Flatter Tacrolimus Pharmacokinetic Profiles in Pediatric Kidney Transplant Recipients Receiving Liquid Formulations: A Quality Improvement Study.

BACKGROUND: Compounded tacrolimus suspensions are frequently used in pediatric kidney transplant recipients who cannot reliably swallow capsules or who receive medication via a gastrostomy tube. Clinicians at our center observed unusually flat mini-pharmacokinetic (mini-PK) profiles in patients receiving liquid tacrolimus. METHODS: We retrospectively analyzed 13 tacrolimus mini-PK profiles (C0, C1, C2, C4) from 7 pediatric kidney transplant recipients receiving compounded liquid tacrolimus, measured by LC-MS/MS in routine clinical care. As a contemporaneous comparator measured by the same ass

Advance Care Planning in Amyotrophic Lateral Sclerosis: A Retrospective Population-based Study.

CONTEXT: In recent decades, autonomy and self-determination in Amyotrophic Lateral Sclerosis (ALS), have gained increasing attention, and Advance Care Planning (ACP) has been incorporated into ALS guidelines. In Italy, Law 219 emphasizes the role of ACP in respecting patients' healthcare preferences. However, longitudinal data on ACP prevalence and impact remain limited. OBJECTIVES: To assess the prevalence of ACP discussions in a population of ALS patients, their effectiveness in end-of-life management, and the impact of Law 219/2017. METHODS: Demographic and clinical data from the PARALS R

Association of the lymphocyte-to-C-reactive protein ratio with long-term mortality in hospitalized older adults with severe dysphagia.

BACKGROUND: Dysphagia is highly prevalent among older adults and is strongly associated with malnutrition, chronic inflammation, and multiple comorbidities, all of which contribute to increased mortality. The lymphocyte-to-C-reactive protein ratio (LCR) has recently gained attention as an immunonutritional biomarker, yet its prognostic relevance in elderly individuals with dysphagia remains unclear. This study examined whether LCR is associated with long-term all-cause mortality in this population. METHODS: This study used a retrospective cohort design involving elderly individuals with dysph

Passage of a percutaneous endoscopic gastrostomy tube through the liver: a case report and review of the literature.

BACKGROUND: Passage of a percutaneous endoscopic gastrostomy (PEG) tube through the liver during PEG is a rare but potentially serious complication. CASE PRESENTATION: Here, we report the case of a 78-year-old male patient with dysphagia who underwent PEG in our hospital. Follow-up abdominal computed tomography (CT) revealed that the PEG tube had passed through the left lobe of the liver. This situation poses a clinical dilemma as to whether to remove the tube or allow it to remain in place. Based on existing literature, transhepatic PEG tubes can be used for long periods of time. In our case

Gastric Antral Perforation Caused by a Percutaneous Endoscopic Gastrojejunostomy Tube: A Case Report.

INTRODUCTION: Percutaneous endoscopic gastrojejunostomy (PEG-J) is used when gastric feeding via percutaneous endoscopic gastrostomy (PEG) is poorly tolerated due to gastroesophageal reflux or delayed gastric emptying. Gastrointestinal perforation related to PEG-J has been reported mainly in infants; gastric perforation in adults is extremely rare. We report a case of gastric antral perforation caused by the tube tip 12 days after exchange from a PEG tube to a PEG-J tube. CASE PRESENTATION: A 49-year-old man with severe dysphagia after brainstem hemorrhage underwent PEG placement using the in

Use of a novel screw-type dilator for endoscopic ultrasonography-guided hepaticogastrostomy via a 22-gauge needle and 0.018-inch guidewire.

Tract dilation is an essential step in completing endoscopic ultrasonography-guided hepaticogastrostomy (EUS-HGS). EUS-HGS using a 22-gauge needle is currently attracting attention; however, the complexity of subsequent dilation and guidewire exchange is problematic. A-60-year-old man with duodenal cancer was referred to our center for the drainage of obstructive jaundice. As endoscopic retrograde cholangiopancreatography was not feasible because of duodenal obstruction, EUS-HGS was performed.

A novel valvuloplastic esophagogastrostomy for laparoscopic proximal gastrectomy: Parachute-tunnel-style technique and animal experiments of relationship between anastomotic stenosis and gastric remnant seromuscular flap blood supply.

