Stapled esophagogastric anastomosis.

In a consecutive, nonrandomized series of 92 patients, esophagogastric anastomosis was performed with a suture technique in 32 patients and with a stapler in 60 patients. Only one anastomotic leak occurred in each group. Stricture later formed in three sutured anastomoses and in eight of the stapled anastomoses. A review of published reports shows anastomotic leak in 3.5 percent of the patients with stapled anastomoses. In patients with sutured anastomoses, leak occurred in approximately 10 percent. The improved results with the stapler do not alter the necessity for meticulous attention to te

Urological trauma in the Pacific Northwest: etiology, distribution, management and outcome.

A computer-assisted review identified 184 patients with genitourinary tract injuries among 5,400 hospitalized for trauma. Particular attention was directed to the controversial groups of patients with blunt renal and posterior urethral injuries. Management of renal injuries was based on clinical criteria. Subsequent renal exploration was necessary in only 1 of 115 patients with renal contusions, or simple or deep lacerations who underwent initial expectant management. Followup was available in all patients with severe renal injuries and in 53 per cent with renal contusions or simple laceration

Early local complications following colostomy closure in cancer patients.

Early surgical complications following colostomy closure in 65 cancer patients operated on at the Istituto Nazionale Tumori of Milan were evaluated retrospectively. The overall complication rate was 24.6 per cent, including infections (13.8 per cent), fistulas (6.1 per cent), wound dehiscence (3.0 per cent), and distal stenosis (1.5 per cent). Type and rate of complications were analyzed to find a correlation with type, site, and location of colostomy, technique of closure, presence or absence of drains, or time interval between construction and closure of colostomy. No statistically significa

Transverse colon volvulus: diagnosis and treatment.

Although considered rare, transverse colon volvulus (TCV) may actually comprise as many as 10% of all cases of colon volvulus. Correct identification clinically is necessary in order to reduce the high mortality. Unlike the treatment of sigmoid volvulus, conservative treatment of TCV is thought to be inadequate. Furthermore, simple proximal colostomy may lead to bowel necrosis. Bowel resection, rather than detorsion procedures, is advocated. The barium or diatrizoate enema examination can readily differentiate TCV from sigmoid and cecal volvulus if one pays careful attention to detail.

Complications in surgery of the colon and rectum.

Postoperative complications after surgery of the colon and rectum are related most often to sepsis, anastomotic disruption, hemorrhage, urinary tract injury, bowel stoma, and sexual dysfunction. Improved safety in colorectal surgery requires comprehensive preparation of the patient with attention to associated diseases, clear surgical judgement, excellence in technical skill, and aggressive postoperative management of any complications that may develop.

[Preoperative evaluation of the circulatory system in patients with morbid obesity].

Results of the assessment of the contractive function of the myocardium, external ventilation and arterial blood oxygenation in patients with morbid obesity of the III-IV stages are presented. The main factor responsible for the state of the myocardium function of the left ventricle is shown to be the degree of blood arterialization. Patients with reduced functional reserves of the myocardium should be given special attention in performing general anesthesia and in the nearest postoperative period.

[Initial diagnosis of anorectal malformations].

An essential prerequisite for the correct surgical approach of the congenital anorectal anomalies is a precise diagnostic work-up demonstrating the level of the atresia and the nature of possibly associated anomalies. To be acquainted with basic embryology allows a better understanding of this pathology. In this study the value of clinical and radiological parameters has been analysed in sixty cases. An accurate diagnosis was established in one half of the patients only by clinical examination. In the other half, the X-ray with the child inverted according to Wangensteen and Rice proved to be

Complications of emergency stoma formation.

Emergency stoma formation in the high-risk patient is a morbid procedure. In a series of 51 stomas in 49 nonelective operative procedures, a morbidity of more than 50% was retrospectively identified. Nine (18%) of 49 patients died, and four (8%) of these patients died of complications directly related to stomal failure or formation. The unplanned nature of the operation and technical difficulties during formation contribute to this overall result. Attention to detail during and after the operative event may help improve the complication rate associated with emergency intestinal stoma formation

Factors affecting the outcome of exteriorized colon repairs.

Of 32 penetrating colon injuries managed by repair and exteriorization during the past 5 years in 16 (50%) of the exteriorized repairs healed and 16 failed, being converted to colostomies. The overall morbidity was 25%, 19% for the healed anastomoses and 31% for those that failed. The two groups were similar with respect to age, location of injury, presence of shock, presence of associated injuries, and degree of fecal contamination. Significant differences were evident in the mechanism of trauma and the severity of tissue damage. Exteriorization of colon suture lines is a viable alternative i

Inherent complications of Hartmann's operation.

