PACT: A practice-driven predictive algorithm for customized transradial prosthetic socket design.

Well-fitting sockets are crucial for successful upper-limb prosthesis use, yet current digital socket design workflows are not standardized and demand considerable clinician effort. In this study, we introduce the Predictive Algorithm for Customized Transradial Socket Design (PACT), which generates socket models from a 3D limb scan. It works by retrieving the most similar limb-socket pair from a reference library of prosthetist-designed prosthetic sockets and applying isotropic and anisotropic scaling adjustments to match the input limb. To validate the algorithm, the PACT-predicted sockets fo

"Ozempic Limb": Understanding the Impact of Semaglutide on Prosthesis Use and Residual Limb Care in Individuals With Lower Limb Amputation.

Glucagon-like peptide 1 receptor agonists (GLP-1 RAs) are well-known medications commonly prescribed for type 2 diabetes management and pharmacologic management of obesity. While GLP-1 RAs have many positive effects on glycemic control and cardiovascular health, the secondary effect of weight loss can present a challenge in maintaining optimal prosthesis fitting and functionality among individuals with prosthetic limbs. More specifically, weight loss can lead to decreased muscle mass and volume of the residual limb, resulting in loosening of the prosthesis. In turn, individuals often require m

Nursing Management of Acute Airway Hyperresponsiveness and Stenosis in a Pediatric Patient Undergoing Adjustable Spinal Device Surgery: A Case Report.

Pediatric adjustable spinal device surgery, while generally low-risk, can pose severe perioperative airway challenges. This case report details the first documented instance of a 6-year-old girl developing acute airway hyperresponsiveness and severe extrinsic tracheal stenosis following sequential spinal rod adjustments. Adhering to CARE guidelines, we describe a multidisciplinary nursing management strategy that successfully addressed this life-threatening dual pathology. Key interventions included protocol-driven management of bronchospasm with deep sedation and bronchodilators, early recogn

Development and Clinical Feasibility of a Computer-Assisted Shape Design and Bending System for Spinal Fixation Rods.

BACKGROUND Spinal pedicle screw-rod fixation is widely used, but manual rod bending is time-consuming and prone to contour inconsistencies. Although computer-assisted technologies have been explored, existing systems remain limited by cost, complexity, and insufficient precision. This study evaluated a novel computer-assisted rod design and bending system, with the goal of testing whether computer-assisted rod bending reduces bending plus installation time vs manual bending in unilateral paired cases and to evaluate safety, feasibility, and rod-screw fitting accuracy. [Treatment with TOPS for short femoral stump].

OBJECTIVE: Treatment with transcutaneous osseointegrated prosthesis systems (TOPS) for short femoral amputation stumps aims to restore independent walking ability after proximal femoral amputation by direct bone-guided prosthesis anchorage. This cannot be safely achieved with conventional socket prostheses due to the mechanically inadequate socket contact surface. INDICATIONS: Treatment of patients with short transfemoral stumps who cannot be mobilized sufficiently with conventional socket prostheses. CONTRAINDICATIONS: Known contraindications as with standard TOPS fittings. SURGICAL TECHNI

Appropriate compression values for the transtibial prosthesis using the AERO prosthetic liner.

Positive model rectification is a key factor in providing a comfortable and successful transtibial prosthetic socket fitting. This study aimed to assess whether a 4% compression value in 3- and 5-mm thickness affordable ethylene-vinyl acetate roll-on (AERO) liners yields acceptable comfort and pressure distribution for transtibial prosthetic users. Stump socks were used to adjust volume in the fitting process with a socket compressed to 4%, and pressure data were collected using 6 force-sensing resistor (FSR400) sensors placed in areas sensitive and tolerant to pressure. Peak pressure was coll

Methods to assess lower limb prosthetic adaptation: a systematic review.

BACKGROUND: Lower limb amputation is a disabling condition with serious psychological, physical, and functional consequences. The adaptation of a prosthetic device can either mitigate or exacerbate these effects. Although many individuals receive lower limb prostheses, rejection rates remain high. Furthermore, while numerous objective and quantitative methods are available to assess the interface between the residual limb and the prosthetic socket, as well as the device performance, prosthetic fitting largely relies on prosthetists' observation and expertise. Accordingly, this review describes

A Clinical Tuning Framework for Continuous Kinematic and Impedance Control of a Powered Knee-Ankle Prosthesis.