BACKGROUND: Among several esophagogastric anastomosis techniques with additional mechanical antireflux functions after laparoscopic proximal gastrectomy (LPG), the double-flap technique (DFT) has received increased attention recently. However, some issues related to the central incision in the gastric seromuscular flap of the DFT still warrant further exploration. The incidence of anastomotic stricture in DFT remains a problem that cannot be ignored. To date, there have been no reports that have focused on the relationship between the blood supply of the flap and anastomotic stenosis. Thus, a

Erector Spinae Plane Block as an Anesthetic Technique for Open Gastrostomy: A Case Report.

The erector spinae plane block (ESPB) is a relatively new technique that has been gaining attention for its versatility in providing thoracoabdominal postoperative analgesia. This case report describes the successful use of a bilateral ESPB with sedation as an anesthetic technique in a 50-year-old male diagnosed with esophageal cancer with a suspected bronchoesophageal fistula, who required an open gastrostomy. Given the patient's condition and the potential high risk of respiratory complications associated with general anesthesia, ESPB was chosen for its potential ability to offer effective s

Cannabis oil in treating Parkinson's disease: improvement of motor and non-motor symptoms: a case report.

Parkinson's disease (PD) is characterized by progressive loss of dopaminergic neurons in the substantia nigra pars compacta, which leads to a reduction in the production of dopamine. Medication with levodopa becomes less effective as the disease progresses. Despite the excellent results observed in clinical practice with the medicinal use of Cannabis in the treatment of PD, the level of scientific evidence is still limited due to the small number of studies published in this field. We present the case of a 77-year-old man diagnosed 22 years ago with PD in an advanced stage, with significant br

Nonseizure Outcomes in Dravet Syndrome: Potential Impact of Pharmacotherapy.

Dravet syndrome is a severe developmental and epileptic encephalopathy characterized by drug-resistant seizures and multiple nonseizure comorbidities. While disease management has mainly focused on seizure control, there is growing recognition of the importance of nonseizure outcomes in treatment evaluation. This review examines evidence for treatment effects on key nonseizure domains including cognitive functioning, adaptive behavior, speech and language, neurobehavior, sleep, motor outcomes, orthopedic sequelae, nutrition/growth, and quality of life. There is limited evidence following anti-

Radiologic Assessment of Malpositioned Indwelling Catheters and Medical Devices: A Retrospective Case Series With Pictorial Review.

BACKGROUND: Indwelling medical devices such as central venous catheters (CVCs), ureteral stents, ventriculoperitoneal (VP) shunts, inferior vena cava (IVC) filters, percutaneous endoscopic gastrostomy (PEG) tubes, and nasogastric (NG) tubes are integral to modern clinical care. Malposition or migration, however, can result in severe complications, including vascular injury, organ perforation, infection, and obstruction. Radiologists are often the first to detect these complications, but interpretation may be challenging due to variable device types, insertion techniques, and patient anatomy.

Is it Cellulitis Again? Erythema Concerning for Infection in Children With High-Risk Solid Tumors Who Have a Gastrostomy Tube.

BACKGROUND: Proper nutrition is known to hasten healing, reduce treatment-related morbidity, and improve outcomes. Children with high-risk solid tumors often have gastrostomy tubes (GTs) placed for supplemental nutrition during cancer therapy. Gastrostomy tubes, however, are not without risks, and many patients develop erythema concerning for infection at the stoma site. Gastrostomy complications are described in the literature, but knowledge regarding this topic is limited. METHODS: In this retrospective descriptive study, the authors reviewed 3 years of clinical data regarding pediatric pat

When a gastrostomy tube goes missing, acute bowel obstruction due to a migrated Foley catheter.

A gastrostomy is a common procedure for patients with swallowing problems or inability to maintain regular oral nutrition. These gastrostomy tubes need special attention, as complications can occur if the tubes are left unattended. In rare scenarios, these tubes can migrate and cause severe life-threatening difficulties such as bowel obstruction and pancreatitis. We present the case of a 76-year-old quadriplegic woman who had a gastrostomy tube. Suddenly, the tube was missing, and after urgent medical care, the gastrostomy tube was found within her bowel. After successful surgery, she recovere
