Provider factors associated with intramedullary nail use for intertrochanteric hip fractures.

BACKGROUND: Intramedullary nails provide no clear outcomes benefit in the majority of patients with intertrochanteric hip fracture, yet their use in the United States continues to increase. Non-patient factors that are associated with intramedullary nail use among Medicare patients have not been examined. The goal of this study was to identify the surgeon and hospital characteristics that were associated with the use of intramedullary nails compared with plate-and-screw devices among elderly Medicare patients with intertrochanteric hip fractures. METHODS: Medicare beneficiaries who were sixty

Outcome of primary resurfacing hip replacement: evaluation of risk factors for early revision.

BACKGROUND AND PURPOSE: The outcome of modern resurfacing remains to be determined. The Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) started collection of data on hip resurfacing at a time when modern resurfacing was started in Australia. The rate of resurfacing has been higher in Australia than in many other countries. As a result, the AOANJRR has one of the largest series of resurfacing procedures. This study was undertaken to determine the results of this series and the risk factors associated with revision. PATIENTS AND METHODS: Data from the AOANJRR we

Younger age increases the risk of early prosthesis failure following primary total knee replacement for osteoarthritis. A follow-up study of 32,019 total knee replacements in the Finnish Arthroplasty Register.

BACKGROUND AND PURPOSE: Total knee replacements (TKRs) are being increasingly performed in patients aged < or = 65 years who often have high physical demands. We investigated the relation between age of the patient and prosthesis survival following primary TKR using nationwide data collected from the Finnish Arthroplasty Register. MATERIALS: From Jan 1, 1997 through Dec 31, 2003, 32,019 TKRs for primary or secondary osteoarthritis were reported to the Finnish Arthroplasty Register. The TKRs were followed until the end of 2004. During the follow-up, 909 TKRs were revised, 205 (23%) due to infe

Clinical practice of defibrillator implantation after myocardial infarction: impact of implant time: results from the PreSCD II registry.

AIMS: Current guidelines recommend implantable cardioverter-defibrillator (ICD) therapy for primary prevention of sudden cardiac death in patients with the reduced left ventricular function (LVEF ≤30%) not earlier than 40 days after myocardial infarction (MI). The aim of the prospective Prevention of Sudden Cardiac Death II (PreSCD II) registry was to investigate the clinical practice of ICD therapy in post-MI patients and to assess the impact on survival. METHODS AND RESULTS: 10,612 consecutive patients (61 ± 12 years, 76% male) were enrolled 4 weeks or later after MI in 19 cardiac rehabilit

Clustering of poor device acceptance and Type D personality is associated with increased distress in Danish cardioverter-defibrillator patients.

BACKGROUND: Psychosocial risk factors tend to cluster together within individuals, likely enhancing the risk of adverse health outcomes. We examined (1) the influence of clustering of poor device acceptance and Type D personality on anxiety and depressive symptoms, and (2) the demographic and clinical determinants of patients with clustering, in a large cohort of Danish implantable cardioverter defibrillator (ICD) patients. METHODS: Patients (N = 557; 81.9% male; mean age = 61.9 +/- 14.3 years) implanted with an ICD between 1989 and 2006 were asked to complete a set of standardized and valida

Meta-analysis of first metatarsophalangeal joint implant arthroplasty.

UNLABELLED: Management of late-stage degenerative joint disease of the first metatarsophalangeal joint (MPJ) is a complex topic that is frequently the source of debate among foot and ankle surgeons. Several surgical interventions have been described to treat this condition. One of the most contested of these treatments is implant arthroplasty of the first MPJ. The primary aim of this meta-analysis was to evaluate the clinical benefit of first MPJ implant arthroplasty in regard to patient satisfaction. Reviewers formally trained in meta-analysis abstraction techniques searched databases and ind

Efficacy of antiarrhythmic drugs in arrhythmogenic right ventricular cardiomyopathy: a report from the North American ARVC Registry.

OBJECTIVES: This study sought to examine the efficacy of empiric antiarrhythmic drugs in a rigorously characterized cohort of arrhythmogenic right ventricular cardiomyopathy (ARVC) patients. BACKGROUND: Antiarrhythmic drugs are important in protecting against ventricular arrhythmias in ARVC, but no studies have provided data in a group rigorously screened for the disease. METHODS: Antiarrhythmic medicines were examined in all subjects with implantable cardioverter-defibrillators (ICDs) enrolled in the North American ARVC Registry. A Cox proportional hazards model was used to account for time

Association of preoperative symptom profile with psychiatric symptoms following subthalamic nucleus stimulation in patients with Parkinson's disease.

