Absence of a prolonged macrophage and B cell response inhibits heart regeneration in the Mexican cavefish.

A balanced immune response after cardiac injury is crucial to successful heart regeneration, but knowledge of what distinguishes a regenerative from a scarring response is still limited. The Mexican cavefish Astyanax mexicanus provides a unique comparative model to study heart regeneration and scarring within a single species. Astyanax surface-dwelling fish are capable of heart regeneration, whereas, like the human heart, their cave-dwelling Pachón counterparts form a permanent scar. Using single-cell transcriptomics and immune perturbations, we identify two distinct and largely sequential imm

Preoperative natremia and AKI after cardiac surgery: an international multicenter prospective cohort study.

BACKGROUND AND GOAL OF STUDY: Whether preoperative natremia levels may play a role in the development of acute kidney injury (AKI) after cardiac surgery is uncertain. We conducted a cohort study to evaluate the influence of preoperative natremia on AKI in high-risk patients undergoing cardiac surgery. MATERIALS AND METHODS: This multicenter prospective cohort study involved 14 British and Spanish hospitals, each under local ethics committee approval. The study population comprised all consecutive patients aged 18 years or older, with a Cleveland Clinic score ≥ 4, undergoing cardiac surgery fr

Integrated Bedside Hemodynamic Assessment and Monitoring of Venous Congestion With Venous Excess Ultrasound (VExUS): A Narrative Review.

This review evaluates the venous excess ultrasound (VExUS) score as a bedside tool for quantifying systemic venous congestion and predicting congestive end-organ injury, particularly acute kidney injury (AKI). By assessing flow patterns in the hepatic, portal, and renal veins alongside inferior vena cava (IVC) size, VExUS enables serial monitoring of congestion trajectories. While highly prognostic in heart failure, its predictive performance varies in more heterogeneous populations. Ultimately, integrating VExUS with cardiac output indices may help clinicians identify specific hemodynamic phe

The impact of an adaptive patient-centered web application on medication optimization in patients with HFrEF: Rationale and design of the SmartHF trial.

INTRODUCTION: Heart failure (HF) remains a significant public health burden, with frequent readmissions often driven by a lack of guideline-directed medical therapy (GDMT) optimization. While clinical decision support (CDS) can improve healthcare processes, patient-centered tools that empower individuals to participate in their own care are increasingly needed. We developed SmartHF, an adaptive web application that provides patients with tailored HFrEF GDMT recommendations based on their clinical data. METHODS: This 12-week, multicenter randomized controlled trial enrolled adults with HFrEF (

Evaluating HEART: Student Engagement and Health System Impact in a Person-Centered Learning Health System Initiative.

BACKGROUND: Healthcare Education and Research Training (HEART) is a cocurricular, engaged learning initiative that supported undergraduate students in acquiring and applying healthcare research skills. The project was guided by a learning health system (LHS)-aligned model focused on the collection, synthesis, and rapid translation of patient-reported experience data into actionable recommendations. In the 2023/2024 academic year, a cohort of 28 students completed the pilot project to report LHS-based recommendations to healthcare directors. This paper reports the qualitative evaluation conduct

Percutaneous left atrial appendage occlusion versus oral anticoagulation in atrial fibrillation: A pragmatic meta-analysis of randomized trials within the DOAC era.

BACKGROUND: Percutaneous left atrial appendage occlusion (LAAO) is an established alternative to oral anticoagulation (OAC). However, its safety and efficacy boundaries remain incompletely defined in dedicated comparisons against direct oral anticoagulants (DOACs). OBJECTIVE: This study applied a pragmatic framework to evaluate the comparative efficacy and safety of LAAO versus OAC in a DOAC-predominant setting. METHODS: PubMed, Embase, and Cochrane Library were searched for randomized controlled trials comparing LAAO with OAC, restricted to a DOAC-predominant comparator. Risk ratios (RR) wi

Living with angina: rethinking angina management in ANOCA through lived experience.

