Primary care providers are now asked to render care to diabetic patients that once was the province of specialists. An increasing number of these patients are using insulin pumps to manage their diabetes. Inherent in the use of pumps are unique day-to-day situations that must be addressed with timely solutions to avoid adverse metabolic effects and to maximize the benefits of this type of therapy. To increase the primary care provider's comfort level with handling these situations, this article examines four problem areas, using a case study format followed by a discussion of the solutions to

BACKGROUND: In a European trial in 8 countries, the subjective well-being of patients on alternative forms of treatment for insulin-dependent diabetes was compared using the 28-item WHO Well-Being Questionnaire, covering four dimensions of depression, anxiety, energy and positive well-being. The objective of the analysis reported here has been to identify the items of the WHO questionnaire which belong to an overall index of negative and positive well-being.
METHODS: Adult patients at 10 study centres in 8 countries who had been on insulin for at least 2 years were invited to participate in a

Zollinger-Ellison syndrome (ZES) should be suspected if a patient has severe peptic ulceration, ulcers and kidney stones, a family history of ulcers or endocrine diseases, watery diarrhoea or malabsorption with or without ulcers, or if hypergastrinaemia is found. Any patient in whom ZES is suspected, and certainly if diagnosed, should be given large doses of antisecretory medication immediately. This should never be stopped except under controlled conditions or unless acid outputs have been reduced surgically. Patients cannot be managed safely without measuring acid outputs. These should be lo

The toxicity of 5-fluorouracil (5-FU) was decreased by two to eight-fold if this drug was injected in mice near the middle of the day (rest span) rather than in the middle of the night (activity span). If the rhythm in 5-FU toxicity is linked to the sleep-wakefulness endogenous circadian cycle across species, the least toxic time in man would correspond to 4.00 hours at night. The availability of a single-reservoir programmable-in-time external ambulatory pump (Chronopump, Autosyringe, Hooksett, USA) allowed us to perform a first test of this hypothesis. Five-FU was infused for 5 consecutive d

Sixty-three outpatients undergoing breast biopsy procedures with local anesthesia were randomly assigned to receive propofol by intermittent bolus injections (n = 21), a conventional syringe infusion pump (n = 21), or a target-controlled infusion (TCI) device (n = 21) for intraoperative sedation. In the first two groups, an initial intravenous (IV) bolus of propofol (0.3 mg/kg) was administered and an attempt was made to maintain the sedation level at an Observer's Assessment of Alertness/Sedation (OAA/S) score of 3 or 4 with either intermittent bolus injections of propofol (10 mg) or a variab

Ten per cent of patients with haemophilia A develop intracranial haemorrhage (ICH) with a mortality rate of 30% and an incidence of psycho-neurological sequelae in 50% of survivors. ICH may be spontaneous or in association with trauma and other pathology. The generally recommended management is conservative replacement therapy using bolus injections of factor VIII and no neurosurgical intervention. Adjusted dose continuous infusion therapy provides an alternative method of factor VIII administration that is simple, more cost effective and safer through the maintenance of stable plasma VIII:C l

OBJECTIVE: The objective was to test the safety and efficacy of a pulsatile oxytocin infusion protocol in which a computer-controlled pump adjusts the oxytocin dose rate on the basis of uterine activity.
STUDY DESIGN: A total of 358 women were enrolled in, and 310 completed, a prospective, randomized clinical trial comparing three protocols for the induction of labor with oxytocin: aggressively managed continuous infusion, conservatively managed continuous infusion, and computer-controlled pulsatile infusion. Results were analyzed with Student t and chi 2 "goodness-of-fit" tests.
RESULTS: Me

Insulin infusion pump therapy is a treatment option that facilitates achieving improved blood glucose control and lifestyle flexibility. These advantages are derived from the physiologic mode of insulin delivery and the pharmacologic advantages of using rapid-acting insulin. Patients who are sufficiently motivated and capable can learn to use the pump so that insulin adjustments can be made to compensate for changing circumstances. Although there are potential risks and side effects of using an insulin pump, these can be avoided through proper education and surveillance. The health care provid

A total of 66 consecutive patients with severe spasticity of spinal cord origin were screened with intrathecal baclofen, and all but two responded with a two-point decrease in their Ashworth spasticity scale and/or spasm scale score. Of these, 62 elected to receive chronic intrathecal baclofen administration by means of an implanted delivery system. These patients have been followed for an average of 30 months (the first three for 81 months). Intrathecal baclofen has been well tolerated and all serious side effects were transient and have been managed by dose adjustments. The pump presently av

