Predictors of Prolonged Hospitalization in Pyogenic Tenosynovitis of the Hand: A Retrospective Cohort Study Focused on Length of Stay.

Background and Objectives: Septic tenosynovitis of the hand remains a surgical emergency associated with significant morbidity and healthcare resource utilization. While prior studies have focused primarily on diagnostic features, microbiology, and functional outcomes, factors influencing prolonged hospitalization remain insufficiently characterized. Length of stay (LOS) represents a pragmatic, patient- and system-centered outcome that may reflect disease burden and treatment complexity. To identify clinical, demographic, and treatment-related factors associated with prolonged hospitalization

Free Flap Reconstruction of Xylazine-Associated Wounds: A Retrospective Review.

BACKGROUND: Xylazine is a veterinary sedative that is added to illicit fentanyl to enhance its effects. Xylazine-associated wounds differ from those found in patients who inject other drugs. They are larger and frequently involve deeper structures, such as bone. The outcomes of reconstruction using free tissue transfer are not well understood. METHODS: All consecutive free flap reconstructions of xylazine-associated wounds at a tertiary care center in the northeast US between January 2021 and December 2024 were retrospectively reviewed. Data were stored in a HIPAA-compliant REDCap database.

Health care costs of major upper-extremity amputations in Ontario: a retrospective matched-cohort analysis with considerations for transplantation.

BACKGROUND: Major upper-extremity amputation, defined as loss above the wrist, profoundly affects all aspects of life and imposes substantial health care burdens. Standard care involves prosthetics, which have limitations, while vascularized composite allotransplantation (VCA) offers improved functionality but raises cost-effectiveness concerns. We sought to evaluate health care use and cost of major upper-extremity amputation in Ontario. METHODS: We conducted a 20-year retrospective matched-cohort study using administrative health care data from Ontario. We matched patients with major upper-

Lymphatic Recovery and Clinical Implications in Hand Allotransplantation: Insights From Indocyanine Green Lymphangiography.

Introduction Hand vascularized composite allotransplantation (VCA) is currently considered a promising reconstructive approach for selected hand amputees. The major limitation of hand VCA is its risk of rejection, which influences the long-term survival of the allograft. The lymphatic system plays a key role in inflammation, antigen presentation, and immune regulation, yet lymphatic regeneration following upper limb VCA is poorly understood. The correlation between donor-recipient lymphatic reconnection and the allograft recovery and rejection is not well understood. This study was designed to

The United Kingdom Hand and Upper Limb Transplantation Service: A report on the first twelve years of a single-center, single-protocol clinical cohort.

BACKGROUND: Following the world's first case (1998), proof of concept was established for hand and upper limb transplantation (HAUL-T). By 2012, there was sufficient evidence for the United Kingdom HAUL-T service to be established on a standard of care basis. A commitment was made to report in detail on this prospective, single-center, single-protocol series. We provide the first comprehensive report on the procedures, surgical, functional, immunological and psychosocial outcomes of the United Kingdom (UK) HAUL-T service. METHODS: Potentially suitable patients undergo sequential psychological

Reducing Health Care's Carbon Footprint: The Environmental Impact of Telephone Clinic Visits for Hand Surgery.

PURPOSE: The health care sector contributes substantially to global greenhouse gas emissions. Telehealth presents an opportunity for reducing health care-associated emissions and patient transportation burden. This study analyzed the environmental and clinical outcomes of telephone encounters conducted at a Veterans Affairs Medical Center Hand Clinic between June 2022 and June 2023. We hypothesized that telehealth would reduce carbon emissions while maintaining effective patient care. METHODS: We conducted a retrospective review of 416 telephone encounters involving 295 patients. Patient demo

Pediatric Upper Extremity Firearm-related Injuries: A Level I Pediatric Trauma Center Experience.

