sepsis without direct CNS infection, currently lacks established pharmacological therapy. Key pathological features include excessive microglial activation and blood-brain barrier (BBB) disruption. Our prior work showed that recombinant Trichinella spiralis 53-kDa glycoprotein (rTsP53) modulates intestinal endothelial tight junctions in septic mice by downregulating inflammation. METHODS: We analyzed inflammatory factor levels and performed bioinformatics analysis on cerebrospinal fluid (CSF) from SAE patients.

[Pressure ulcer following compression otoplasty garment: A rare but severe case report].

INTRODUCTION AND IMPORTANCE: Otoplasty is a common procedure in plastic surgery. A compression garment is frequently recommended by surgeons after the procedure; however, it is not free from risk of serious complications. We report the case of a pressure ulcer following otoplasty, associated with the use of an inappropriate compression garment. CASE PRESENTATION: A 7-year old girl was evaluated for correction of prominent ears. The procedure was uneventful. On the day after surgery, the compression garment that had been prescribed during the preoperative consultation could not be checked. She

Prediction of venous thromboembolism after metabolic and bariatric surgery using machine learning approach: a MBSAQIP study.

BACKGROUND: Several studies have developed predictive models for venous thromboembolism (VTE) after metabolic and bariatric surgery (MBS), but none have incorporated complications that increase VTE risk. This study aimed to evaluate the risk factors for VTE following MBS using machine learning (ML) algorithms, using models that either excluded or included time-aware postoperative complication variables-these were engineered to include only complications that occurred before the VTE event. METHODS: We used the MBSAQIP database from 2020 to 2023. XGBoost, random forest classifiers, support vect

Pancreaticoduodenectomy After Preoperative Biliary Drainage: A Single-Center Cohort Study of Piperacillin/Tazobactam Plus Fluconazole Prophylaxis and Infectious Outcomes.

Background Pancreaticoduodenectomy (PD) is associated with high postoperative morbidity, particularly infectious complications. In patients undergoing preoperative biliary drainage (PBD), bile contamination is common and may lead to a mismatch between standard cephalosporin-based prophylaxis and biliary pathogens. Methods We conducted a retrospective single-center cohort study including 65 patients undergoing PD between December 2021 and May 2025. Of these, 42/65 (64.6%) underwent PBD. Patients received perioperative antibiotic prophylaxis with cefazolin or piperacillin/tazobactam plus flucona

Effects of sleeve gastrectomy on rivaroxaban pharmacokinetics, efficacy, and safety.

BACKGROUND: Thromboembolic events are potentially serious complications in patients undergoing bariatric surgery. Several studies have investigated the use of rivaroxaban in patients undergoing bariatric surgery. Further evidence is required to determine whether postsurgical anatomical and physiological changes affect the pharmacokinetics of rivaroxaban. This study aimed to investigate the pharmacokinetic, efficacy, and safety of rivaroxaban in patients who underwent sleeve gastrectomy surgery. METHODS: Included patients who admitted for sleeve gastrectomy surgery and were scheduled to receiv

Is Low-Dose Aspirin the Safest Prophylaxis for Venous Thromboembolism Following Primary Total Hip Arthroplasty?

BACKGROUND: Venous thromboembolism (VTE) remains a serious complication following primary total hip arthroplasty (THA), prompting routine use of perioperative chemoprophylaxis. In 2022, the International Consensus Meeting (ICM) endorsed low-dose aspirin (LDA) as the preferred VTE prophylactic agent for both hip and knee arthroplasty patients. Prior THA studies lack stratification by VTE risk and direct comparisons between aspirin monotherapy, combination regimens, and alternative anticoagulants. This study aimed to evaluate the safety and efficacy of LDA monotherapy compared to other anticoagu

Reduction of hemorrhagic complications after non-focal renal biopsy with pre-procedure desmopressin administration.

PURPOSE: Ultrasound-guided non-focal renal biopsy (NFRB), which is performed to assess systemic renal processes, is associated with a 1.5%-14% bleeding risk. The goal of this study is to evaluate the safety and efficacy of 1-deamino-8-D-arginine vasopressin (DDAVP, or desmopressin) in mitigating bleeding risk when administered prior to NFRB. METHODS: This retrospective observational study includes 158 patients who underwent NFRB between March 2022 and June 2025 at the study hospital before and after the implementation of a DDAVP administration protocol. Beginning in July 2023, every patient a

Hyperbranched polycation-based hierarchical assembles as vaccine carrier for cascaded antigen delivery and enhanced immunity response.