Angina with non-obstructive coronary arteries (ANOCA) is an important cause of chronic angina and reduced quality of life. Despite advances in diagnostic testing and growing recognition of coronary vasomotor disorders, many of those living with ANOCA continue to experience ongoing symptoms despite treatment. The existing literature largely focuses on diagnostic methods and pharmacological management, while comparatively little attention has been paid to the lived experience of symptom control. The objective of this perspective paper, produced with the co-founders of the International Heart Spa

Development and external validation of a risk prediction tool for recurrent wheeze attacks in preschool children in England.

INTRODUCTION: Almost half of all children experience wheeze before their sixth birthday, with a subset experiencing recurrent attacks that increase healthcare burden. Early identification of children at high risk could enable timely interventions, but diagnostic tools are limited. The aim of the present study was to develop and validate a prognostic model predicting the risk of recurrent wheeze attacks (primary outcome) and hospitalisations (secondary outcome) within 12 months of an initial preschool acute wheeze presentation. METHODS: We developed a multivariable logistic regression model us

Evaluation and Classification of Atrial Arrhythmias from Signatures in Electrographic Flow Metrics.

In patients undergoing atrial fibrillation (AF) ablation, it can be difficult at times to distinguish between AF, micro-re-entrant atrial flutters (AFLs), and focal atrial tachycardias (ATs). Understanding the mechanism of arrhythmogenesis can guide ablation lesion sets. Electrographic flow (EGF) mapping displays global atrial wavefront propagations in near-real time. Patterns of EGF can show both regions of stable flow direction over time and disorganized electrical wavefronts. In so doing, it can differentiate mechanisms of arrhythmogenesis. The objective of this study was to evaluate the EG

Deep Learning-Based 3D CT Body Composition Is Associated With Mortality in Patients Undergoing Transcatheter Aortic Valve Replacement.

BACKGROUND: Recent advances in artificial intelligence enable fully automated 3-dimensional computed tomography (CT)-based body composition quantification. We investigated whether automated CT-derived thoraco-abdominopelvic body composition measures are associated with mortality after transcatheter aortic valve replacement. METHODS: In this retrospective multicenter study, consecutive patients undergoing transcatheter aortic valve replacement with preprocedural CT imaging were included between 2014 and 2023. A deep learning-based pipeline performed automated 3-dimensional segmentation of thor

Sn/Cl Co-Doped ZnO Cathode Interfacial Layer for High-Performance Organic Photodetectors.

Organic photodetectors (OPDs) hold significant promise for applications spanning medical monitoring, bioimaging and night vision due to their unique advantages, such as flexibility, light weight, and potential for large-area fabrication. Nevertheless, the dark current density (Jd) of OPDs exhibits a pronounced increase under applied reverse bias, which consequently degrades the specific detectivity (D*). To tackle this challenge, this work proposes an interface engineering strategy based on Sn/Cl co-doped ZnO (SCZO). The doping effectively reduces oxygen defects in ZnO, causing the valence ban

Astrocytic glutamate transporter loss drives chemoreflex hyperactivity and cardiorespiratory dysfunction in a heart failure model.

Heart failure (HF) is a leading cause of morbidity and mortality worldwide. Breathing instability and exaggerated chemoreflex activation contribute to adverse HF outcomes, yet the central mechanisms linking respiratory control to cardiac dysfunction remain poorly defined. The retrotrapezoid nucleus (RTN) is a key brainstem chemosensory region regulating ventilatory and autonomic output, but whether glial dysfunction within this circuit contributes to HF pathophysiology is unknown. We hypothesized that loss of astrocytic glutamate transporter 1 (GLT-1) in the RTN disrupts glutamate homeostasis,

Testosterone as part of hormone replacement therapy in menopausal women - what do we know so far? A Narrative Review.

Testosterone, widely known as the male hormone, is also produced naturally in women and plays an important role throughout their lifespan. There has been a growing use of testosterone for a range of symptoms in menopausal women, including low sexual desire, low mood, fatigue, poor memory, and heart and bone health. This review brings together the current evidence and guidelines on testosterone as part of hormone replacement therapy (HRT) in menopausal women, including its impact on reproductive and non-reproductive tissues; when it is indicated to be prescribed and in what forms; safety and ga
