Optical Coherence Tomography for Quantitative Point-of-Care Ear Imaging.

OBJECTIVE: To determine whether an optical coherence tomography (OCT) otoscope can provide quantitative morphological measurements of the tympanic membrane (TM) that distinguish abnormal ears from normal controls. METHODS: Patients with ear disease and normal volunteers were imaged using an OCT otoscope. Integrated video otoscopy with simultaneous OCT imaging guided probe positioning. OCT volumes (< 5 s each) were acquired and fused to generate wide-field volumetric images of the TM and middle ear (ME). Datasets were analyzed using semi-automated segmentation to quantify TM thickness and surf

Myofibrosarcoma of the maxillary sinus with orbital floor involvement: a case report.

Myofibrosarcoma is a rare spindle-cell malignancy with myofibroblastic differentiation, and primary maxillary sinus involvement is exceptional. A 36-year-old woman presented with progressive unilateral nasal obstruction, purulent rhinorrhea, and hemifacial pain. Computed tomography and magnetic resonance imaging demonstrated a destructive 44 × 45 × 57 mm right maxillary sinus mass involving the orbital floor and extending into the nasal cavity, ethmoid cells, and pterygopalatine fossa. Histology and immunohistochemistry supported the diagnosis of myofibrosarcoma. Partial maxillectomy with tita

Convolutional neural network metal artifact reduction in ultra-high resolution photon counting detector CT of middle- and inner-ear implants.

BACKGROUND: Metallic inner- and middle-ear implants with submillimeter structures requiring ultra-high resolution (UHR) imaging invariably result in metal artifacts at CT, which can obscure adjacent microstructures and subtle pathologic findings. Current metal artifact reduction techniques can be less effective for small metal objects or metal in heterogeneous background anatomy, and can sometimes introduce new artifacts or noise. PURPOSE: To develop and evaluate an image-domain convolutional neural network to reduce metal artifacts near inner- and middle-ear implants in UHR photon-counting-d

Unexpected Identification of Two Medialized Tympanostomy Tubes in an Adult: A Case Report.

Tympanostomy tube placement is one of the most commonly performed otologic procedures and is widely used in the management of chronic otitis media with effusion (OME), recurrent acute otitis media (AOM), and Eustachian tube dysfunction (ETD). Although generally safe, complications may occur, and medialization of a tympanostomy tube into the middle ear space is a rare but recognized complication that may be associated with surgical removal due to the potential for chronic inflammation, conductive hearing loss, or middle ear injury. We present a case of two medialized tympanostomy tubes identifi

Nuclear protein in testis midline carcinoma of nasal cavity: a case report and review of the literature.

Nuclear protein in testis (NUT) midline carcinoma (NMC) is an extremely rare, highly aggressive, and poorly differentiated malignant neoplasm that predominantly arises from the midline epithelial structures of the head and neck. It is driven by genetic rearrangements of the NUTM1 gene, with the most common aberration being a translocation that forms a fusion gene with BRD4 on chromosome 19p13.1. The diagnosis of NMC is challenging due to its non-specific clinical and histological features, and it typically requires a combination of immunohistochemical detection of NUT protein and molecular ass

Multidomain Auditory Differences in Tinnitus With a Clinically Normal Audiogram: Accounting for Extended High-Frequency Hearing.

OBJECTIVE: Tinnitus can occur in individuals with clinically normal audiograms, suggesting that conventional threshold-based measures may not capture subtle auditory-system differences. This study aimed to characterize peripheral and subcortical auditory function in tinnitus using a multidomain, exploratory multivariate framework. METHOD: Twenty individuals with tinnitus and clinically normal conventional audiograms (0.125-8 kHz) and 25 age-matched controls underwent a multidomain auditory assessment, including auditory brainstem response (ABR), otoacoustic emission, medial olivocochlear supp

Myoepithelioma of the nasal septum: A case report with radiological and pathological insights.

