BACKGROUND: Heart failure-related cardiogenic shock (HF-CS) accounts for a growing proportion of cardiogenic shock (CS)-related admissions to contemporary cardiac intensive care units. Limited data exist comparing nonischemic (NICM) and ischemic cardiomyopathy (ICM) in this setting.
METHODS AND RESULTS: We sought to examine the differences in patients' characteristics, in-hospital treatments and outcomes in individuals admitted with ICM and NICM HF-CS. The study population included CS admissions within the Critical Care Cardiology Trials Network registry from 2017-2022. CS due to acute myocar

BACKGROUND: Periprosthetic femur fractures are a known complication after hip hemiarthroplasty (HA) in geriatric patients. The relationship between femoral stem design and fracture risk remains unclear. This study aimed to assess (1) the association between stem geometry and fixation on periprosthetic femur fracture risk and (2) the effect of a femoral stem collar.
METHODS: The American Joint Replacement Registry data were analyzed for HA cases in patients aged 70+ years from 2012 to 2021. We identified 56,828 primary HAs for the diagnosis of femoral neck fracture. Patient demographics and re

The increasing prevalence of non-cardiac implantable electrical devices (NCIEDs), including deep brain stimulators (DBS), spinal cord stimulators (SCS), and an increasing list of other devices, presents a significant challenge to conscientious perioperative care teams. The major anticipated operative complications of having a pre-existing indwelling device are twofold. For the first category, there is the profound threat of direct thermal injury to tissue surrounding the device in question, as well as the lateral thermal spread. Of course, when the surrounding tissue is neural and may involve

Venous spasm is a rare yet clinically relevant complication during pacemaker implantation that can obstruct venous access and delay procedures. Despite its significance, it is infrequently reported in the literature and not clearly understood, thus lacking a standardized management protocol. This review aimed to synthesize current literature on venous spasm occurring during pacemaker or any other cardiac device implantation, highlight common features, and summarize effective management strategies. A comprehensive search of PubMed, Embase, and ScienceDirect through July 2025 identified 17 relev

BACKGROUND: Spinal cord stimulation (SCS) systems that deliver fixed amplitudes expose target tissue to varying electrical fields due to the changing lead-to-cord distance accompanying postural shifts and other body movements. Inconsistent stimulation results in periods of overstimulation or under-stimulation perceived by patients as discomfort or potentially inadequate pain relief. CL-SCS may be applied to provide a comfortable level of perception down to imperceptible stimulation, commonly preferred in higher frequency and multiplexed programming. Here we report outcomes from a study evaluat

HYPOTHESIS AND BACKGROUND: As the incidence of reverse total shoulder arthroplasty (rTSA) continues to rise, better understanding of the long-term risks and complications is necessary to determine the best choice of implant. The majority (75%) of rTSAs performed in New Zealand use either SMR (Systema Multiplana Randelli; Lima-LTO) or Delta Xtend (DePuy Synthes). The aim of this registry-based study was to compare implant survival, risk of revision, and reasons for revision between the 2 most frequently used rTSA prostheses: SMR and Delta Xtend.
METHODS: Using data from the New Zealand Joint R

Novel implant systems have design modifications that seek to improve total knee arthroplasty (TKA) survivorship. We evaluated overall and cause-specific revision risk for a newer generation implant system compared with its predecessor from the same manufacturer. We conducted a cohort study using data from the United States-based Kaiser Permanente Total Joint Replacement Registry. Adult patients who underwent primary, fully cemented, fixed-bearing TKA for osteoarthritis between 2009 and 2022 were identified. Only two implant systems from the manufacturer were included: the newer generation (n =

BACKGROUND: Use of cementless TKA has grown after encouraging data from contemporary implants. Yet registry studies have shown inferior survivorship of cementless fixation when treated as a monolithic class aggregating contemporary and historic designs. Two contemporary cementless TKA designs with distantly different fixation strategies have emerged in the last 2 decades, mostly focused on tibial fixation: porous tantalum and twin-peg tibia and, more recently, porous titanium and a spikes-and-keel tibia. However, their survivorship in comparison with cemented options and between each other rem

BACKGROUND: Evidence of any benefit in the long-term performance of highly crosslinked polyethylene (HXLPE) for total knee arthroplasty (TKA) is lacking. We sought to evaluate revision risk for HXLPE versus conventional polyethylene (CPE).
METHODS: Data from a US-based health care system's Total Joint Replacement Registry were used to conduct a cohort study. Adult patients who underwent primary fixed-bearing TKA with patella resurfacing for osteoarthritis were included (2001 to 2023). The final study sample included 232,527 TKAs: 105,028 HXLPE and 127,499 CPE. The primary outcome was revision

BACKGROUND: Residual mitral regurgitation (rMR) ≤1+ has been associated with improved 1-year outcomes after mitral valve transcatheter edge-to-edge repair, regardless of the mean mitral pressure gradient (MPG). Prior evidence is limited to 30-day echocardiographic follow-up and patients treated with the MitraClip (Abbott Structural Heart). Whether rMR and MPG assessed at discharge are associated with outcomes after PASCAL mitral valve transcatheter edge-to-edge repair (Edwards Lifesciences) remains unknown.
METHODS: REPAIR (Registry of Pascal for Mitral Regurgitation) is an investigator-initi

BACKGROUND: Patients with a mechanical aortic valve replacement (AVR) require lifelong anticoagulation with warfarin. International normalized ratio (INR) goals for mechanical AVRs can change throughout treatment as risk factors change. Appropriate INR goal selection is crucial as goals that stray from evidence-based recommendations can lead to increased risk of bleeding, thrombosis or unnecessary monitoring.
OBJECTIVE: The purpose of this study was to evaluate the appropriateness of INR goals in patients with mechanical AVRs and identify opportunities for pharmacist-led intervention.

Sinus bradycardia, sinoatrial block, sinus arrest, and bradycardia-tachycardia syndrome are manifestations of intermittent or persistent sinus node dysfunctions (SNDs). SND is classified as intrinsic, when related to an anatomic disease of sinoatrial cells, or extrinsic, when bradycardia is caused by inappropriate vagal outflow. Chronotropic incompetence is often associated with SND, especially in elderly patients. When symptoms are related to SND, pacemaker implantation is the recommended therapy, and rate-adaptive pacing modes are often preferred in cases of chronotropic incompetence. Closed
