Computational flow dynamics and preclinical assessment of a novel hemodialysis catheter.

We examined flow characteristics and recirculation of a novel dialysis catheter with helically contoured lumens, which optimize the vectors of blood entering and leaving the device. Computational flow dynamics were used to measure shear stress, residence time (RT), Platelet Lysis Index (PLI), and recirculation of the study catheter, termed the VectorFlow catheter, compared with the Palindrome catheter. Bench and animal models were used to measure recirculation compared with other dialysis catheter designs. The VectorFlow catheter was associated with an 18% reduction in mean shear stress compar

Health-related quality of life and all-cause mortality in patients with diabetes on dialysis.

BACKGROUND: This study tests the hypotheses that health-related quality of life (HRQOL) in prevalent dialysis patients with diabetes is lower than in dialysis patients without diabetes, and is at least as poor as diabetic patients with another severe complication, i.e. foot ulcers. This study also explores the mortality risk associated with diabetes in dialysis patients. METHODS: HRQOL was assessed using the Short Form-36 Health Survey (SF-36), in a cross-sectional study of 301 prevalent dialysis patients (26% with diabetes), and compared with diabetic patients not on dialysis (n = 221), diab

A randomised controlled trial of Heparin versus EthAnol Lock THerapY for the prevention of Catheter Associated infecTion in Haemodialysis patients--the HEALTHY-CATH trial.

BACKGROUND: Tunnelled central venous dialysis catheter use is significantly limited by the occurrence of catheter-related infections. This randomised controlled trial assessed the efficacy of a 48 hour 70% ethanol lock vs heparin locks in prolonging the time to the first episode of catheter related blood stream infection (CRBSI). METHODS: Patients undergoing haemodialysis (HD) via a tunnelled catheter were randomised 1:1 to once per week ethanol locks (with two heparin locks between other dialysis sessions) vs thrice per week heparin locks. RESULTS: Observed catheter days in the heparin (n=2

Low blood levels of long-chain n-3 polyunsaturated fatty acids in US hemodialysis patients: clinical implications.

BACKGROUND: Cardioprotective and other clinical benefits of long-chain n-3 polyunsaturated fatty acids (PUFA) are inversely related to dietary intake and hence blood content. We therefore investigated, in the first study of its kind, the blood content and distribution of these fatty acids in a large representative population of US hemodialysis patients. METHODS: Frozen sera were obtained from 400 individuals who were part of a large, contemporary, representative cohort of US incident hemodialysis patients. Long-chain n-3 PUFA were measured in total serum lipids and in the neutral and polar se

Systemic barriers to optimal hemodialysis access.

Arteriovenous fistulas (AVFs) are considered the gold standard for hemodialysis vascular access based on their superior patency, low complication rates, improved adequacy, lower costs to the health care system, and decreased risk of patient mortality. The goals of the Fistula First Breakthrough Initiative are to achieve a prevalent AVF use rate of at least 66% nationally, to decrease central venous catheter use, and to reduce overall vascular access complication rates among patients on hemodialysis. Achieving optimal vascular access is a complex process and, unlike other dialysis outcomes, req

[Effect of different levels of glucose in the dialysate on the risk of hypoglycaemia during hemodialysis in diabetic patients].

INTRODUCTION: Hypoglycemia can occur during hemodialysis in diabetic chronic renal failure patients when using a dialysate without glucose. With dialysis solutions with glucose 90 mg/dL or more, this is preventable, but diabetic patients could present higher intradialytic glycemias. OBJECTIVE: We tested a dialytic fluid with a lower level of glucose trying to prevent both hypo and hyperglycemia in these patients. METHODS: Twenty diabetic individuals from our hemodialysis unit were submitted to 3 sessions with dialysis solutions with two different glucose concentrations--55 mg/dL (Phase 55),

Quality of life in automated and continuous ambulatory peritoneal dialysis.

OBJECTIVE: Despite a lack of strong evidence, automated peritoneal dialysis (APD) is often prescribed on account of an expected better quality of life (QoL) than that expected with continuous ambulatory peritoneal dialysis (CAPD). Our aim was to analyze differences in QoL in patients starting dialysis on APD or on CAPD with a follow-up of 3 years. METHODS: Adult patients in the prospective NECOSAD cohort who started dialysis on APD or CAPD were included 3 months after the start of dialysis. Two single-lumen noncuffed catheters in the jugular vein as long-term vascular access: a preliminary report.

Two single-lumen, noncuffed catheters in the same jugular vein have been used as preferred vascular access in our hemodialysis (HD) and apheresis patients in past years. The aim of this retrospective study was to analyze the clinical outcome of such a vascular access and the reasons for catheter removal. In 129 adult patients, aged 69 ± 13 years, 56% males, treated by HD (121 patients) or apheresis (8 patients), two single lumen, pre-curved 8 Fr catheters (Medcomp, Harleysville, PA, USA) inserted into the same jugular vein were used as vascular access between January 2009 and April 2010. Olfactory function improves following hemodialysis.