A series of 67 patients treated by Hartmann's operation was studied, special attention being given to complications originating from the rectal stump. In 13 patients, complications--abscesses of the rectal stump and fistulas from the rectal stump to the small intestine--were seen. Eight of those occurred in patients with leakage at the anastomotic site after anterior resection and in patients with radionecrosis of the rectum. It is concluded that the risk of rectal stump complications is slight in patients with diverticulitis and with carcinoma, who made up the large majority of those in whom

Temporary ileal cecostomy.

Results obtained with temporary ileal cecostomy in 23 patients are reported. Particular attention is given to the incidence of anastomotic leakage, morbidity, mortality, hospitalization and nursing. The results achieved were successful and, although the series presented is small, the authors suggest that this technique should be further considered as an alternative to the more standard diversion techniques of colostomy in colonic resection, particularly where the integrity of the anastomosis is likely to be at risk, as in the anterior resection of the rectum and especially in lower anterior re

Massive foreign object ingestion.

Although foreign object ingestion is usually a benign event, it may result in serious complications. Presented is the case of a 41-year-old man admitted secondary to the ingestion of 71 metallic objects, including a wrench, wire springs, and a razor blade. The patient was observed in hospital for eight days, during which time there was no evidence of passage of any objects. On the eighth hospital day the patient underwent a gastrostomy for removal of the foreign objects. Post-operative recovery was uneventful. Guidelines for both the diagnosis and therapy of foreign body ingestions are discuss

Ureteric complications following treatment of gynaecological cancer.

The incidence, development and diagnosis of ureteric injuries secondary to radical operative or radiological treatment of uterine malignancy is described. Special attention is paid to the importance of early detection and management. Immediate peroperative repair of surgical ureteric damage gives most favourable results. Also in postoperatively diagnosed ureteric lesions the definitive treatment has to be carried out without delay. Ureteroneocystostomy, psoas-hitch procedure or transureteroureterostomy are the most preferable methods. In the late ureteric strictures secondary to radiotherapy t

The low stapled anastomosis.

The complications of 100 stapled anastomoses onto the rectum or anus are reviewed. Anastomotic leakage in very low cases is the major problem; no leak occurred if the anastomosis was performed above 7 cm, but 3 leaks in 17 patients occurred if the anastomosis was performed between 5.5 and 7 cm, and 10 leaks occurred in 52 patients if the anastomoses was less than 5 cm from the anal margin. A covering colostomy is essential for these lower anastomoses, and for those not covered by a colostomy, an immediate colostomy with a distal loop washout is performed, if a leak develops. Attention to techn

Simultaneous onset of ulcerative colitis in the rectum and in a segment of colon used for colpopoiesis: report of a case.

A patient is described in whom a segment of colon used to construct an artificial vagina was therefore removed from intestinal continuity and was affected by typical idiopathic ulcerative colitis. The onset of disease was simultaneous in this loop of colon and in the rectum. There was no evidence of local infection. An increased ratio of IgE-plasma cells was found in both the mucosa of the grafted colon and that of the rectum, but anticolonic antibodies were not detected in the serum. After removal of the loop of colon used for colpopoiesis, the patient continued to suffer from distal proctoco

Morbidity and mortality following intraperitoneal closure of transverse loop colostomy.

A retrospective analysis of 153 patients having intraperitoneal closure of transverse loop colostomies was performed. The mortality was 1.4 per cent. The morbidity rate was 15 per cent, including 7 per cent wound infection was not significantly improved by the use of systemic or nonabsorbable intestinal antibiotics. Intraperitoneal drainage resulted in the highest rate of wound infection. However, the use of intraperitoneal drains seems justified for the control of fecal fistula if it should occur. The lowest incidence of complication was noted when colostomies were closed in 2--4 months. Part

Low complication rate of colostomy closures.

Colostomy closure, a procedure often relegated to the less experienced member of the surgical team, is not a benign procedure. Early reports in the literature have clearly indicated that colostomy closure is attended by an exorbitantly high complication rate. The complication rate of 12% at the Medical Center of Central Georgia from 1972 to 1977 was very much lower than those reported in the literature in the past decade. We believe this relatively low rate of complication is basically due to: (1) proper mechanical and chemical preparation of the colon preoperatively, (2) delayed primary closu

Technical complications of ileostomy.

A review of 45 patients with ileostomy revealed a complication rate of 24%. A higher incidence of complications was seen in those patients who were obese (80%), who had chronic ulcerative colitis (45%), or who had an emergency ileostomy because of a surgical complication (50%). Strict attention to technic should prevent the majority of these complications.
