Paradoxical reactions during treatment of miliary tuberculosis: a 25-year single-centre case series and literature review.

BACKGROUND: Paradoxical reactions during otherwise effective anti-tuberculosis treatment can resemble treatment failure or drug resistance. Their frequency and clinical spectrum in miliary tuberculosis are incompletely characterised. AIMS: To describe the timing, manifestations, management and outcomes of paradoxical reactions identified in a defined cohort of patients with miliary tuberculosis and to place these events in the context of the published literature. METHODS: We retrospectively reviewed the records of 63 patients with miliary tuberculosis managed at a single centre between 1981

Clinical Characteristics, Diagnostic Performance, and Treatment Outcomes of Extrapulmonary Tuberculosis at a Tertiary Care Center in Oman: A 10-year Retrospective Cohort.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) accounts for a substantial proportion of the global tuberculosis (TB) burden and continues to pose major diagnostic and therapeutic challenges, particularly in low-incidence settings. Comprehensive descriptions of the clinical spectrum, diagnostic yield of available laboratory modalities, and treatment-related toxicity of EPTB in the Arabian Gulf are limited. We aimed to describe the clinical characteristics, diagnostic performance of microbiological and histopathological tests, and safety profile of first-line anti-TB therapy in adult patients wi

A rare case of unusual anti-tuberculosis drug reaction in disseminated tuberculosis with pre-existing Multisystem Inflammatory Syndrome in Children (MIS-C).

Children with Multisystem Inflammatory Syndrome in Children (MIS-C) often have weakened immune systems and more prone to secondary infections such as tuberculosis (TB). MIS-C is a rare case but serious complication of COVID-19. It commonly affects the gastrointestinal, cardiovascular, hematologic, and mucocutaneous systems. This study presents a case of MIS-C in a 15-year-old girl complicated by respiratory failure, increased intracranial pressure with status epilepticus, grade II TB meningitis, severe dengue, disseminated TB, anti-tuberculosis drug induced hepatotoxicity, and cutaneous drug e

Admission systemic inflammation predicts swallowing decline in older adults with pulmonary tuberculosis.

BACKGROUND & AIMS: Tuberculosis remains a serious infectious disease worldwide. However, the status of swallowing function in older adults with pulmonary tuberculosis and the impact of systemic inflammation on swallowing decline during hospitalization have not been fully elucidated. We aimed to examine whether systemic inflammation at admission affects swallowing function decline in older patients with pulmonary tuberculosis. METHODS: This was a single-center, retrospective observational study. The inclusion criteria were patients aged ≥65 years with pulmonary tuberculosis who were admitted t

Challenges encountered in managing tuberculosis in children: a case report.

BACKGROUND: The management of pediatric tuberculosis involves several challenges, as illustrated in these three cases. Challenges are numerous and each case demonstrates the difficulty and delay in diagnosis, social constraints that delay the diagnosis, and drug dosing schedules that cause serious side effects in children. All three cases received the Bacillus Calmette-Guérin vaccine at birth, and each has documented confirmation of vaccination. CASE PRESENTATION: A 1-year-old Sri Lankan Muslim baby was transferred to a tertiary care center with a week-long fever of unknown origin, accompanie

Factors associated with severity of pulmonary tuberculosis in people living with and without HIV: a systematic review.

INTRODUCTION: Tuberculosis is a leading infectious cause of death and morbidity in people with and without HIV. Better identification of severe forms could help target early interventions to reduce mortality and severe morbidity. Existing severity scores do not fully take into account important disease characteristics such as bacillary load, extent of lung involvement, and disease dissemination, and most of them were not developed specifically for people living with HIV. We sought to identify factors associated with tuberculosis severity described in the literature in order to propose a consen

Successful treatment of pre-extensively drug-resistant tuberculous meningoencephalomyelitis in pregnancy with a bedaquiline and delamanid-based regimen: a case report.

BACKGROUND: Management of pre-extensively drug-resistant tuberculosis (pre-XDR-TB) during pregnancy poses a formidable clinical challenge, particularly when complicated by tuberculous meningoencephalomyelitis (TBMEM), While the WHO operational handbook provides a general framework for drug-resistant TB, robust evidence and specific guidelines for managing severe CNS involvement in pregnancy remain limited. CASE PRESENTATION: We report the successful outcome for a mother and infant in an extremely high-risk case. The patient, at 17 + 6 weeks of gestation following in vitro fertilization (IVF),

Genital tuberculosis initially presenting as pyometra and progressing to miliary tuberculosis: A case report.