In order to evaluate the severity of behavioral complications after deep brain stimulation of the subthalamic nucleus (STN-DBS) for Parkinson's disease and to explore possible predictive factors, the authors evaluated 22 patients for pre- and postoperative symptoms using a neurobehavioral battery. Compared to the time before STN-DBS, several behavioral symptoms had worsened in terms of prevalence and severity and appeared de novo in other patients. Apathy, anxiety, and suicidal ideation increased significantly, while depressive symptoms appeared stable. Compared with patients who improved, pat

Invasive monitoring in patients with heart failure.

The syndrome of heart failure is characterized by symptoms that are relatively insensitive and nonspecific. Physical diagnosis may be unreliable even in the hands of experienced clinicians, despite the presence of significantly elevated filling pressures or a significantly depressed cardiac output. Instrumentation and devices such as the insertion of a pulmonary artery catheter and the implantable hemodynamic monitor have a major role in the diagnosis and management of cardiovascular disease. They provide a means of measuring intracardiac pressures for point-in-time measurements (cardiac cathe

Association of physician certification and outcomes among patients receiving an implantable cardioverter-defibrillator.

CONTEXT: Allowing nonelectrophysiologists to perform implantable cardioverter-defibrillator (ICD) procedures is controversial. However, it is not known whether outcomes of ICD implantation vary by physician specialty. OBJECTIVE: To determine the association of implanting physician certification with outcomes following ICD implantation. DESIGN, SETTING, AND PATIENTS: Retrospective cohort study using cases submitted to the ICD Registry performed between January 2006 and June 2007. Patients were grouped by the certification status of the implanting physician into mutually exclusive categories:

The everolimus-eluting stent in real-world patients: 6-month follow-up of the X-SEARCH (Xience V Stent Evaluated at Rotterdam Cardiac Hospital) registry.

OBJECTIVES: The purpose of this study was to investigate the impact of everolimus-eluting stents (EES) in comparison with bare-metal stents (BMS), sirolimus-eluting stents (SES), and paclitaxel-eluting stents (PES) on the 6-month clinical outcomes in an all-comer population. BACKGROUND: EES have been shown to be effective in the context of randomized trials with selected patients. The effect of EES implantation in more complex, unselected patients cannot be directly extrapolated from these findings. METHODS: In total, 649 consecutive unselected patients treated exclusively with EES were enro

Impact of center volume on outcomes of left ventricular assist device implantation as destination therapy: analysis of the Thoratec HeartMate Registry, 1998 to 2005.

BACKGROUND: More than 400 patients with end-stage heart failure underwent left ventricular assist device (LVAD) implantation of LVAD as destination therapy (DT) after the US Food and Drug Administration approval of DT in 2002. Because most of these patients had surgeries at hospitals that were newly accredited, we sought to examine the impact of LVAD center volume on the outcomes of DT. METHODS AND RESULTS: From July 1998 through December 2005, a total of 377 patients underwent implantation of HeartMate I LVAD as DT at 68 centers in the United States. Using data from the Thoratec DT Registry,

Using a webcast support service: experiences of in-person attendees of an implantable cardioverter defibrillator support group.

OBJECTIVE: Most patients with implantable cardioverter defibrillators (ICDs) adjust well to living with the device; however, some experience difficulties. Support groups assist in coping with the psychologic effects of living with an ICD. The study's aim was to examine acceptability of the in-person attendees of an ICD support group that was cast on the Internet. SAMPLE: A patient satisfaction survey describing the participants' experience was used as a measure of acceptability in this non-experimental, survey, pilot study. METHODS: The survey assessed reactions of the in-person participants

Child pectus excavatum: correction by minimally invasive surgery.

INTRODUCTION: Pectus excavatum (PE) is a congenital deformity essentially responsible for an unattractive aspect, much more rarely for compression problems. The classical treatments consist either in filling the excavation or in open thoracic reconstruction (the Ravitch technique). Alternatively, the treatment described by Nuss raises the sternum with a retrosternal metallic bar placed under thoracoscopic guidance. We present the preliminary results of a series of 25 children operated on using this technique. HYPOTHESIS: The minimally invasive procedure described by Nuss is a valid surgical s
