[Syndrome shift following resection of a pathologic intestine and construction of an anus praeter: a comparison of ulcerative colitis, Crohn disease, rectal cancer and familial polyposis].

Postoperative changes of symptoms were studied in 347 patients with Crohn's disease, ulcerative colitis, carcinoma of the rectum, and familial polyposis. A semi-standardised psychosocial questionnaire and a list of complaints, according to v. Zerssen, were used as tools of investigation. The incidence of postoperative disease in patients with Crohn's disease and ulcerative colitis, within the first year from surgery, was higher with significance than that recorded from carcinoma and polyposis patients within the same period of time. Problems relating to identification of syndrome shift are dis

Innovative surgical procedures and stoma care in the future.

Innovative surgical procedures require particular attention to the technical, educational, and emotional issues. Patients need thorough education to understand surgical options, complications, and expected outcomes. Because of the new aspect of these procedures, patients and significant others may feel quite isolated from others. Recognizing behaviors which may indicate this problem and seeking opportunities to provide emotional support and role modeling from other patients are most therapeutic. The quest for continence provides the patient with alternatives to be cured of disease and avoid th

Squamous cell carcinoma occurring in a skin-grafted ileostomy stoma. Report of a case.

A squamous cell carcinoma arose in a skin-grafted ileostomy stoma 26 years after proctocolectomy for Crohn's colitis. The patient presented with peristomal ulceration and pain of several months' duration. Attention is drawn to this late complication of ileostomy which requires wide local excision and relocation of the stoma.

Diet for cystic fibrosis. Nutritional requirements and prescriptions.

Diet plays no direct role in neutralizing the effects of the basic defect in cystic fibrosis, but it can prevent some of the acquired damage caused by complications. Adequate caloric intake provides the energy needed for exercise and cell metabolism, a strong diaphragm, normal cellular immunity, and a positive psychological outlook. Clinical management is aimed at achieving good nutrition. Patient education about nutrition and use of dietary supplements should be started immediately upon diagnosis. Careful clinical examinations and regular follow-up are necessary to detect complications that w

Transsacral approach to the rectum.

While the proximal or upper third of the rectum is readily accessible through the lower abdomen, lesions of the distal rectum are difficult to reach both from abdomen and the perineum. The transsacral approach (Kraske) gives excellent exposure of the mid and lower rectum. The experience using this approach in sixteen cases for various indications which include local excision of benign tumours, segmental excision, repair of recto-vesical fistula, perineal excision of rectum and abdominal excision of rectal carcinoma and low colo-rectal anastomosis with preservation of ano rectal sphincters is r

Mineral oil embolization and lipid pneumonia in an infant treated for Hirschsprung's disease.

A 5-month-old infant girl with Hirschsprung's disease died 1 month after colostomy as a result of mineral oil embolism and lipid pneumonia. She had received multiple mineral oil enemas and irrigations as treatment for impacted stools. Mineral oil peritonitis was present on the surface of the bowel adjacent to the stoma and covered large portions of the surface of the liver and spleen. There was 600 cc of slightly cloudy and bloody peritoneal fluid. The peritoneal exudate contained bacteria that were associated with only a minimal inflammatory reaction. This case calls attention to a previously

Ileostomy in situ.

The technique of tube ileostomy has received little attention. It offers the advantage of complete fecal diversion without the need for subsequent operative closure. We emphasize the value of this technique and suggest an easier method for inserting the ileostomy tube.

Evolving trends in enteral alimentation.

Nutrition has become a cornerstone of surgical patient care. With better understanding of metabolic and compositional requirements, great advances have been made in the area of total parenteral nutrition. Recent attention to full utilization of alimentary tract function has prompted a resurgence of interest in dietary formulas and methods of delivery. Three new approaches to the alimentary tract provide better access for feeding. Needle catheter jejunostomy allows early alimentary tract utilization following operations, while percutaneous endoscopic gastrostomy and jejunostomy provide long-ter

Complications of posterior sagittal anorectoplasty.

From 1982 to 1985, 23 patients underwent posterior sagittal anorectoplasty procedures: 12 as primary treatment for congenital anorectal malformations, 9 for treatment of fecal incontinence following a prior pull-through procedure, and 2 for treatment of fecal incontinence following trauma. Six patients (26%) developed seven complications specifically related to the procedure. One patient with a cloacal anomaly had partial dehiscence of the sacroperineal incision following total reconstruction. This resulted in retraction of vaginal and anal openings, which, however, have remained separate and

Anal sphincter function after surgery for high imperforate anus--a long term follow-up investigation.

