Evaluation of deep neural network-based strategies for the compensation of sensorineural hearing loss.

Conventional hearing aid (HA) algorithms, developed based on computational and non-differentiable auditory processing models, do not typically compensate for cochlear synaptopathy (CS). Traditional HAs apply fixed or rule-based gain adjustments within predefined frequency bands and compression ratios, instead of model-based fitting that numerically optimizes processing from neural representations of normal and impaired hearing. To compensate for combined CS and outer-hair-cell (OHC) loss, deep neural network (DNN)-based closed-loop HA systems are gaining traction. Here, we present several DNN-

Association between duration of hearing-aid use and frailty in community-dwelling older adults with hearing loss in Japan: a cross-sectional study.

BACKGROUND: Frailty is prevalent among older adults with hearing loss. —a condition potentially triggering social isolation, communication difficulties, and reduced participation in physical and social activities, which in turn may accelerate the development of frailty. Hearing aids help mitigate these consequences by improving communication and social engagement. However, the relationship between duration of hearing-aid use and frailty is unclear. This study examined the above-mentioned relationship in older adults with hearing loss. METHODS: A cross-sectional survey was conducted between Jun

An Evaluation of Preset Collection Size in Over-the-Counter Hearing Aids.

OBJECTIVES: Over-the-counter (OTC) hearing aids are available as self-fitting and preset-based devices. Preset-based devices may be more accessible to older adults with limited technological literacy, but it is unclear whether users prefer settings from larger preset collections that incorporate individual preference deviations or if simpler collections based only on prescriptive targets are sufficient. This study investigated whether potential OTC hearing aid users prefer gain-frequency configurations from larger preset collections, and whether these preferences impact speech perception in qu

Economic evaluation of hearing aid use and quality of life in older adults with hearing impairment in India.

Hearing impairment is a growing public health challenge among older adults in India, with low uptake of hearing aids despite potential benefits for communication, social participation, and health-related quality of life (HRQoL). This study evaluated the cost-utility of hearing aid provision for older adults (≥60 years) with hearing impairment in India to inform policy and financing decisions. A cost-utility analysis compared hearing aid use with no hearing aid over a 3-year decision tree and a 15-year Markov model. Data were collected in 2023 from 636 participants (276 users, 360 non-users). H

Noise Management Preferences During Long-Term Hearing Aid Usage and Their Relation to Audiologic Factors.

Preferences for advanced hearing aid (HA) noise management features, such as directionality and noise reduction (DIR + NR), differ significantly among users. Due to the lack of standardized clinical guidelines for fine-tuning these features, audiologists often rely on individual user preferences. However, this approach doesn't always ensure optimal hearing outcomes. The goal of this study was to examine how users respond to these advanced features in everyday acoustic environments, with two main goals: to assess how sensitive users are to changes in DIR + NR settings compared to subtle gain ad

The Cost Effectiveness of Upgrading Cochlear Implant Sound Processors in Australia: A Novel Application of Individual-Level Microsimulation Models.

BACKGROUND: Cochlear implants (CIs) can be used to address severe to profound sensorineural hearing loss, which is more prevalent in older adults. CI system reliability depends on both internal implants and external sound processors. Over time, sound processors age and may experience failure or faults and need repairing. However, spare parts for obsolete sound processors are harder to access. Upgrading sound processors could help CI users benefit from technology improvements; however, this may be prohibitively expensive for users. This study assessed the cost effectiveness of upgrading obsolet

Self-Fitting Versus Professional-Fitting Hearing Aids: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Background: Adult hearing loss is common, yet hearing-aid uptake remains inadequate due to systemic and financial barriers in traditional audiologist-based fitting models. Over-the-counter (OTC) and self-fitting strategies have emerged to improve accessibility, allowing users to select and adjust their devices independently. However, concerns remain that reducing professional involvement may compromise electroacoustic accuracy, speech-in-noise outcomes, and overall patient satisfaction. Methods: A comprehensive literature search across PubMed, Web of Science, CENTRAL, Scopus, and Embase was co

Prospective evaluation of hearing aid fitting adjusted toward one-third functional gain in patients with sensorineural hearing loss.

