Complications in colon and rectal surgery. Early diagnosis and management.

BACKGROUND: Surgery for colon and rectum has significant complications. PURPOSE: This review article was undertaken to discuss the prevention, recognition, and management of the most common complications after colon and rectal surgery. RESULTS: Some serious complications such as anastomotic leak and wound infection can be minimized with good bowel preparation, proper use of antibiotics, good technique, and judgment. Complications such as bladder dysfunction, and sexual dysfunction can also be minimized by the proper anatomic dissection which can avoid an unnecessary disruption of the pelvic

Neonatal intestinal obstruction: reducing short-term complications by surgical refinements.

Over the last ten years advances in neonatal intensive care, such as the advent of parenteral nutrition, have led to a significant decrease in the mortality rates of neonates with intestinal obstruction. Nevertheless, surgical short-term complications associated with the intestinal anastomosis performed, such as stenosis or leakage, and long-term nutritional complications due to extensive resections are still responsible for mortality and for a high morbidity rate. 39 newborns referred to our hospital in the past 3 years for intestinal obstruction were retrospectively evaluated with particular

The definitive stoma: complications and treatment in 50 patients.

Intestinal stomas remain required in many operations for inflammatory bowel disease, for colorectal tumours, trauma and diverticulitis. During one year, 50 patients (20 ileostomies and 30 colostomies) underwent an intestinal stoma and were followed by an enterostomal therapist. The morbidity, the local complications and the treatment are analyzed. An appropriate skin protection is mandatory in the early post-operative period after performing ileostomy or right colostomy. The mucocutaneous dehiscence can be treated by local protection. The quality of life of patients with a stoma is improved by

Small intestinal malabsorption and colonic fermentation of resistant starch and resistant peptides to short-chain fatty acids.

Some starch and protein, as well as fiber, remains unabsorbed in the small intestine and is degraded by anaerobic bacteria to short-chain fatty acids, hydrogen, methane, and carbon dioxide in the large intestine. The production of butyrate from starch has received the most attention, because butyrate seems to possess several important functions in the large bowel, including antineoplastic properties. In 16.6% fecal homogenates, starch polysaccharides, whether digestible or resistant to in vitro hydrolysis by amylase, pectin, and glucose, were all completely degraded to equal amounts of short-c

Effect of intervention in inflammatory bowel disease on health-related quality of life: a critical review.

UNLABELLED: Health-related quality of life (HRQOL) is a quantitative measurement of subjective perception of health state, including emotional and social aspects. It can be reliably measured with several valid instruments. Previous reviews of the literature suggested inadequate attention to HRQOL in studies of interventions in inflammatory bowel disease. PURPOSE: This study was undertaken to assess the current status of the quality of measurement of HRQOL in studies of inflammatory bowel disease and to review the clinical conclusions warranted by the literature. METHOD: Medicine was searched

Suicide as an outcome for medical disorders.

The association between suicide and medical disorder has not received as much attention as the association between suicide and psychiatric disorder. We identified by statistical overview medical disorders with an altered suicide risk. We found reports on the mortality of 63 medical disorders (ICD9 001-289, 320-999) said to have an altered suicide risk. English-language reports were located on MEDLINE with the search terms "disease name with mortality and follow-up"; and from the reference lists of these reports. We abstracted 235 reports of mortality studies of medical disorders with 2 years o

Intussusception in preterm infants.

Two cases of intussusception in infants born at 26 and 30 weeks' gestation are described. The two infants presented in the neonatal period with abdominal distension, intolerance of feeds, and rectal bleeding. An initial diagnosis of necrotising enterocolitis was made and the infants were treated medically. This led to a delay in the diagnosis of the intussusception. Published work on neonatal intussusception is reviewed and attention is drawn to the fact that the presenting signs and symptoms can be similar to those of necrotising enterocolitis. A diagnosis of intussusception should therefore

Partial colectomy and coloanal anastomosis for idiopathic megarectum and megacolon.

Adult patients with an idiopathic megarectum or megacolon can experience severe constipation requiring surgical treatment. Some of these patients have a proximal colon of normal diameter, with dilatation involving only the left or distal colon and rectum. The results of partial colonic and rectal resection with coloanal anastomosis in such patients have been reviewed. Seven patients (two female and five male) underwent a coloanal anastomosis over a seven-year period. The median age at operation was 19 years, the mean age at onset of symptoms was five years, and the mean follow-up period was on

Inflammatory bowel disease. Part I: Nature and pathogenesis.

