Peritoneal Tuberculosis Mimicking Cancerous Peritonitis With Elevated Serum Cancer Antigen 125 and Carbohydrate Antigen 19-9 Levels: A Case Report.

Peritoneal tuberculosis (PTB) is a rare extrapulmonary manifestation that poses diagnostic challenges due to nonspecific clinical features and can mimic carcinomatous peritonitis, particularly in patients with ascites and elevated serum cancer antigen 125 (CA125) and carbohydrate antigen 19-9 (CA19-9) levels. We report a 75-year-old woman with rheumatoid arthritis who was receiving certolizumab and developed massive ascites, elevated tumour markers, and pulmonary nodules. Laparoscopy revealed numerous miliary yellow-white nodules in the parietal peritoneum, and biopsy showed epithelioid granul

Clinical and CT Features of HIV-Negative and HIV-Positive Patients with Abdominal Tuberculous Lymphadenopathy.

Background: The diagnosis of abdominal tuberculous lymphadenopathy (ATBL) remains challenging in clinical practice. Patients with ATBL and HIV infection may have atypical clinical and computed tomography (CT) features. The aim of this study was to investigate the impact of HIV infection on the clinical and CT features of ATBL patients. Methods: From January 2012 to March 2023, 178 patients with untreated ATBL were retrospectively analyzed. Patients with ATBL were classified into HIV-negative group (n = 152) and HIV-positive group (n = 26). In addition to the clinical characteristics of the pat

Toxoplasmic encephalitis unmasked during treatment for miliary tuberculosis in a patient with human immunodeficiency virus infection: a case report and literature review.

BACKGROUND: Toxoplasmic encephalitis (TE) and tuberculoma are the leading causes of ring-enhancing brain lesions in patients with human immunodeficiency virus (HIV) infection. Because imaging findings of the two conditions overlap and cerebrospinal fluid (CSF) tests lack sensitivity, timely diagnosis is critical. Herein, we report the rare case of a patient with HIV infection and miliary tuberculosis who developed intracranial mass lesions after antiretroviral therapy (ART) initiation with high-dose prednisolone administration. CASE PRESENTATION: A 53-year-old Nepalese man who had been living

Tuberculosis-triggered cytokine storm with hemophagocytic lymphohistiocytosis and tuberculous spondylitis in an apparently immunocompetent host: a case report and literature review.

Hemophagocytic lymphohistiocytosis (HLH) secondary to disseminated tuberculosis (TB) is a rare, life-threatening hyperinflammatory syndrome. We present a 60-year-old man with recurrent fever and syncope. Workup revealed cytopenias, hyperferritinemia (peak 5,802 ng/mL), elevated C-reactive protein (CRP), and hepatic dysfunction, fulfilling HLH-2004 criteria. Imaging showed miliary lung nodules and tuberculous spondylitis at T9. Bone marrow biopsy confirmed hemophagocytosis, and next-generation sequencing identified Mycobacterium tuberculosis. This case demonstrates that disseminated TB can trig

BCGitis and BCGosis: Clinical Spectrum, Immunological Mechanisms, and Risk Management.

Bacille Calmette-Guérin (BCG) remains the only licensed vaccine against tuberculosis (TB), administered to >100 million neonates annually. It confers approximately 70-80% protection against tuberculous meningitis and miliary TB in early childhood, under-pinning its continued use in high-burden settings. As a live-attenuated vaccine, however, BCG can rarely cause adverse reactions ranging from self-limited local lesions to life-threatening disseminated BCG disease (BCGosis), which almost exclusively occurs in infants with severe primary or acquired immunodeficiencies such as SCID, MSMD, CGD, or

Disseminated tuberculosis complicated by ARDS in a third-trimester healthcare worker.

Miliary tuberculosis (TB) in pregnancy presents unique diagnostic and therapeutic challenges due to overlapping physiological changes, imaging limitations, and fetal considerations. We describe a 42-year-old third-trimester healthcare worker who presented with prolonged fever and rapidly progressive hypoxaemia. Initial investigations were inconclusive, and imaging showed subtle reticulonodular opacities. Her condition worsened into severe ARDS, requiring emergency caesarean delivery and mechanical ventilation. Placental histopathology revealed necrotizing granulomatous inflammation, and fundos

Tuberculosis notifications among children and young adolescents, 2008-2019 compared to 2020-2023, Brazil.