Tumor vaccines have emerged as a promising strategy in cancer immunotherapy, and their therapeutic efficacy hinges on successive activation of multi-step cascade immune responses, including the uptake of antigens by dendritic cells, the presentation of antigens by mature DCs to T cells and the activation of T cells, and the recognition and killing of tumor cells by effector T cells. However, distinct or even conflicting characteristics (e.g., charge, size, stability) of vaccine delivery system are required at different stages to overcome multi-stage biological barriers remain a pivotal challen

Cost efficiency of topical tranexamic acid in reduction mammaplasty surgeries.

BACKGROUND AND AIMS: Perioperatively used, topically administered tranexamic acid (TXA) has attracted increasing interest in plastic surgery, and several studies have compared the effectiveness and safety of topical and intravenous (IV) administration. Topical TXA has been reported to have an equal or superior effect in reducing postoperative bleeding and transfusion requirements, with no increase in reported adverse events. Although prophylactic topical TXA is believed to result in cost savings, cost-effectiveness analyses concerning the use of TXA in breast surgery have not yet been reported

Routine contrast-enhanced CT screening identifies asymptomatic pulmonary embolism after minimally invasive esophagectomy: incidence and associated factors.

BACKGROUND: Esophagectomy is associated with a higher incidence of venous thromboembolism, including pulmonary embolism (PE), than other gastrointestinal cancer surgeries. However, the true incidence of asymptomatic PE remains unclear. This study aimed to determine the incidence and associated factors of PE using routine contrast-enhanced computed tomography (CECT) after minimally invasive esophagectomy (MIE). METHODS: A total of 134 consecutive patients who underwent MIE between May 2022 and December 2023 under a standardized postoperative CECT protocol were retrospectively analyzed. Routine

Association of a Fixed 1 mg Dose of Granisetron for Preventing Postoperative Nausea and Vomiting (PONV) in Patients Undergoing Laparoscopic Colorectal Surgery With Multiple PONV Risk Factors: A Retrospective Observational Study.

Introduction Granisetron is a selective 5-hydroxytryptamine type 3 (5-HT3) receptor antagonist approved in Japan for the prophylaxis of postoperative nausea and vomiting (PONV). According to the Japanese package insert, a fixed intravenous dose of 1 mg is recommended; however, evidence for its effectiveness in patients undergoing laparoscopic colorectal surgery in a clinical context characterized by multiple established PONV risk factors remains limited. We evaluated the association between prophylactic granisetron 1 mg and PONV-related outcomes in this setting. Methods This single-center retr

Optimal surgical management of patients with severely reduced left ventricular ejection fraction undergoing coronary artery bypass grafting.

Patients with severely reduced left ventricular ejection fraction (LVEF ≤ 30%) undergoing coronary artery bypass grafting (CABG) face heightened peri-operative risk. Prophylactic mechanical circulatory support with the surgical Impella 5.5 pump may enhance hemodynamic stability, yet its clinical value remains uncertain. Here, we report our institutional experience in managing patients with reduced LVEF undergoing CABG. We retrospectively reviewed adults who underwent isolated elective or urgent CABG at a single center between January 2020 and July 2024. Patients supported with preoperative Imp

A Pathology-Responsive Hydrogel with On-Demand Ion Release for Microenvironment Remodeling and Diabetic Wound Regeneration.

The healing of diabetic wounds is often impeded by severe bacterial infections, persistent inflammation, excessive accumulation of reactive oxygen species (ROS), and abnormal matrix degradation. Although conventional free silver nanoparticles (AgNPs) possess broad-spectrum antimicrobial activity, their inherent tendency to aggregate and inactivate, as well as its high cytotoxicity limits their clinical application. Moreover, a single bactericidal strategy is insufficient to completely reverse the complex pathogenic microenvironment of the wound. To address these challenges, this study develope

Establishing the clinical utility of integrated quasistatic acoustic tweezing thromboelastometry for blood coagulation analysis.

BACKGROUND: Rapid and accurate coagulation analysis is essential for managing critical care and surgical patients, patients with coagulation disorders, and patients with cardiovascular disease on anticoagulant therapy. Traditional assays often require large sample volumes, posing iatrogenic risks in vulnerable populations, including pediatric and elderly patients. Integrated quasistatic acoustic tweezing thromboelastometry (i-QATT) offers a comprehensive, noncontact assessment of coagulation using a 4- to 6-μL drop of blood that minimizes diagnostic errors associated with sample-container surf
