Malignant respiratory-digestive fistulas.

PURPOSE OF REVIEW: Malignant tracheoesophageal or bronchoesophageal or, less commonly, esophageal-lung parenchyma fistulas are late developments of advanced cancer of the esophagus, lung or mediastinum. Patients present mainly with intractable cough and repeated respiratory infections. Rapid deterioration and death results if this condition is left untreated. RECENT FINDINGS: The use of the antiangiogenesis drug bevacizumab along with radiation therapy have been linked to the development of malignant tracheoesophageal fistula/malignant bronchoesophageal fistula in patients treated for both sm

Attention to small details: big deal for gastrostomies.

Gastrostomy tubes are used in the pediatric population when long-term enteral feeding is needed. A common method of placement is percutaneously with endoscopy (PEG, percutaneous endoscopic gastrostomy). Although PEG placement is a straightforward procedure most of the time, it can be associated with a significant rate of minor complications and a smaller but significantly important rate of major complications. Some of these complications may also occur after any type of gastrostomy. We will present representative case studies outlining major complications and discuss how we may be able to prev

Complications of a gastrostomy tube used for decompression of an inoperable bowel obstruction in a patient with advanced cancer.

Percutaneous gastrostomy tubes (G-tubes) are used to decompress symptomatic patients with inoperable malignant bowel obstruction. Complications of G-tubes have rarely been reported in the palliative care literature. Nonetheless, complications are not uncommon and could interfere with the quality of life of patients, as illustrated by our case report. Given the increased use of percutaneous G-tubes, more attention needs to be paid to the prevention, early detection, and treatment of potential adverse events. Patients and their caregivers must receive appropriate education about complications of

Influences and predictors of long-term quality of life in head and neck cancer survivors.

OBJECTIVE: To examine the impact of clinical predictors (pretreatment variables) and other influences (treatment and posttreatment variables) on long-term quality of life (QOL) in patients treated for squamous cell carcinoma of the upper aerodigestive tract. We hypothesized that baseline QOL and comorbidity would be predictors of QOL 1 year after treatment. DESIGN: Retrospective cohort study. SETTING: Academic Medical Center in Seattle, Washington. PATIENTS: Patients (N = 173) with baseline (pretreatment) and 1-year posttreatment QOL data. MAIN OUTCOME MEASURE: Head and neck-specific QOL s

Endoscopic classification for reflux pharyngolaryngitis.

The quality of life in patients who have undergone surgery for esophageal cancer is frequently disturbed by postoperative gastroesophageal reflux disease or pharyngolaryngeal reflux disease. Recently, there have been many reports on gastroesophageal reflux disease after esophagectomy, and only a few on pharyngolaryngeal reflux disease. There is not yet any convenient endoscopic classification of reflux pharyngolaryngitis. We designed a new classification for reflux pharyngolaryngitis based on endoscopic findings. Our new classification consists of the five grades from 0 to IV based on (i) the

[Vascular endothelial growth factor D promoting the growth of lymphatic vessel in gastric carcinoma].

OBJECTIVE: To investigate the relationship between vascular endothelial growth factor D (VEGF-D) and metastasis of lymphatic vessel in gastric carcinoma. METHODS: The human VEGF-D cDNA was amplified from total RNA isolated from human normal gastric tissue, then it was inserted into T-A clone plasmid and subcloned into pEGFP eukaryotic expression vector. After the full-length sequence expected was confirmed by enzymatic digestion and sequencing, the human gastric carcinoma cell line SGC7901, which expressed a low level of VEGF-D, was transfected with the pEGFP/VEGF-D expression vector. Predictors of acute grade 4 swallowing toxicity in patients with stages III and IV squamous carcinoma of the head and neck treated with radiotherapy alone.

