[Transtibial amputation].

OBJECTIVE: To make a transtibial stump as long as possible, free from local and phantom pain with a maximum of terminal load bearing. In order to preserve the knee joint, an ultra-short tibial stump of 5-6 cm may be indicated. INDICATIONS: A hindfoot amputation level is not possible to achieve. CONTRAINDICATIONS: In amputations for peripheral vascular diseases, amputations through the distal third of the tibia are not recommended. If they still heal, the level selection might have been too proximal. SURGICAL TECHNIQUE: According to Verduyn and Burgess, a long posterior muscular flap coverin

Effect of peroneal electrical stimulation versus an ankle-foot orthosis on obstacle avoidance ability in people with stroke-related foot drop.

BACKGROUND: Walking ability of people with foot drop in the chronic phase after stroke is better with functional electrical stimulation (FES) of the peroneal nerve than without an orthotic device. However, the literature is not conclusive on whether peroneal FES also is better than an ankle-foot orthosis (AFO) in this regard. OBJECTIVE: This study aimed to identify potential benefits of peroneal FES over an AFO with respect to the ability to negotiate a sudden obstacle. DESIGN: The study design was a within-subject comparison between FES and AFO using repeated measures. METHODS: Twenty-four

Anatomic fit of six different radial head plates: comparison of precontoured low-profile radial head plates.

PURPOSE: Bulky implants may lead to symptomatic soft tissue irritation after open reduction and internal fixation of radial head and neck fractures. The purpose of our study was to compare the anatomic fit of precontoured radial head plates. METHODS: We stripped 22 embalmed human cadaveric radiuses of soft tissues. We investigated 6 radial head plates: (1) the Medartis radial head buttress plate (MBP), (2) the Medartis radial head rim plate (MRP), (3) the Synthes radial neck plate (SNP), (4) the Synthes radial head plate (SHP), (5) the Acumed radial head plate (AHP), and (6) the Wright radial

[Splints in birth-related brachial plexus injuries].

Most cases of obstetrical brachial plexus palsies are mild traction injuries which resolve under physical therapy within several weeks or months. Severe ruptures or avulsion injuries of the plexus can lead to lifelong impairment of the upper extremities. Hence, in severe brachial plexus injuries the indications for brachial plexus reconstruction should be evaluated, early. At the age of about 3 months, the infant should be presented in a centre specialised in obstetrical brachial plexus palsies. In almost all cases intensive physical therapy is performed. In addition, many patients require spl

Trinity amputation and prosthesis experience scales: a psychometric assessment using classical test theory and rasch analysis.

OBJECTIVE: To perform a detailed psychometric analysis using both classical test theory and Rasch analysis of the three main scales of the Trinity Amputation and Prosthesis Experience Scales (TAPES) in people with a lower-limb amputation. DESIGN: A sample of 498 persons who were prosthesis users with a lower-limb amputation was retrospectively studied, pooled from a number of studies undertaken across the United Kingdom and Ireland in the past decade in which the TAPES had been completed as part of a postal survey. Both factor analysis techniques and Rasch analysis were performed on TAPES dat

The Femoral Epicondylar Frame to track femoral rotation in optoelectronic gait analysis.

Relative movement of skin markers to underlying bone limits a valid interpretation of axial femorotibial rotation in noninvasive optoelectronic gait analysis. A distal femoral clamp is a practical solution for thigh marker placement, however, existing devices are still susceptible to measurement errors at increased angles of knee flexion. We developed the Femoral Epicondylar Frame (FEF), which should result in less femoral rotational measurement error due to its anatomic fitting and controlled pressure adjustment. Seven subjects with a total knee replacement in situ, mean age 71 years, mean bo

Magnetic mini-mover procedure for pectus excavatum II: initial findings of a Food and Drug Administration-sponsored trial.

PURPOSE: The Magnetic Mini-Mover Procedure (3MP) uses a magnetic implant coupled with an external magnet to generate force sufficient to gradually remodel pectus excavatum deformities. This is an interim report of the evolution of the 3MP during a Food and Drug Administration-approved clinical trial. METHODS: After obtaining Institutional Review Board approval, we performed the 3MP on 10 otherwise healthy patients with moderate to severe pectus excavatum deformities (age, 8-14 years; Haller index >3.5). Operative techniques evolved to improve ease of implantation. Patients were evaluated mont

Memory and executive function predict mobility rehabilitation outcome after lower-limb amputation.

PURPOSE: Post-amputation rehabilitation is physically and cognitively demanding. Understanding which specific cognitive domains mediate outcome is critical to the development of interventions. METHOD: A cohort undergoing post-amputation rehabilitation was assessed before limb fitting and followed up at 6 months (n = 34). The average age was 60.69 years (SD = 13.98). 82.4% of the sample was male. 79.4% had amputations because of peripheral arterial disease. Memory, visuospatial function, executive function, praxis, emotion and language were assessed at Time 1 (first prosthetic clinic attendanc

Inpatient rehabilitation of survivors of purpura fulminans with multiple limb amputations: a case series.

