Comprehensive hearing aid program within an ear, nose, and throat office.

Hearing aid dispensing within an otolaryngological/audiological clinic is a well accepted fact, and offers total hearing care in a professional setting. The effectiveness of such a program as to wearing habits and acceptance of services rendered, to my knowledge, has not been studied. This paper discussed data obtained from a questionnaire sent out to 1100 hearing aid patients. The questionnaire was designed to study satisfaction with clinic services and costs, acceptance and adjustment, communication abilities and wearing habits. Statistical analysis was performed by the Department of Biometr

Subjective comparisons of binaural versus monaural amplification.

Patient preference for monaural versus binaural hearing aids was studied to evaluate the subjective differences noted between these two types of fittings. Subjects were 30 patients with bilateral hearing impairment who were being fit with amplification for the first time. Subjects wore both monaural and binaural hearing aids for controlled periods of 1 hr each and 2 days each. This was followed by an additional 3 days during which the fittings were alternated at the subjects' own discretion. Subjects reported preferences and subjective differences after each experimental phase. Interviews were

Myoelectric prosthetic fitting in young children.

Fitting myoelectrically controlled hand prostheses to young children has previously been considered unrealistic and even contraindicated. In 1971 the first preschool child was supplied with a myoelectric prosthesis at the Regional Hospital in Orebro, Sweden. A total of 40 young unilateral below-elbow congenital amputee children have thus far been fitted. The youngest children were 16 months of age when fitted. Such prostheses are clearly better integrated and accepted by children than by most adults. However, certain condiderations must be taken into account: the stump parameters ought to be s

Speech perception processes and fitting of hearing aids.

A model of speech perception is modified to take into account the adaptational factors involved in speech perception and learning. In connection with fitting of hearing aids, the model permits a description and identification of factors influencing the immediate and long-term benefits of a hearing aid. Various aspects of the model in connection with the rehabilitation process using hearing aids are considered and a new step-wise adjusting procedure for the assessment of hearing aids is proposed. Our observations imply that speech audiometry is a poor indicator of the long-term benefit of a hea

Prostheses, pain and sequelae of amputation, as seen by the amputee.

Results of a survey of 19 organizations belonging to World Veterans Federation indicate that major complains of amputees include; poor fitting, poor dissemination of knowledge to doctors and amputees regarding new prostheses, lack of opportunity for "input" from amputees at research level and inadequate measures to deal with phantom and stump pain. Suggested improvements by amputees; decrease in weight of prostheses, reduction in maintenance for swing and stance-phase control units development of recreational prostheses, more frequent checks through use of X-ray and film techniques, particular

The pre prosthetic management of the edentulous patient.

The problems to be solved are those largely caused by prolonged wearing of old, inadequate dentures where faulty occlusion, ill-fitting bases, poor denture hygiene, parafunctional habits combine to produce denture stomatitis and papillary hyperplasia. A programme is set out to deal with these problems and to adjust abnormal frenum and muscle attachment.

Methods for the evaluation of hearing aids.

Methods of hearing-aid evaluation are sub-divided into four broad categories, prescriptive selective amplification, selection from a set of existing hearing aids, use of a fixed frequency-gain characteristic, and adaptive fitting of a master hearing aid. The advantages, disadvantages, and controversies associated with each approach are reviewed and major research needs are discussed in the light of this review. Among the more important research needs are i) the development of improved speech tests that have low test-retest variability and which can discriminate between hearing aids with much g

Adaptive testing in audiology.

A general approach to adaptive testing of the up-down type is described. Audiological applications of the procedure are described, including measurement of the auditory threshold, estimating the spread of the subject's response curve, testing for sequential response dependencies, and finding PB-max. Guidelines are provided for the practical implementation of these procedures using relatively simple rules for controlling stimulus levels and for estimating the parameters of interest. Also described is the Simplical Method, a multivariate adaptive procedure which, like the transformed up-down met

On the problems of fitting an individual hearing aid: a case story.

