The relationship between catheter-related bloodstream infection and multi-drug resistant bacteria: a five-year retrospective study.

BACKGROUND: Catheter-related bloodstream infection (CRBSI) is a severe hospital-acquired infection primarily associated with using central venous catheters (CVCs). With the rise of antibiotic resistance and the emergence of multidrug-resistant organisms (MDROs), there is a growing concern about the relationship between MDROs and CRBSI. OBJECTIVE: To explore the association between MDROs and CRBSI. METHODS: A retrospective study was conducted on hospitalized patients with implanted CVCs over 54 months (January 2019 to June 2023). RESULTS: We included 50,718 patients with indwelling central v

Risk of Cardiac Implantable Electronic Device Infection With Temporary Tunneled Hemodialysis Catheters.

BACKGROUND: Compared to permanent surgically created hemodialysis (HD) vascular access, tunneled vascular access catheters for HD may pose an even higher risk of blood stream infection (BSI). For patients with transvenous cardiac implantable electronic devices (CIEDs), the risk of device infection associated with tunneled HD catheter is not well-characterized. METHODS: The Nationwide Readmissions Database (NRD) 2016-2021 was used for analysis. Patients with end-stage kidney disease (ESKD) and a CIED who were admitted for any reason were identified by ICD-10 codes and stratified based on wheth

Device-Related Adverse Events and Modes of Failure in Renal Denervation Systems: Insights From the Food and Drug Administration Manufacturer and User Facility Device Experience (FDA MAUDE) Database Analysis.

Renal denervation (RDN) therapy has emerged as a device-based treatment option for patients with resistant hypertension. There are currently two Food and Drug Administration (FDA) approved RDN systems in the United States: the ParadiseR RDN system (Recor Medical, Palo Alto, California) and the Symplicity SpyralTM RDN system (Medtronic, Minneapolis, Minnesota). There are limited data on commonly reported adverse events and failure modes associated with these devices. Our objective is to assess the frequently reported adverse events and modes of failure for RDN therapy with the two FDA-approved

Site-specific complications of central venous catheterization under systematic ultrasound guidance: a target trial emulation revisiting the 3SITES study.

BACKGROUND: Central venous catheterization is the most common invasive procedure in intensive care units but remains burdened by infectious, thrombotic, and mechanical complications. Although real-time ultrasound guidance is now widely adopted, its effect on site-specific differences in overall complication rates has not been established. The 3SITES randomized clinical trial previously demonstrated lower infection and thrombosis rates with subclavian access but higher mechanical complications. However, as only a third of its procedures were ultrasound-guided in this study, these findings may n

Definition and diagnostic criteria of clinical obesity.

Current BMI-based measures of obesity can both underestimate and overestimate adiposity and provide inadequate information about health at the individual level, which undermines medically-sound approaches to health care and policy. This Commission sought to define clinical obesity as a condition of illness that, akin to the notion of chronic disease in other medical specialties, directly results from the effect of excess adiposity on the function of organs and tissues. The specific aim of the Commission was to establish objective criteria for disease diagnosis, aiding clinical decision making

Importance of risk adjusting central line-associated bloodstream infection rates in children.

OBJECTIVE: Central line-associated bloodstream infection (CLABSI) is one of the most prevalent pediatric healthcare-associated infections and is used to benchmark hospital performance. Pediatric patients have increased in acuity and complexity over time. Existing approaches to risk adjustment do not control for individual patient characteristics, which are strong predictors of CLABSI risk and vary over time. Our objective was to develop a risk adjustment model for CLABSI in hospitalized children and compare observed to expected rates over time. DESIGN AND SETTING: We conducted a prospective c