During and after many surgical procedures the patient's arterial blood pressure must be artificially decreased to a lower than normal level. Although there are alternatives, infusion of the drug sodium nitroprusside (SNP) is frequently the preferred technique to achieve this controlled hypotension. The fast action of the drug and the danger of a too low pressure make manual control of the SNP infusion flow rate, even if done by an expert, a difficult and demanding task. This is mainly due to an occasional large unpredictable variability over time of the patient's sensitivity to SNP, and to the

It has been shown in recent years that optimal treatment delays or prevents diabetic late complications. The goal for therapy is therefore near norm-glycemia. The precondition is intensive education of the patient and the parents, as well as exact diet with known amounts of carbohydrates, multiple insulin injections and home blood glucose monitoring. We propose two different schemes for insulin therapy according to age: conventional insulin therapy in the younger child, and intensified insulin therapy with the insulin pen in adolescents. Conventional insulin therapy consists of two daily injec

A standardized intravenous regimen has been assessed, in 25 insulin-treated diabetic women, for insulin and dextrose therapy in labour and delivery. Adjustments to insulin infusion rate are determined by trends in blood glucose as well as by absolute concentration, in order to approach normoglycaemia. Blood glucose was 5.0 (SD 1.7) mmol l-1 on arrival in labour (or at 0800 h before planned delivery) and was maintained at 6.0 (SD 1.8) mmol l-1 with insulin 0-5 U h-1 for up to 29 h before delivery, when it was 6.3 (SD 2.1, range 3.0-9.0) mmol l-1 with insulin infusion rate 0-4 U h-1. This presentation describes the problem of insulin-dependent diabetes mellitus and the various approaches to creating intelligent instrumentation for management of this type of diabetes. The article traces closed-loop diabetes control from 1974 to the present. Development and implementation of a microprocessor device aimed at achieving this in a practical sense are described. Accordingly, a chronological description of the earliest devices and their shortcomings is given, focusing on the use of blood glucose sensors and vascular access devices. A description of open-loop diabetes control and a

The hospital pharmacist is in a unique position to assist in the management and monitoring of the diabetic patient. He can alert the nurse and physician concerning medication errors involving the use of insulin and oral sulfonylureas. Medication errors may not only jeopardize the metabolic control of a person with diabetes but may also result in the development of significant hypoglycemia. Insulin orders should be written clearly and concisely. The advocation for intensive split-dose regimens and the use of insulin infusion devices require careful attention to technique, administration times a

Manual titration of sodium nitroprusside (SNP) is widely used for treatment of hypertension following cardiac surgery. This study compared conventional manual control with control by a research prototype of an automatic infusion module based on a proportional plus integral plus derivative (PID) negative feedback loop. Two groups of coronary artery bypass patients requiring SNP for postoperative hypertension were studied prospectively. In the first group, hypertension was controlled by manual adjustment of the SNP infusion rate, and in the second, the infusion rate was controlled automatically.

Diabetes mellitus is a group of disorders that have in common abnormal insulin function resulting in disordered carbohydrate, protein and fat metabolism, the cardinal feature being elevated blood glucose levels. Acute complications include diabetic ketoacidosis, hyperosmolar non-ketotic "coma" and hypoglycemia. Long-term complications develop over several years and cause more morbidity and mortality than the acute complications. Although the pathogenesis of long-term complications is still unknown, it is likely that it relates to the abnormal metabolic state that occurs in diabetes. Current te

Nine patients in preterm labor requiring long-term tocolysis were managed with subcutaneous terbutaline administered via a portable infusion pump. All had failed oral tocolytic therapy and were therefore faced with prolonged hospitalization. In this feasibility study, the patients were treated at home and monitored with portable tocodynamometers and home nursing visits. Uterine activity data were transmitted via telephone to the study center and terbutaline pump infusion rates were adjusted accordingly. Terbutaline pump therapy consisted of a combination of low-dose continuous basal infusion s

In about one third of patients with violent spasticity due to spinal trauma, multiple sclerosis, and diffuse brain injury adequate control with oral antispastic medication cannot be achieved and successful rehabilitation is severely handicapped. In the past these patients were subjected to destructive chemical procedures or extensive surgery. The authors present the results of management of uncontrollable spasticity by means of continuous intrathecal administration of baclofen with a totally implantable gas driven pump system (Infusaid). 30 patients were treated between June 1985 and January.

The use of an i.v. report form to regularly monitor and adjust the schedules of large volume parenteral solutions and irrigations is discussed. The departments of pharmaceutical services and nursing instituted a program that allowed both departments to compare the schedules of large volume parenterals and irrigations for each shift. This program centers around the use of an i.v. report form, for which clinical nurses record information concerning the progress of i.v. fluids and irrigations. The completed report is forwarded to the pharmacy, where pharmacy personnel use the information to conti