INTRODUCTION: Firearm injuries have become increasingly more common in the pediatric population; however, there is a paucity of literature examining the management of these pediatric firearm-related injuries (FRI) specifically as they affect the upper extremity. This study identifies demographic and environmental risk factors in pediatric upper extremity FRIs and evaluates the severity of injury, concomitant injuries, and rates of surgical intervention in pediatric patients treated at a Level I pediatric trauma center over 20 years. METHODS: We completed a retrospective analysis on 540 patien

Combined Application of Negative Pressure Wound Therapy and Dermal Substitute for Coverage of a Complex Cutaneous Defect: A Case Report.

Abrasion burns are a type of skin injury commonly associated with friction trauma, such as those caused by automobile accidents. These wounds, although superficial in appearance, can involve large areas of the integument and require surgical treatment to achieve adequate functional and aesthetic healing. We present the case of a female patient who suffered an extensive abrasion burn on her upper limb following a high-energy vehicle accident. The initial approach included surgical cleansing and placement of negative pressure therapy (vacuum-assisted closure system (VAC®)) to prepare the wound b

Advancing vascularized composite allotransplantation: essential factors for upper extremity transplant program development.

Vascularized Composite Allotransplantation (VCA) offers a unique option to restore form and function after limb loss or facial trauma that cannot be satisfactorily accomplished through traditional prosthetics or reconstructions. Establishing a successful Upper Extremity Transplantation (UET) program requires strong leadership and a structured surgical team, and extensive interdisciplinary collaboration. We conducted a qualitative study among 12 health care professionals and patients. Informed consent was obtained per protocol, and semi-structured interviews were conducted online and recorded.

Management of Giant Type II Arteriovenous Malformation of the Neck With Heart Failure in a Pediatric Patient With Coil and NLE Embolization: A Case Report.

PURPOSE: Although arteriovenous malformations (AVMs) are typically benign lesions, massive ones can lead to severe complications, including heart failure. Notably, there is no standardized treatment for AVMs. Moreover, management of AVMs can be challenging when these lesions involve vital organ systems, such as the head and neck. In this report, we describe embolization of a massive cervical AVM in a pediatric patient with heart failure. CASE REPORT: A 10-year-old boy presented with severe heart failure secondary to a massive AVM in the neck (right side). Despite 3 unsuccessful arterial embol

Reconstruction techniques for upper extremity crush injuries with massive tissue loss and open fractures: a prospective study.

OBJECTIVE: Crush injuries and open fractures are often accompanied by extensive tissue loss, rendering clinical and surgical management quite challenging, particularly in the upper extremities. The primary goal in these cases is to obtain a functional and cosmetically acceptable limb. However, the management of complex crush injuries (involving extensive tissue loss and open fractures) is associated with a variety of complications, ranging from infection to amputation. In this study, we aimed to analyze the clinical outcomes of reconstruction for managing complex upper extremity crush injuries

Extended Paraumbilical Perforator Flap Pedicled With the Inferior Epigastric Artery for Coverage of Type III Circumferential Electrical Burns of the Wrist.

Type III electrical burns on the wrist are characterized by circumferential wounds, three dimensional with sandwich-like necrosis, and progressive blood circulation disturbances. Limb salvage is challenging, and success in meeting this challenge depends on vascular reconstruction and wound coverage. This article is intended for the following purposes: to investigate the principles of wound debridement, the management of involved blood vessels, and the clinical effects of the extended paraumbilical perforator flap pedicled with the inferior epigastric artery for coverage of type III circumferen

Are Vascularized Fibula Autografts a Long-lasting Reconstruction After Intercalary Resection of the Humerus for Primary Bone Tumors?

BACKGROUND: A vascularized fibula graft (VFG) is the vascular autograft most frequently used to restore large segmental long bone defects, particularly in the upper limb. Because the use of a vascularized fibula involves an operation in an uninvolved extremity with potential morbidity, it is important to document that this type of reconstruction is successful in restoring function to the humerus. However, the long-term results of VFG after intercalary resection of the humeral diaphysis for bone tumors are still unknown. QUESTIONS/PURPOSES: (1) What was the complication rate of reconstruction?