OBJECTIVE: To evaluate the impact of the coronavirus disease 2019 (COVID-19) pandemic on tuberculosis-related notifications and mortality in Brazilian children and young adolescents aged 0-14 years. METHODS: We conducted an ecological study using interrupted time series analysis with data from DATASUS, a nationwide open-access database of the Brazilian health ministry. We compared the notification rates of overall active tuberculosis, laboratory-confirmed tuberculosis, miliary and meningeal tuberculosis and tuberculosis-related deaths between the pre-pandemic (January 2008 to February 2020) a

Delayed Pulmonary Manifestations of Miliary Tuberculosis Following a Normal Initial High-Resolution Computed Tomography.

Miliary tuberculosis (TB) is a potentially fatal form of TB resulting from the widespread dissemination of Mycobacterium tuberculosis. Although the presence of pulmonary miliary infiltrates usually facilitates diagnosis, atypical cases lacking these characteristic imaging findings frequently confound clinicians. Here, we describe the case of an 82-year-oldJapanese woman with miliary TB who initially presented with fever but showed no abnormalities on high-resolution computed tomography (HRCT) and microbiological tests; hence, fever of unknown origin was diagnosed. The delayed appearance of mil

Miliary Nodules in the Lungs Not Always Due to Tuberculosis: A Report of a Rare Case.

Allergic bronchopulmonary aspergillosis (ABPA) is a disease that occurs due to a pulmonary immune system reaction to the antigens of Aspergillus Fumigatus. Central bronchiectasis, mucoid impaction, mosaic attenuation, centrilobular nodules, and tree-in-bud opacities are the most commonly observed CT scan findings of ABPA. Miliary molting is one of the rare radiological manifestations of ABPA. ABPA is usually seen in uncontrolled asthma and is very uncommon in patients without a prior history of asthma. We are reporting a case of ABPA that presented a miliary nodule in radiology and was never d

Systemic Lupus Erythematosus Vasculitis Causing Perforation Peritonitis in Miliary Tuberculosis: A Disease in Disguise.

Systemic lupus erythematosus is a complex disease to manage and is complicated further by coexisting comorbidities. We present the case of a 25-year-old female patient who arrived at the emergency department with complaints of abdominal pain for three days, accompanied by a history of fever and non-bilious vomiting for two days. She had no history of trauma or chronic use of painkillers. She was diagnosed with pulmonary and abdominal tuberculosis (TB) four months previously and has been on anti-tubercular therapy since then. Anterior segment mass in miliary tuberculosis: a confusing scenario.

Tuberculosis (TB) is one of the most prevalent diseases in the Indian subcontinent. Ocular TB presenting as iris mass is rare; furthermore, invasion of anterior segment without choroidal invasion is even more scarce. Presented here is a case report detailing the condition of a patient in late adolescence who presented with diminution of vision and pain in the left eye. Slit lamp biomicroscopy revealed a nodular vascular mass emerging from the iris. Subsequently, the patient underwent a fine needle aspiration biopsy from the anterior chamber, which yielded a positive result for acid-fast bacill

Isolated nodular hepatic tuberculosis in an immunocompetent patient following dexamethasone treatment for COVID-19.

Hepatic tuberculosis is a rare extrapulmonary manifestation of pulmonary or miliary tuberculosis, typically seen in immunocompromised individuals. Isolated nodular hepatic tuberculosis is exceptionally uncommon in immunocompetent individuals and often mimics other hepatic lesions such as hepatocellular carcinoma. We present a 35-year-old male who developed isolated nodular hepatic tuberculosis following high-dose dexamethasone treatment for COVID-19. Imaging and fine-needle aspiration cytology suggested hepatocellular carcinoma, prompting surgical hepatic segmentectomy. Postoperative histopath

Epidemiological and clinical analysis, and outcomes of tuberculosis co-infection among people living with HIV in Türkiye (2014-2024) ClinSurv HIV cohort: A large case series.