Once regarded as medical curiosities, ulcerative colitis and Crohn's disease have achieved a remarkable change in status recently and today are among the more compelling of all human illnesses. The cause(s) of inflammatory bowel disease (IBD) are not known. Genetic, environmental, microbial, and immunologic factors are involved, but the precise mechanisms are obscure. The incidence of ulcerative colitis is relatively stable, while Crohn's disease continues to increase in frequency. In 10% to 15% of patients, it is hard to differentiate between ulcerative colitis and Crohn's colitis; however, p

Gastrointestinal disease and oral contraception.

Oral contraceptive steroids play a major role in modern family planning. With the present tendency to decrease the doses of both estrogens and progestogens, any factor that reduces the bioavailability of the lower-dose preparations may have an impact on contraceptive protection. Although ethinyl estradiol, the most commonly used oral estrogen, is liable to an enterohepatic circulation as unchanged drug, the commonly used progestogens are not. At present, no convincing evidence exists in the human subject that disruption of the enterohepatic circulation by antibiotics or antacids does reduce co

Surgical therapy for Crohn's disease.

Most patients with Crohn's disease will require at least one operation for that condition, either an operation to correct a complication of Crohn's disease (abscess, fistula, or bleeding) or for intractability (the failure of medical management to provide relief of disabling symptoms). Proper timing of surgery and careful preoperative preparation of the patient with special attention to control sepsis and to improving nutritional status will make the operation safer. Because of the tendency for Crohn's disease to progress despite medical or surgical therapy recurrences after operation are comm

[Syndrome shift following resection of a pathologic intestine and construction of an anus praeter: a comparison of ulcerative colitis, Crohn disease, rectal cancer and familial polyposis].

Postoperative changes of symptoms were studied in 347 patients with Crohn's disease, ulcerative colitis, carcinoma of the rectum, and familial polyposis. A semi-standardised psychosocial questionnaire and a list of complaints, according to v. Zerssen, were used as tools of investigation. The incidence of postoperative disease in patients with Crohn's disease and ulcerative colitis, within the first year from surgery, was higher with significance than that recorded from carcinoma and polyposis patients within the same period of time. Problems relating to identification of syndrome shift are dis

Innovative surgical procedures and stoma care in the future.

Innovative surgical procedures require particular attention to the technical, educational, and emotional issues. Patients need thorough education to understand surgical options, complications, and expected outcomes. Because of the new aspect of these procedures, patients and significant others may feel quite isolated from others. Recognizing behaviors which may indicate this problem and seeking opportunities to provide emotional support and role modeling from other patients are most therapeutic. The quest for continence provides the patient with alternatives to be cured of disease and avoid th

Squamous cell carcinoma occurring in a skin-grafted ileostomy stoma. Report of a case.

A squamous cell carcinoma arose in a skin-grafted ileostomy stoma 26 years after proctocolectomy for Crohn's colitis. The patient presented with peristomal ulceration and pain of several months' duration. Attention is drawn to this late complication of ileostomy which requires wide local excision and relocation of the stoma.

The technique of tube ileostomy has received little attention. It offers the advantage of complete fecal diversion without the need for subsequent operative closure. We emphasize the value of this technique and suggest an easier method for inserting the ileostomy tube.

The ParkS S ileal pouch and anal anastomosis after colectomy and mucosal proctectomy.

Attention to detail is crucial to the success of the operation described. Surgeons contemplating performing it should first be experts in pelvic surgery and are advised to personally observe and participate in the procedure performed by surgeons currently experienced in this technique.

Distal ileostomy drip feedings in neonates with short bowel syndrome.

When a newborn infant presents with high intestinal atresia, the proximal segment of the bowel is usually grossly distended and atonic. The anastomosis of this segment to the smaller and unused distal segment will usually result in little or no propulsion of contents distally. Many techniques have been employed to correct this problem. A common surgical approach is immediate end-to-end anastomosis, followed by parenteral alimentation until return of normal function. This can take many weeks, and requires special attention to fluid loss and complications associated with parenteral alimentation.