Objective: Configuring a prosthetic leg is an integral part of the fitting process, but the personalization of a multi-modal powered knee-ankle prosthesis is often too complex to realize in a clinical environment. This paper develops both the technical means to individualize a hybrid kinematic-impedance controller for variable-incline walking and sit-stand transitions, and an intuitive Clinical Tuning Interface (CTI) that allows prosthetists to directly modify the controller behavior. Methods and procedures: Utilizing an established method for predicting kinematic gait individuality alongside

Quantifying dynamic coupling and fit degeneration of transtibial prosthetics using 3D motion capture.

BACKGROUND: Assessing the fit and suitability of lower limb prosthetics is heavily reliant upon clinician observation and subjective feedback, often resulting in negative impacts on the comfort and quality of life of amputees. This is particularly important in the early stages of new socket fitment. OBJECTIVE: This paper aims to use optoelectronic motion capture to measure the 5 degree of freedom coupling effectiveness of a newly fitted prosthetic limb during the adjustment period (6 weeks) of an active transtibial amputee. METHODS: A 2-part calibration and trial data collection system was e

Immediate effects of ankle-foot orthosis on gait coordination and kinematics in subacute stroke.

BACKGROUND: Ankle-foot orthoses (AFOs) are commonly prescribed after stroke, yet their immediate effects on bilateral hip-knee coordination and joint-specific contributors to functional gains are unclear. RESEARCH QUESTION: What are the immediate effects of AFO use on bilateral coordination and joint-level kinematics, and which changes relate to performance gains? METHODS: Forty-eight individuals with subacute stroke (mean age 57.9 ± 13.9 years; 28 men/20 women; FAC median 3.0) performed overground 10-m trials under pre-AFO and immediate post-AFO conditions (no acclimatization); eight had tw

Clinical outcomes following treatment of extracapsular hip fractures with long compared with short cephalomedullary nails.

AIMS: The use of cephalomedullary nails for the fixation of extracapsular hip fractures has increased in recent years, but high-quality evidence to guide surgical decision-making between long and short cephalomedullary nails is lacking. The aim of this study was to compare the quality of life (QoL) and the risks of mortality and complications in patients treated with long and short cephalomedullary nails. METHODS: The World Hip Trauma Evaluation (WHiTE) study was a multicentre, prospective cohort study that collected data from patients aged ≥ 60 years who received operative treatment for thei

A digital process to replicate the socket-foot alignment in below-knee running specific prostheses.

A well-fitting socket and a fine-tuned foot alignment are crucial elements in a running-specific prosthesis to allow Paralympic athletes with below-knee amputation to express their full competitive potential. For this reason, once a satisfactory socket-foot configuration is established after dynamic alignment, it is fundamental to reproduce the same conditions when constructing the definitive carbon fiber socket, and when renewing or constructing a back-up prosthesis, without dismantling the original. In addition, to cope with emerging needs of the athlete, it would be beneficial to implement

An adaptive prosthetic socket for people with transtibial amputation.

It is essential that people with limb amputation maintain proper prosthetic socket fit to prevent injury. Monitoring and adjusting socket fit, for example by removing the prosthesis to add prosthetic socks, is burdensome and can adversely affect users' function and quality-of-life. This study presents results from take-home testing of a motor-driven adaptive socket that automatically adjusted socket size during walking. A socket fit metric was calculated from inductive sensor measurements of the distance between the elastomeric liner surrounding the residual limb and the socket's inner surface

Prosthetic fitting and mortality after major lower extremity amputation.

BACKGROUND: Studies suggest that ambulation after major lower extremity amputation (LEA) is low and mortality after LEA is high. Successful prosthetic fitting after LEA has a significant quality of life benefit; however, it is unclear if there are benefits in post-LEA mortality. Our objective was to examine a contemporary cohort of patients who underwent LEA and determine if there is an association between fitting for a prosthetic and mortality. METHODS: We reviewed all patients who underwent LEA between 2015 and 2022 at two academic health care systems in a large metropolitan city. The expos
