INTRODUCTION: Exposure to prolonged microgravity is associated with cephalad fluid shifts that potentially could affect tympanomastoid ventilation and auditory mechanisms. We conducted a retrospective review of NASA astronaut on-orbit hearing assessments (OOHAs) and middle-ear pressure measurements obtained during International Space Station missions to search for evidence of spaceflight-related hearing changes and disturbances of middle-ear aeration.
METHODS: Nine NASA astronauts conducted on-orbit hearing assessments and tympanometry at least twice while on space station missions. Kuduwave

BACKGROUND: Cholesteatoma is a progressive disease characterized by abnormal proliferation of keratinizing squamous epithelium within the temporal bone. Although both congenital and acquired forms have been recognized, the molecular mechanisms underlying its development remain poorly understood. Although surgery is the only curative option, the high recurrence rates underscore the need to identify genetic risk factors. The rs17822931 (538G > A, G180R) polymorphism in the ABCC11 gene has been associated with cholesteatoma risk in the Japanese population. However, ethnic variation in the frequen

Safety of hearing devices attached to the tympanic membrane depends on applied forces and resulting movements of the ossicles when pulling off the device. A robotized temporal bone measurement setup was designed to apply and measure defined pull-off forces and as well as the corresponding displacements of the round-window membrane via Laser-Doppler-vibrometry. Here, we investigate in five temporal bones the pull-off dynamics of the Vibrosonic Hearing Contact Lens® the silicone membrane of which being formed to match the tympanic-membrane shape of individual temporal bones. INTRODUCTION: Various couplers exist for the Vibrant Soundbridge implant to connect the actuator to a middle ear structure. While the surgical indication mainly distinguishes between purely sensorineural hearing loss and mixed hearing loss, and audiological indication ranges are designed accordingly, we believe that the biomechanical distinction between forward and reverse cochlear stimulation is more meaningful. To evaluate this, we apply all common couplers in temporal bone experiments and correct the measurements for adequate comparability.

Erosive middle ear disease (EMED) involves a spectrum of otologic conditions that involve chronic inflammatory processes, including chronic otitis media with or without cholesteatoma, that contribute to osseous erosion of the middle ear and ossicular chain. Treatment of EMED involves the eradication of the disease followed by ossicular chain reconstruction to optimize hearing outcomes. There are many challenges regarding ossiculoplasty in EMED, including the potential for microscopic residual disease, variability of ossicular chain status following disease eradication, and concern for recurren

Ossicular chain defects are commonly encountered in otologic practice. The search for the ideal ossicular psosthesis is an ongoing debate. In this article, we describe our experience with the use of autologous cartilage tympanoplasty. Septal floor cartilage is harvested via the Killian incision and remodelled to fit the various ossicular defects identified in the middle ear. We describe the procedure from harvesting of cartilage through to ossiculoplasty. Different scenarios are described, as encountered intra operatively. We have not experienced extrusion when using this material. The cartila

The middle ear amplifies sound waves transmitted through air, allowing for sufficient sound pressure to be received in the cochlea through a few interesting mechanisms of the tympanic membrane and ossicular chain, which is composed of the malleus (hammer), incus (anvil), and stapes (stirrup). We discuss how these structures serve as an acoustic conduit, alleviating the large impedance mismatch between air and fluid; and we provide an overview of the anatomy, physiology, and biomechanics of the middle ear, which are important concepts relevant to ossiculoplasty. By combining surgical expertise

The surgical approach for ossiculoplasty, also known as ossicular chain reconstruction, is chosen based on the extent of concomitant middle ear or mastoid disease, such as chronic otitis media or cholesteatoma. The primary objective of the surgical approach is to adequately expose the middle ear ossicles, diagnose the ossicular chain pathology, and facilitate reconstruction. Surgeons may employ microscopes and/or endoscopes for visualization and access the ossicular chain through a variety of surgical corridors including the transcanal, endaural, transmeatal, postauricular, and transmastoid/tr

INTRODUCTION: In our previous report, we described a surgical technique for cochlear implantation in canal wall down (CWD) ears. Specifically, this involved extending the endaural incision superiorly, harvesting a large temporal fascia flap, completely removing the tympanic epithelium, inserting the electrode array, obliterating the middle ear cavity with the temporal fascia flap, and sealing the external auditory canal with a tragal cartilage plug. This report details the safe application of this technique in a patient with severe ear malformation and suspected CHARGE syndrome, supplemented w

PURPOSE: Cochlear implantation (CI) significantly improves hearing in individuals with severe to profound sensorineural hearing loss. However, variations in surgical techniques may affect middle and inner ear mechanics. This study explores the effect of transcranial transcanal wall-Veria technique for CI on middle ear mechanics, measured using wideband absorbance (WBA).
METHODS: Ten children (4-7 years) who underwent CI using the transcranial Transcanal wall-Veria technique were compared to age- and gender-matched controls with normal-hearing children. Tympanometric measures such as static co