OBJECTIVE: To prospectively evaluate whether modifying DSL version 5-based hearing aid (HA) fittings by adjusting gain on HA fitting software so that measured functional gain (FG) approached a one-third gain (1/3G) target could provide appropriate fitting outcomes in patients with sensorineural hearing loss. METHODS: Twenty-four patients (48 ears) with bilateral sensorineural hearing loss underwent initial HA fitting using the DSL version 5 prescription formula. FG was measured at 250-4000 Hz, and HA gain was adjusted on HA fitting software so that FG approached the target 1/3 G. Speech discr

Outcomes of real-ear measurement-based fitting in unilateral hearing aid users: Influence of contralateral hearing.

OBJECTIVES: Real-ear measurement (REM) is commonly used to verify hearing aid fittings; however, its added benefit over non-REM fitting in unilateral users and the influence of contralateral ear hearing remain unclear. In this retrospective study, we compared short-term audiometric and patient-reported outcomes between an REM-based fitting pathway and a manufacturer-first-fit pathway followed by routine clinical fine-tuning in unilateral hearing-aid users, and examined associations with contralateral-ear hearing. METHODS: We retrospectively analyzed data from 105 unilateral hearing aid users

Long-Term Electromyographic Monitoring of the Stapedius Reflex via Implanted Electrodes in Sheep: Toward Objective Autonomous Cochlear Implant Fitting.

Objective fitting measures offer a means to circumvent the subjectivity of cochlear implant programming, with the stapedius reflex representing one robust predictor of the maximum comfortable loudness level. With the present study, it was investigated whether long-term electromyographic measurements of the stapedius muscle using implanted electrodes are feasible. In nine sheep, myoelectrical activities were recorded intraoperatively and synchronized with middle ear admittance as a reference signal. For acoustic stimulation pure tones with different frequencies were used. The electrodes were pl

Influence of hearing in the non-implanted ear and age on long-term speech recognition benefit for adult cochlear implant users with asymmetric hearing loss.

Most cochlear implant (CI) users with normal hearing or mild to moderately-severe hearing loss in the non-implanted ear, known as asymmetric hearing loss (AHL), experience significant benefits in spatial hearing as compared to pre-operative abilities, though individual outcomes vary. This study analyzed the influence of the hearing in the non-implanted ear and age on long-term outcomes for 35 adult CI users with AHL. Unaided thresholds were measured behaviorally at annual post-activation visits and were used to calculate a pure-tone average (PTA; 0.5, 1, 2, 4 kHz). Sentence recognition in nois

Anatomy-based fitting for cochlear implant recipients.

PURPOSE OF REVIEW: Frequency-to-place mismatch is the discrepancy between the frequency information delivered by a cochlear implant electrode and the natural cochlear place frequency location that is stimulated. Frequency-to-place mismatch has been shown to negatively influence cochlear implant outcomes. Anatomy-based fitting (ABF) aligns filter frequencies with cochlear frequency place to minimize or eliminate frequency-to-place mismatches. This article reviews the current evidence on variability in electrode array placement, influence of frequency-to-place mismatches on cochlear implant reci

[Empowering patients through app-based cochlear implant self-adjustment].

Treatment of patients with severe hearing loss or deafness using cochlear implants (CI) is nowadays clinical routine. In the Hannover Medical School alone, more than 500 patients are treated with CI annually, meaning that the pool of patients with CI increases significantly each year. Worldwide, there are over 1 million patients with a CI system; in Germany the figure is estimated at over 60,000. With such a huge number of CI patients, centralized annual routine follow-up of all patients in the implant centers is obviously not possible in the long term. Many patients would also like to reduce
