The mechanical performance of ambulation using spring-loaded axillary crutches. A preliminary report.

Crutches with the extension post replaced by a spring-loaded post were compared with standard axillary crutches with respect to stresses developed during the stance phase of locomotion. Seven subjects were tested using both standard and spring-loaded crutches. The crutches were fitted with strain gauges; crutch length and grip location were adjusted for each subject using accepted standards. Subjects ambulated using a three-point "swing-through" gait pattern while strain gauge data were recorded. Spring-loaded axillary crutches had 22% smaller shock waves upon initial contact and 24% smaller p

In-shoe casting technique for specialized sports shoes.

The use of vacuum casting to create better-fitting orthoses for special athletic shoes has been discussed. By vacuum-forming a negative cast within the confines of special athletic shoes, such as bicycling cleats or fencing shoes, a better-fitting orthosis can be fabricated with little or no special adjustments or modifications necessary to create the best fit and function.

Analytical description of minimum energy expenditure surfaces.

Mechanical energy expenditure during level walking was evaluated and graphed for two unilateral, below-knee amputees over time and a range of adjustments of the flexion-extension alignment angle. The resulting mechanical energy surfaces were then least-squared fitted with an analytical function that was linear in time and quadratic in flexion-extension alignment angle. The least-squares analysis showed that there was a flexion-extension adjustment that minimized the mechanical energy expenditure and that this optimal adjustment was very close to the design point set by certified prosthetists.

Function after lower limb amputation.

Functional ability and social dependence were investigated by personal interview of 107 lower limb amputees surviving 1-5 years postoperatively. Among eight independent variables studied by multiple regression analysis, increased age was associated unfavorably with physical ability and social dependence. Independence from social provisions preoperatively showed favorable relationships with functional capacity and postoperative dependence. Above-knee or bilateral amputation and postoperative pain were associated with reduced functional ability, but not with social dependence. No significant ass

Alignment of lower-limb prostheses.

Alignment of a prosthesis is defined as the position of the socket relative to the other prosthetic components of the limb. During dynamic alignment the prosthetist, using subjective judgment and feedback from the patient, aims to achieve the most suitable limb geometry for best function and comfort. Until recently it was generally believed that a patient could only be satisfied with a unique "optimum alignment." The purpose of this systematic study of lower-limb alignment parameters was to gain an understanding of the factors that make a limb configuration or optimum alignment, acceptable to

Mobile arm supports: criteria for successful use in muscle disease patients.

Twenty-nine subjects who had been fitted with mobile arm supports 6 months to 11 years prior to the initiation of this study were evaluated to determine criteria for successful use of such supports. All subjects were diagnosed as having Duchenne muscular dystrophy or spinal muscular atrophy. Three methods were used to collect data: 1) review of medical charts, 2) interview with patient and family member(s), and 3) observation of the patient in his wheelchair for sitting stability, adjustment and control of the mobile arm support, and speed and efficiency in self-feeding with and without the su

Temporary above-knee prostheses and training programme during chemotherapy.

A temporary prosthesis has been developed for above-knee amputees who receive long-term post-amputation chemotherapy. The temporary prosthesis has an adjustable laminated quadrilateral socket, the size of which is adjusted by metal screws. Fifteen patients were fitted with the temporary prosthesis. Initial fittings were carried out after a period averaging 46 days from amputation. All of the patients were able to walk with one crutch after about one month from initial fitting. Although patients often had to discontinue their prosthetic training owing to chemotherapy, they could resume wearing

[A simple device for attaching a halo].

A fitting device is described which serves to adjust a halo; thus its application may be carried out by the surgeon alone without assistance. A modification of the device is described which may be used for fixation of a patient's head during surgery in the operating theater. With two exceptions all parts of the device are standard products of industrial or medical instrument manufacturers. Reference 4 lists these parts and describes the specially designed components. Thus, the device can easily be copied.

Prosthetic programme after above-knee amputation in children with sarcomata.

A programme for early mobilisation using a temporary prosthesis was evaluated in 17 children who had had an amputation above the knee for sarcomata. The temporary prosthesis had a performed adjustable polypropylene quadrilateral socket which was able to accommodate changes in the size of the stump during the first few months after amputation. The adjustable sockets were assembled onto wooden knee-shank-foot units or onto modular components covered with foam. The wooden units were better for routine use as more adjustment was possible between the socket and the knee and because they were more d

Myoelectric prosthetic fitting in young children.

Fitting myoelectrically controlled hand prostheses to young children has previously been considered unrealistic and even contraindicated. In 1971 the first preschool child was supplied with a myoelectric prosthesis at the Regional Hospital in Orebro, Sweden. A total of 40 young unilateral below-elbow congenital amputee children have thus far been fitted. The youngest children were 16 months of age when fitted. Such prostheses are clearly better integrated and accepted by children than by most adults. However, certain condiderations must be taken into account: the stump parameters ought to be s

Prostheses, pain and sequelae of amputation, as seen by the amputee.

Results of a survey of 19 organizations belonging to World Veterans Federation indicate that major complains of amputees include; poor fitting, poor dissemination of knowledge to doctors and amputees regarding new prostheses, lack of opportunity for "input" from amputees at research level and inadequate measures to deal with phantom and stump pain. Suggested improvements by amputees; decrease in weight of prostheses, reduction in maintenance for swing and stance-phase control units development of recreational prostheses, more frequent checks through use of X-ray and film techniques, particular

Management of adult poliomyelitis patients in the Solomon Islands.

An account is given of a realistic rehabilitation service for the treatment of crawling adult poliomyelitic patients in an area of the developing world. A classification of the crawling patterns encountered is presented. The simple methods by which rehabilitation was carried out for 120 patients are described. Methods consisted of surgery to straighten and align the lower limbs when necessary, the fitting of inexpensive appliances, and the teaching of crutch walking. At long-term review, 80 percent of the patients had responded satisfactorily to treatment. Rehabilitation in an economically and

Effect of training on the use of weight-bearing orthoses.

Weight-bearing orthoses have been disigned with the primary goal of reducing or eliminating weight bearing in skeletal structures. In evaluating various designs and adjustments of these orthoses, we found that the training of the subject in the use of his orthosis was one of the most significant factors in controlling weight bearing. Subjects fitted with instrumented ischial weight-bearing and patellar-tendon-bearing orthoses walked over a force plate which measured forces imparted to the floor. The force data from the orthoses and force plate were computer-processed with single frame, multipl
