AIM: To present a model of teaching general practice to medical students as a part of care for refugees in Mostar, Bosnia and Herzegovina. METHOD: With an international support, 33 medical students (from the third study year on) participated in a total of 51 field visits to 4 refugee camps near Mostar, Bosnia and Herzegovina, over a period of two years. Some students made more than 30 visits. Together with residents in family medicine at the Mostar University Hospital, the students performed physical examinations and small interventions, and distributed packs of different pharmaceutical agent

[Pseudocyst of the neck after traumatic avulsion of spinal C6-T2 nerve roots--case report].

A case is presented of an avulsion injury of the left brachial plexus in a woman who--after surgical reconstruction of the plexus--developed a liquid fistula from a giant pseudocyst localized in this area. A prolonged persistence of the pseudocyst resulted in a severe pain syndrome and symptoms of vascular disorders in the upper limb. Since results of local treatment (punctures, pressure dressings) were unsatisfactory, the authors decided to close the liquid fistula by suturing the damaged dural sac by means of laminectomy. The necessity of particular caution when operating in the area of inte

Complications associated with the Sistrunk procedure.

OBJECTIVE: To assess the type and rate of complications in the postoperative period of pediatric patients undergoing the Sistrunk procedure. STUDY DESIGN: A retrospective review of patients with a diagnosis of thyroglossal duct cyst who had not had corrective surgery previously. An attempt to standardize the study was made as follows: all patients had surgery under the direction of one surgeon, using the Sistrunk procedure with minor modifications from its original description. Complications were divided into major and minor categories. METHODS: Charts were reviewed for age, sex, preoperativ

Proper handling of soft tissue in the acute phase.

Proper handling of soft tissue in the acute phase demands proper attention to fundamental principles in wound evaluation, preparation, and repair. Foundational to immediate wound care is appropriate hemostasis and anesthesia. Correct soft tissue cleaning techniques are important, and the surgeon should understand special situations in which contamination may be a problem, including bites. The controversial issue of antibiotic prophylaxis is discussed. Certain wounds are not amenable to closure, but, where closure is indicated, orientation and preparation of the repair are important. The simple excision.

Elliptical excisional surgery offers several advantages over other cutaneous surgical techniques (i.e., shave excision, saucerization) which require healing by second intention. In general, an elliptical excision exhibits faster healing, better cosmesis, and yields a specimen for histologic exam and review of margins, and with proper attention to the details of cutaneous anatomy, familiarity with instrumentation, local anesthetics, suture materials, and observance of meticulous surgical technique, is performed with relative ease in the dermatology outpatient office setting.

Guided tissue regeneration. Nonabsorbable barriers.

Nonabsorbable barriers are considered the material by which all other barriers are judged. They have a well-established record of safety and efficacy. They are not a panacea, however. Practitioners must take special care when selecting both patients and surgical sites for GTR. Extensive experience, superior surgical ability, and meticulous attention to detail are also required if one is to achieve predictably favorable results.

Proposal to develop a detailed safety program for general/laser surgical patients infected with AIDS.

Medical health personnel have acquired serious and rarely fatal infections in the operating room from AIDS patients. Yet, there are no officially approved AIDS safety programs designed to protect medical health care personnel in the operating room. A sequential safety program is proposed beginning with the following steps: 1) evaluation and staging of the patient and associated complications; 2) protection of the major surgical and laser instruments; 3) use of an efficient evacuator system to avoid plumes of laser fragments from polluting the operating room environment; 4) protection of the op

Wrestler's ear: pathophysiology and treatment.

Management of auricular hematomas has always been a challenge for physicians. This injury is very common among wrestlers since early olympic competition times. Initial treatment usually involves simple aspiration and a compression bandage. Most of these hematomas recur; then it must be decided how to manage this recurrent problem and the eventual "cauliflower ear." A treatment is proposed that directs attention to the pathophysiology of the injury and involves total excision of the newly formed fibroneocartilaginous layer. Unless this layer is successfully removed the hematoma will persist and

Postoperative management of epikeratoplasty.

Epikeratoplasty is the surface implantation of devitalized human corneal tissue onto a recipient cornea for optical rehabilitation. Its postoperative management can be divided into three stages: (1) epithelialization, (2) suture management, and (3) restoration of visual function. Careful attention to each of the healing phases and the multiple potential complications that commonly arise is required for a satisfactory level of visual performance to be achieved in a high proportion of cases.

Postoperative management.

Postoperative vascular compromise can result from a problem at the anastomotic site, including vessel damage, improperly placed sutures, trauma to the vessel, or atherosclerosis. Systemic factors include decreased blood flow, vascular spasm, and atherosclerosis. External compression such as hematoma, tight skin closure, or a large drain pressing on a vessel can compromise flow. In patients who are to undergo elective microsurgery, angiography at the recipient site is helpful to select healthy vessels. Anastomosis is performed outside the zone of injury, and a patency test is performed after an

Complications arising from ritual circumcision: pathogenesis and possible prevention.

Over a period of six years, five seriously ill infants were referred with complications arising from ritual circumcision performed by nonphysicians. Four infants developed systemic infections; in three of them, including one with early meningitis, there was septicemia. The common predisposing factor was an excessively tight dressing over an infected penile wound, leading to urinary retention, urinary tract infection and septicemia. In the fifth infant, the glans had been partially amputated and required a suture; a second dressing tightly applied to control the bleeding was removed without sub

Minimizing postsurgical scars.

You will want to minimize postoperative scars for your and your patient's satisfaction. There are operative procedures and suturing techniques that will help you but, ultimately, a fine result can be most often obtained by meticulous attention to each detail, from the prep through the planning, the execution of the procedure, and the postoperative care.

The immediate postoperative care of a child with cleft lip: time-proved suggestions.

An excellent cleft lip repair demands and depends upon good postoperative care. The strain caused by crying can be minimized by eliminating restraints and by early hospital dismissal. Protein and vitamin C are important in healing. A nongreasy pressure dressing capable of absorbing serum is applied for the first night only, then the suture line is sponged with cold water after each feeding. Following suture removal, effective bridging is achieved by applying perforated Zonas adhesive directly to the wound. The care outlined has been used successfully for 93 cleft lip patients over 24 years. Bo

Primary closure of pilonidal cystectomy.

A method of primary closure following excision of pilonidal cysts and sinus was used in 38 cases. The criteria for patient selection are described as is the technique and procedure. Special attention must be placed on the obliteration of all "dead spaces" to eliminate accumulation of serum and blood. The dressing is not changed for a minimum of eight days, and pressure must be maintained for that time to assure success. In all cases complete closure was obtained. The advantages of this technique over the open procedure is explained.