For a fourteen-year old girl with a pronounced ski-slope bilateral hearing loss of 100-120 dB and with a dynamic range of approx. 10 dB, a tailor-made true binaural hearing aid was made. Establishing the acoustical characteristics of the hearing aid based on details from audiograms proved not to yield satisfactory results. Consequently an adjustment of the acoustical data according to the method of trial and error was initiated. By applying this method, a satisfactory result was obtained. The suitability of an audiogram to describe the optimum characteristics of hearing aids may thus be questi

Caries and periodontal status in patients fitted with removable partial dentures.

A 6-year follow-up study was carried out on 28 patients fitted with removable partial dentures. Prior to the prosthetic treatment all patients were given oral hygiene instruction and periodontal therapy where indicated. The removable partial dentures were carefully planned and designed. During the follow-up period the patients were examined at yearly intervals when remotivation, scalings, conservation and prosthetic adjustments were carried out as required. Patient cooperation was excellent and no significant deterioration of the periodontal status of the remaining teeth could be found. In add

A clinical evaluation of semiprecious alloys for dowels and cores.

The semiprecious metals investigated and hard dental gold alloys may be manipulated in a similar manner. The semiprecious metals produced accurate and well-fitted dowels needing minimal adjustment. Being harder than type III gold alloy, they would be ideal for casting dowels and cores. Before considering these semiprecious alloys as final restorations, however, they must be further examined for resistance to tarnish and corrosion and their marginal adaptability and burnishability.

Presbyacusis, hearing aids and aging.

This paper considers the factors of presbyacusis and age-related psychological changes in older adults in terms of their effects on the use or potential use of hearing aids as a means of improving aural communication. Data are presented on the prevalence of hearing impairment and the extent of hearing aid use in aging listeners. Four major classes of presbyacusis are described (sensory, neural, metabolic, and cochlear conduvtive) and a hearing aid prognosis is made for each type of disorder. It is shown that aging produces deficits in pure-tone thresholds, the speech reception threshold, and s

[The problem of taking dental impressions in acquired microstomia].

Two cases of severe cicatricial atresia of the oral rima with total edentia are reported. These patients present considerable problems when impressions of the dental arches have to be obtained. To make the labial tissues more elastic and in the attempt to increase rimal space, a perilabial collar of resin is made. Using the normal silicon-based synthetic elastomers, a first mould can be obtained directly in the mouth. This in its turn is filled with a more fluid elastic material and set in the mouth. From this initial impression, a chalk model can be obtained and from this an individual silico

Management of adult poliomyelitis patients in the Solomon Islands.

An account is given of a realistic rehabilitation service for the treatment of crawling adult poliomyelitic patients in an area of the developing world. A classification of the crawling patterns encountered is presented. The simple methods by which rehabilitation was carried out for 120 patients are described. Methods consisted of surgery to straighten and align the lower limbs when necessary, the fitting of inexpensive appliances, and the teaching of crutch walking. At long-term review, 80 percent of the patients had responded satisfactorily to treatment. Rehabilitation in an economically and

Nonprecious metal alloys for fixed restorative dentistry.

Non-noble alloys are available for use in the fabrication of cast restorations. These alloys are not necessarily designed for subsequent application of porcelain. Because they are promoted for use in place of the traditional gold alloys, significant changes in technique and armamentarium are necessary. Casting of acceptable clinical quality can be obtained, but not without additional equipment and personnel training. Obtaining casting of gold alloy quality on a routine basis may not be easily achieved. Casting into detail is possible (see Fig. 5), although greater surface roughness of the cast

Effect of training on the use of weight-bearing orthoses.

Weight-bearing orthoses have been disigned with the primary goal of reducing or eliminating weight bearing in skeletal structures. In evaluating various designs and adjustments of these orthoses, we found that the training of the subject in the use of his orthosis was one of the most significant factors in controlling weight bearing. Subjects fitted with instrumented ischial weight-bearing and patellar-tendon-bearing orthoses walked over a force plate which measured forces imparted to the floor. The force data from the orthoses and force plate were computer-processed with single frame, multipl