Purpura fulminans (PF) is a rare, rapidly progressive syndrome characterized by intravascular thrombosis and necrosis of the skin and subcutaneous tissues. Survivors are often left with considerable impairments, including organ dysfunction, adjustment disorder, extensive wounds, and amputations. This retrospective case series presents 4 adult patients who underwent multiple limb amputations secondary to acute infectious PF. All patients underwent acute inpatient rehabilitation, were fitted with prostheses during their stay, and achieved a minimal assistance level for most of their self-care an

Comparison of vacuum-formed removable rigid dressing with conventional rigid dressing after transtibial amputation: similar outcome in a randomized controlled trial involving 27 patients.

BACKGROUND AND PURPOSE: Following transtibial amputation, a rigid dressing of plaster of Paris has been reported to have advantages over a soft dressing regarding wound healing and reduction of edema, but use of the former may be limited by difficulties in application and in gaining access to the wound. An easily applicable and removable vacuum-formed rigid dressing (ORD) has recently been introduced. We compared the ORD with a conventional rigid plaster of Paris dressing with regard to wound healing, time to fitting of a prosthesis, and function with the prosthesis. METHOD: Patients undergoi

Using the common sense self-regulation model to determine psychological predictors of prosthetic use and activity limitations in lower limb amputees.

PURPOSE: There are around 800 lower limb amputations performed each year in Scotland, however one unpublished study showed that 20% of fitted transtibial amputees do not use, and 20% only occasionally use, their prosthesis at one-year follow-up. This finding has implications for patients' well-being and healthcare cost efficiency. Leventhal's common sense self-regulation model (CS-SRM), a social cognition model in health psychology, was used to determine if psychological variables would predict prosthetic use and activity limitations in lower limb amputees. METHOD: A longitudinal study recrui

Motor adaptation during dorsiflexion-assisted walking with a powered orthosis.

A robotic ankle-foot orthosis (AFO) that provides powered assistance could adjust to varying gait dynamics much better than a rigid AFO. To provide insight into how humans would adapt to a powered AFO, we studied the response of neurologically intact subjects walking with an active dorsiflexion assist orthosis proportionally controlled by tibialis anterior electromyography (EMG). We examined the two mechanical functions of ankle dorsiflexors in gait (power absorption at heel strike and power generation at toe-off) by recruiting two groups of healthy subjects: Group One, called Continuous Contr

Design of lower limb prosthesis with contact pressure adjustment by MR fluid.

This paper reports on the development of a new lower limb prosthesis that can change its volume and hardness based on the users requirements. The size and viscosity of several Magneto-Rheological fluid filled bags, fixed on the inner side of the socket is changed, in order to vary the socket properties. TSB (total surface bearing) sockets have been most selling ones during these two decades. From the user's point of view, it is excellent in this type of sockets that the weight of user is supported with the entire socket surface. However, it is impossible to cope with the volume change of the u

Locating the axis of rotation when fitting an elbow orthosis: a comparison of measurement and palpation.

No other previously published studies consider the relative motion of orthotic components positioned on the upper arm and the forearm. This study therefore measured the location and direction of the axis of rotation of an orthotic component fixed to the forearm in relation to an orthotic component fixed to the upper arm, and compared the results with those obtained by palpation. A plane flexion or extension motion of the forearm component in relation to the component on the upper arm can be described as a pure rotation about a fixed centre. However, activation of the biceps or triceps shifts t

Evaluating the relationships between the postural adaptation of patients with profound cerebral palsy and the configuration of the Seating Buggy's seating support surface.

We are currently investigating the physiological polymorphism of wheelchair users with profound cerebral palsy and the properties of the Seating Buggy (developed by S. Nishimura, 1998) to clarify important and general elements of wheelchairs for widespread use. Cerebral palsy is a diagnostic term used to describe a group of motor syndromes resulting from disorders in early brain development. Recently, it has been shown that the Seating Buggy produces functional head-neck alignments and active control of sitting balance for people with profound cerebral palsy. The Seating Buggy is a wheelchair

Traumatic below-elbow amputations.

Prehension, intelligence, and erect posture distinguish humans from lower animals. Hands are instrumental for our survival and welfare. We use our hands when we work, recreate, and communicate. A handshake, a touch, a sign, or signal has significant social and communicative meanings. Hands play a major role in defining the skill level of our activities and our level of social expression and integration. Indeed, refined psychomotor precision of hand function may distinguish some individuals among us, gifting society with its more skilled craftsmen, surgeons, artisans, musicians, athletes, and t