PURPOSE: The purpose of the study was to investigate the predictive factors for acute grade 4 swallowing toxicity in an attempt to identify which patients may benefit from early intervention with enteral feeding during curative radiation treatment for localised Stages 3-4 squamous cell carcinoma of the head and neck. It was hypothesised that craniocaudal length of the treatment field to the upper neck and pharynx would correlate with grade 4 swallowing toxicity due to the increased volume of pharynx irradiated. PATIENTS AND METHODS: Toxicity data were collected prospectively as part of a phas

Prevention and management of complications after colon interposition for corrosive esophageal burns.

We present our experience in the management of complications after a colon interposition for corrosive esophageal burns. From April 1976 to December 2006, 85 patients with caustic esophageal burns were included in this study. The superior belly median incision with an anterior border incision of the left sternocleidomastoid was used. Anastomosis between the colon and the cervical esophagus was performed in 68 and between the colon and pharyngeal portion in 14 patients. An esophageal scar part resection and gastric-esophageal anastomosis was performed in one patient who had been given an unsucc

Nutritional therapies in Parkinson's disease.

Advise patients with Parkinson's disease (PD) to consume a balanced diet, with special attention to adequate intake of dietary fiber, fluids, and macro- and micronutrients. Regularly reassess patients' nutritional history and anthropomorphic measures (height and weight), particularly in patients with advanced disease. PD-related psychosocial as well as physical and cognitive limitations increase susceptibility to subacute and chronic malnutrition. Nutritional requirements may change with PD progression or after surgical therapy for PD. Patients and caregivers may benefit from counseling by a d

Clinical costs of feeding tube placement.

BACKGROUND: Although small-bore tube placement is common, insertion can lead to serious complications. We investigated the use of radiographs, fluoroscopy, feeding tubes, and complications associated with blind feeding-tube placement. METHODS: The electronic and paper records of adult patients receiving a small-bore feeding tube in 2005 were retrospectively reviewed for the following variables: demographics, desired location (gastric or postpyloric), number of radiographs, number of tubes per individual, time interval between medical prescription, tube placement and delivery of the diet, comp

External bolster placement after percutaneous endoscopic gastrostomy tube insertion: is looser better?

BACKGROUND: Abdominal and gastric wall inflammation, infection, and necrosis after percutaneous endoscopic gastrostomy (PEG) placement is a topic of importance but of limited study. Healing of the abdominal wound after PEG placement is thought to be dependent on a number of factors, including the patient's nutrition status, comorbid disease status, and postprocedural PEG wound care. Another important factor that may influence wound healing is tissue tension and compression. This requires special attention to the correct placement of the external bolster of the PEG tube against the abdominal wa

A simple adjunct for safer change of PEG.

Percutaneous endoscopic gastrostomy (PEG) is a procedure commonly performed in children who require long-term enteral nutritional support. Although simple, PEG insertion carries the risk of severe and potentially life-threatening complications. Attention to detail is paramount in preventing such complications. Removal is also associated with complications. We describe a simple and inexpensive technique, just requiring a loop nylon suture, which is a useful adjunct to standard endoscopic removal or change of PEG. This modified technique is particularly useful in the untoward event of gastric se

[Esophageal atresia type I. Is impossible possible?].

INTRODUCTION: Treatment of esophageal atresia with "long gap" remains difficult and controversial. According to the idea that esophageal anastomosis is imposible in most cases, several esophageal substitution methods have been proposed, as esophagocoloplasty, gastric transposition or reversed gastric tube. Nevertheless reconstruction of native esophagus is accepted as the best option if posible. "Long gap" definition is imprecise, expressed by variability in percent of these cases in total esophageal atresias reported in different series in literature. We report our experience in seven cases t

Artificial nutrition and hydration at the end of life: ethics and evidence.

The case of Terri Schiavo resulted in substantial media attention about the use of artificial nutrition and hydration (ANH) especially by percutaneous endoscopic gastrostomy (PEG). In this article, I review ethical and legal principles governing decisions to choose or forgo ANH at the end of life, including issues of autonomy and decision-making capacity, similarities and differences between ANH and other medical treatments, the role of proxies when patients lack decision-making capacity, and the equivalence of withholding and withdrawing treatment. Evidence for palliative or life-sustaining b