37 children were treated for high imperforate anus during the years 1944-1965. Eighteen of these 37 participated in a follow-up examination at the age of 18-35 years. The examination consisted of an anorectal manometry, EMG of the external sphincter, a digital anal examination, and an interview. Special attention was paid to anal continence. Fourteen patients did not admit any major anal problems, and eight of them had a good continence. Anorectal manometry showed an absence of internal sphincter function in all patients. During voluntary squeeze the continent patients increased anal pressure

The ParkS S ileal pouch and anal anastomosis after colectomy and mucosal proctectomy.

Attention to detail is crucial to the success of the operation described. Surgeons contemplating performing it should first be experts in pelvic surgery and are advised to personally observe and participate in the procedure performed by surgeons currently experienced in this technique.

Distal ileostomy drip feedings in neonates with short bowel syndrome.

When a newborn infant presents with high intestinal atresia, the proximal segment of the bowel is usually grossly distended and atonic. The anastomosis of this segment to the smaller and unused distal segment will usually result in little or no propulsion of contents distally. Many techniques have been employed to correct this problem. A common surgical approach is immediate end-to-end anastomosis, followed by parenteral alimentation until return of normal function. This can take many weeks, and requires special attention to fluid loss and complications associated with parenteral alimentation.

Esophagogastrostomy using the EEA stapling instrument.

A technique of esophagogastric anastomosis using the end-to-end anastamotic (EEA) stapler following esophagectomy is described in detail. Twenty-two patients underwent esophagectomy for carcinoma at various levels. There were no anastomotic leaks and no operative mortalities. Complications included wound infection (3), pneumonia (1), and late stricture (2). The strictures resulted from use of the small (25-mm) cartridge and responded to dilatation. Technical details of the technique include complete division of the esophagus before inserting the stapler, use of the pursestring instrument, use

Nutritional considerations in the patient with disabling brain disease.

Increased nutritional requirements are now recognized as the typical sequellae of head injury. Whether similar nutritional demands routinely accompany nontraumatic, disabling brain diseases is uncertain. Experience with patients suffering from head injury indicates that clinical criteria for differentiating the severity of neurological impairments categorize rather poorly the levels of nutritional need. Based on available data, "nutritional risk" can only be assigned to broad patient categories. Head-injured patients seem to be at nutritional risk and, by virtue of increased caloric and protei

Esophageal atresia.

Esophageal anomalies present a challenge to the pediatric surgeon and demand close attention and care from the nursing staff as well as the surgical staff. We have encountered a survival rate of 96 per cent for all infants undergoing operation for esophageal atresia. An emergency gastrostomy is recommended as an immediate procedure when the diagnosis is established. In many instances, it can be performed with local anesthesia if the infant is particularly small or has developed aspiration pneumonia. The definitive operation is delayed until the infant is in optimum condition to permit a genera

Barium peritonitis.

Perforation of the gastrointestinal tract during the course of barium contrast examination represents a lethal circumstance if untreated. The reported mortality of barium peritonitis in collected series is approximately 50 per cent. The authors' personal experiences with seven cases of barium peritonitis are presented for review and analysis. Aggressive fluid resuscitation, broad-spectrum antibiotics, early operative intervention, and lavage are important factors in increasing patient survival. Depending upon the operative findings, appropriate surgical procedures may include closure, resectio

The important "impostors" in the differential diagnosis of inflammatory bowel disease.

When evaluating patients suspected to have inflammatory bowel disease, we generally focus on the important differential features which distinguish ulcerative colitis from Crohn's disease. A distinction between them is possible in 80-90% of cases when accepted clinical, radiographic, and pathologic criteria are applied. The remaining idiopathic cases are usually classified as colitis, indeterminate type. In this presentation, major attention will also be directed to a variety of other disorders which may masquerade as idiopathic inflammatory bowel disease. The pathologic features to be describe

Penetrating colon trauma.

Of 56 patients with penetrating colon injuries over 6 years, 15% underwent exteriorized repair, 21% received a colostomy, and 64% were treated with primary repair. Of the eight exteriorized repairs, four required conversion to colostomies. In the entire group of 12 colostomies, there were two deaths, four abscesses, and one empyema. Thirty-six patients (64%) underwent primary repair. In this group there was one superficial wound infection, one empyema, but no intra-abdominal abscesses. Eighty per cent had associated injuries. All laparotomy incisions in the primary repair group except two were