BACKGROUND: Tuberculosis (TB) is one of the most common opportunistic infections in people living with HIV (PLHIV). Mycobacterium tuberculosis may cause more TB in all stages of HIV infection than in the general population, with the incidence of TB and the spread of pulmonary TB to other organs increasing as the CD4 count decreases. OBJECTIVE: In this HIV cohort study, we aimed to evaluate the clinical features, diagnosis, and prognosis of TB among PLHIV in Türkiye. MATERIALS AND METHODS: We conducted a retrospective cohort study to analyze clinical outcomes and identify determinants of mort

Miliary Lung Lesions Mimicking Tuberculosis: A Case of Metastatic Lung Adenocarcinoma.

Miliary patterns on chest imaging frequently prompt urgent consideration of disseminated infectious diseases such as tuberculosis (TB), particularly in patients from endemic regions. However, non-infectious aetiologies like metastatic malignancy must also be considered. We report the case of a 39-year-old previously healthy male with miliary pulmonary lesions initially treated empirically for disseminated TB. Despite therapy, his condition deteriorated until biopsy results confirmed metastatic lung adenocarcinoma. This case highlights the diagnostic challenges in TB-endemic regions and undersc

Spontaneous Resolution of Miliary Pulmonary Nodules Following Intravesical Bacillus Calmette-Guérin (BCG) Therapy: A Case Report and Literature Review.

Intravesical Bacillus Calmette-Guérin (BCG) immunotherapy is a well-established treatment for non-muscle-invasive bladder cancer. Although it is typically associated with local irritative symptoms, rare systemic and pulmonary complications can occur, including hypersensitivity pneumonitis and miliary tuberculosis. We report the case of a 70-year-old man who developed diffuse miliary pulmonary micronodules and ground-glass opacities after his 12th BCG instillation. Despite imaging findings suggestive of disseminated infection, the patient remained clinically stable, without fever, hypoxia, or s

Metastatic Lung Adenocarcinoma Mimicking Miliary Tuberculosis: A Case Report.

Lung adenocarcinoma (LA) can present with a wide range of morphological patterns and may mimic disseminated infectious diseases such as miliary tuberculosis (TB), posing significant diagnostic challenges and potentially delaying appropriate treatment. We report a case of metastatic pulmonary adenocarcinoma that was initially misdiagnosed as miliary TB. A 35-year-old nonsmoking male presented with a progressive dry cough and shortness of breath. Chest imaging revealed diffuse bilateral micronodules and a pericardial effusion, raising suspicion for miliary TB. Although acid-fast bacilli smears a

Barriers to Care Leading to Fatal Consequences: A Case of Tuberculosis Sepsis Amid Policy Change.

Mycobacterium tuberculosis is an acid-fast bacterium with a diverse range of clinical manifestations and is considered rare in Western countries. Tuberculosis (TB) remains a global health concern and can pose several challenges in diagnosis and treatment. Paired with changing immigration policies, immigrant populations can face several barriers to healthcare. This case illustrates the impact of immigration policy on clinical outcomes and how government policies can act as a barrier to care. We present a case of a 26-year-old Guatemalan man with no known past medical history who came to the eme

Isolated splenic tuberculosis mimicking a solid splenic neoplasm in an immunocompetent patient: A surgical case report.

INTRODUCTION AND IMPORTANCE: Tuberculosis is the leading cause of death by an infectious agent globally, especially among immunocompromised patients. Extrapulmonary tuberculosis (EPTB) can occur as a sequela of pulmonary tuberculosis through lympho-hematogenous or miliary spread, or can occur in isolation. EPTB rarely occurs in isolation in a single organ, especially in immunocompetent patients. Limited data exist on the incidence of isolated splenic tuberculosis in immunocompetent patients. CASE PRESENTATION: This report highlights a case of a 45-year-old immunocompetent female with a one-mo