Female genital tuberculosis is a rare and often asymptomatic condition, which makes its diagnosis challenging. Early recognition of uncommon clinical presentation of tuberculosis is crucial for the better management of it. We report herein a case of genital tuberculosis that initially presented as pyometra and subsequently progressed to miliary tuberculosis. An 86-year-old woman with a history of immunosuppressive therapy for bullous pemphigoid presented with fever and septic shock and was diagnosed with pyelonephritis. Computed tomography (CT) incidentally revealed pyometra, from which Escher

Death associated with miliary tuberculosis (Koch's disease): A case report.

Miliary tuberculosis is a disseminated form of tuberculosis (TB) that is often life-threatening. Due to its vague and diverse clinical presentation in some instances the diagnosis is only confirmed postmortem. A 42-year-old male presented to the emergency department in an altered mental state. Despite intensive management, he succumbed to cardiac arrest. An autopsy revealed disseminated granulomas in multiple organs, with Ziehl-Neelsen staining confirming miliary tuberculosis. Septicaemia was declared the cause of death, highlighting the challenge of diagnosing miliary TB antemortem. Healthcar

A 70-Year-Old Woman Receiving Immunosuppressive Therapy Presenting With Iliopsoas Abscess, Pulmonary Miliary Tuberculosis, and Secondary Hemophagocytic Lymphohistiocytosis.

BACKGROUND Miliary tuberculosis (TB) carries an increased risk in immunosuppressed patients. Extrapulmonary manifestations include hemophagocytic lymphohistiocytosis (HLH) and iliopsoas abscess. The coexistence of multiple extrapulmonary involvements can result in a significant diagnostic challenge. In the treatment of TB-associated HLH, the use of immunosuppressive agents remains controversial because of the risk of exacerbating TB. This report describes the case of a 70-year-old woman with rheumatoid arthritis presenting with an iliopsoas abscess, pulmonary miliary tuberculosis, and secondar

Invasive Fungal Infection Requiring Dif ferential Diagnosis from Miliary Tuberculosis Based on Imaging.

Invasive fungal infections (IFIs) have increased in recent years due to population aging and a rising number of immunocompromised patients. The prognosis for IFIs remains poor, making accurate diagnosis and appropriate treatment essential. Low culture positivity rates and the difficulty in distinguishing IFIs from diseases such as tuberculosis on computed tomography imaging further complicate diagnosis and may delay. We encountered a case of invasive candidiasis leading to respiratory failure, in which differentiation from miliary tuberculosis on imaging was challenging. Autopsy revealed histo

Beyond the Flare: A Case of Disseminated Tuberculosis and Thrombosis Masquerading as IBD Exacerbation.

Differentiating inflammatory bowel disease (IBD) flares from infectious complications in patients on anti-TNFα therapy presents a significant diagnostic challenge. This case report describes a 54-year-old woman with ileocolic Crohn's disease on adalimumab who presented with systemic and gastrointestinal symptoms mimicking a flare. Initial computed tomography revealed enteritis, mesenteric lymphadenopathy, and extensive inferior vena cava thrombosis. Despite treatment with corticosteroids and broad-spectrum antibiotics, the patient developed respiratory symptoms. Further evaluation confirmed di

A retrospective cohort study of tuberculosis disease between in vitro fertilisation-embryo transfer and natural pregnancy in Shenzhen China.

OBJECTIVE: This study aimed to compare the clinical profiles and maternal-foetal outcomes of tuberculosis (TB) disease in pregnant women following in vitro fertilization and embryo transfer (IVF-ET) versus natural conception (NC), and identify the associated high-risk factors. METHODS: A retrospective analysis included 133 pregnant women with TB disease, stratified into IVF-ET (n = 16) and NC (n = 117) groups. Clinical, radiological, and laboratory parameters were evaluated, and logistic regression identified risk factors for stillbirth or preterm birth. RESULTS: The IVF-ET group exhibited s

An unusual presentation of miliary tuberculosis with medullary nephrocalcinosis in an infant.

Miliary tuberculosis (MTB) is a rare and potentially fatal manifestation of Mycobacterium tuberculosis infection, resulting from hematogenous dissemination of the primary infection. Although more commonly seen in immunocompromised individuals, infants are also susceptible due to their immature immune systems. We present the case of an 88-day-old female with MTB and extrapulmonary disease involving the liver, spleen, kidneys, and retina. Imaging demonstrated diffuse pulmonary nodules, hepatic and splenic granulomas, and nephrocalcinosis. This case highlights the hallmark radiologic features see