Myoepithelioma is a rare benign salivary gland tumor, accounting for <1% of all salivary gland neoplasms, with only a little over ten cases of nasal cavity or paranasal sinus involvement reported in the English literature. Due to its rarity, lack of specific imaging features, and significant histological overlap with other tumors, the diagnosis poses a challenge for clinicians, radiologists, and pathologists. We present a case of a 68-year-old male with progressively worsening nasal obstruction over 6 months, ultimately diagnosed with nasal myoepithelioma. Approximately 3 months after endoscop

Expanding the genetic landscape of seromucinous hamartoma: novel HMGA2::PHF14 gene fusion - a case report.

Seromucinous hamartomas (SH) are benign polypoid proliferations of small eosinophilic glands arising in the sinonasal tract. Together with respiratory epithelial adenomatoid hamartoma (REAH), they form a morphological continuum, and their hamartomatous designation has been increasingly questioned. Some authors now consider them stationary precancers potentially linked to tumor development through atypical sinonasal glands arising via the SH pathway. The molecular genetic background of SH remains poorly characterized, with no recurrent genetic alterations identified and no unequivocal molecular

Nasal In Situ Hydrogels: A Boon For Anti-Emetic Drug Delivery.

Nasal route presents an alternative avenue for achieving local or systemic drug effects owing to its non-invasiveness, expansive surface area, rapid drug absorption, and circumvention of first-pass effect. It offers a promising route for brain targeting too, as it bypasses the blood-brain barrier, thus facilitating direct nose to brain drug delivery. However, traditional routes of administering anti-emetic drugs, primarily oral or parenteral, suffer from drawbacks like poor patient compliance and low drug bioavailability. The nasal route is a favorable alternative, albeit limited by the short

Maxillary sinus myoepithelioma: a case report.

INTRODUCTION: Pleomorphic adenoma (PA) in the sinonasal tract is very rare, with few cases documented in the literature. Myoepithelioma is a recognized variant of PA characterized by predominant myoepithelial differentiation and the absence of ductal structures. CASE PRESENTATION: A 67-year-old man presented with a right-sided sinonasal mass. Cross-sectional imaging revealed a lesion originating from the right maxillary sinus and extending into the ipsilateral nasal cavity and nasopharynx. An initial biopsy favored a diagnosis of inverted papilloma, and the patient subsequently underwent endo

Middle ear cholesteatoma: biological basis, invasiveness heterogeneity, and challenges in severity assessment.

PURPOSE: Middle ear cholesteatoma (MEC) is a benign but biologically aggressive middle ear lesion characterized by progressive and destructive bone erosion. However, current evaluation systems remain heterogeneous and focus on limited anatomical structures, leading to insufficient clinical predictive value. This review aims to integrate current evidence on the biological mechanisms of MEC-associated bone erosion and assessment systems, with the goal of informing the development of more standardized and clinically meaningful severity evaluation models. METHODS: A narrative review was performed

Calcium Pyrophosphate Deposition Disease of the Temporomandibular Joint Extending into the External Auditory Canal and the Middle Ear, Coexisting with Otosclerosis: A Case Report.

Calcium pyrophosphate deposition disease (CPPD) is caused by CPP crystals and rarely forms tumors in joints. The authors describe a case of tumoral CPPD in the temporomandibular joint (TMJ), extending into the external auditory canal (EAC) and the middle ear. A 71-year-old man was referred to the department with a tentative diagnosis of CPPD in the right TMJ. Computed tomography showed the mass extending into the EAC, tympanic cavity, and bony Eustachian tube, with opacification of the mastoid cells. The audiogram revealed bilateral mixed hearing loss, suggesting the coexistence of bilateral o

Proliferation versus inflammation: Immunohistochemical profiling of feline chronic rhinitis and nasal lymphoma.

Lymphoma is the most common nasal cavity tumor in cats. While most recent research efforts have characterized neoplastic cell markers in feline alimentary lymphoma, only few reports are dedicated to immunohistological features of nasal lymphoid tumors beyond T- or B-cell phenotyping. In comparison, feline lymphoplasmacytic chronic rhinitis represents chronic, progressive lymphoplasmacytic inflammation, causing cyclical tissue damage and repair. Sixty-five cases of lymphoplasmacytic rhinitis and 15 cases of feline diffuse, large cell, immunoblastic nasal lymphoma were selected and submitted for