Olfactory function has been shown to be affected in chronic kidney disease; however, studies are contradictory and little is known on the effects of dialysis. To resolve these issues we tested olfactory function in 24 healthy controls and in 28 patients with chronic kidney disease receiving hemodialysis (20 patients) or peritoneal dialysis (the other 8). As assays for olfactory function we measured smell identification, n-butanol and acetic acid thresholds, Kt/V urea, percentage reduced urea, and weights before and after dialysis. Olfactory function was also self-rated by the participants. Com

Renal failure in multiple myeloma: a medical emergency.

Up to 50% of newly diagnosed plasma cell myeloma (PCM) patients can present with renal insufficiency, 20% with severe renal impairment and 10% requiring dialysis. PCM patients account for 2% of the dialysis population, adding 5000 new patients each year worldwide. Dialysis-dependent PCM patients have a 2.77 higher risk of death compared with other dialysis-dependent patients without this diagnosis. Renal failure and especially dialysis dependency is an independent poor prognostic factor in PCM, with the majority unable to achieve dialysis independence. Renal failure in PCM is a medical emergen

Outcomes in patients on home haemodialysis in England and Wales, 1997-2005: a comparative cohort analysis.

BACKGROUND: The UK national policy promotes expansion of home haemodialysis, but there are no recent data on characteristics and outcomes of a national home haemodialysis population. METHODS: We compared incident home haemodialysis patients in England and Wales (n = 225, 1997-2005) with age- and sex-matched incident peritoneal dialysis, hospital haemodialysis and satellite haemodialysis patients with follow-up until 31 December 2006. Cox regression analyses included time-dependent changes of wait-listing for transplantation (a proxy for health status), start of home haemodialysis and transpla

Kinetic analysis of peritoneal fluid and solute transport with combination of glucose and icodextrin as osmotic agents.

BACKGROUND: Controlling extracellular volume and plasma sodium concentration are two crucial objectives of dialysis therapy, as inadequate sodium and fluid removal by dialysis may result in extracellular volume overload, hypertension, and increased cardiovascular morbidity and mortality in end-stage renal disease patients. A new concept to enhance sodium and fluid removal during peritoneal dialysis (PD) is the use of dialysis solutions with two different osmotic agents. AIM: To investigate and compare, with the help of mathematical modeling and computer simulations, fluid and solute transport

The MDRD equation predicts peritoneal dialysis-delivered creatinine clearances from serum creatinine.

BACKGROUND AND OBJECTIVES: The four- and six-variable Modified Diet in Renal Disease equations (MDRDEs) are empiric expressions that estimate GFR in patients with kidney disease. No method currently exists to directly compare MDRDE estimates of clearance by the failing biologic kidney to the clearances achieved by dialytic modalities. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Because the MDRDEs estimate GFR, which in turn determines much of the small molecule clearance by the biologic kidney, the aim of this study was to determine if the MDRDEs would predict the measured creatinine clear

Modalities of continuous renal replacement therapy: technical and clinical considerations.

Continuous renal replacement therapies (CRRT) are continuous forms of renal functional replacement used to manage acute kidney injury (AKI) in the critically ill patient. Depurative mechanisms include convection, diffusion, and membrane adsorption utilizing high-flux highly permeable biocompatible dialysis membranes. The simultaneous infusion of replacement fluid permits fluid removal without intravascular volume contraction and better hemodynamic stability, metabolic control to almost normal parameters, and removal of large-size toxins and cytokines. Moreover, CRRT allows better long-term cle

Phosphate removal with several thrice-weekly dialysis methods in overweight hemodialysis patients.

BACKGROUND: Currently available phosphate binders are associated with either hypercalcemia or high costs, which limit their use in hemodialysis patients. Whether modifying dialysis prescription to intensify small-solute clearance also leads to better phosphate clearance is unknown. STUDY DESIGN: Randomized crossover trial. PARTICIPANTS: Large patients (>80 kg; N = 18) who could not achieve adequate Kt/V during a standard 4-hour thrice-weekly prescription of maintenance hemodialysis. INTERVENTION: 2 high-flux dialyzers in parallel for 4 hours in comparison to 3 other dialysis modalities (4 h

Rationale and design of the HEALTHY-CATH trial: a randomised controlled trial of Heparin versus EthAnol Lock THerapY for the prevention of Catheter Associated infecTion in Haemodialysis patients.

BACKGROUND: Catheter-related bacteraemias (CRBs) contribute significantly to morbidity, mortality and health care costs in dialysis populations. Despite international guidelines recommending avoidance of catheters for haemodialysis access, hospital admissions for CRBs have doubled in the last decade. The primary aim of the study is to determine whether weekly instillation of 70% ethanol prevents CRBs compared with standard heparin saline. METHODS/DESIGN: The study will follow a prospective, open-label, randomized controlled design. Inclusion criteria are adult patients with incident or preval

Costs of dialysis--a regional population-based analysis.

BACKGROUND: Population-based estimates of costs of renal replacement therapy are scarce in the literature. The aim of our study was to calculate the costs of long-term dialysis in 2006 on the basis of patient-specific data from a well-defined population in a region in western Germany (n = 310,757). METHODS: Cost estimation was performed from the perspective of the statutory health insurance. All dialysis patients from the study region (n = 344, 54% male, mean age (+/-SD) 69 +/- 13 years, 42% diabetic) were assessed for the costs of the dialysis procedures, dialysis-related hospital admissions

Formation of downhill esophageal varices as a rare but serious complication of hemodialysis access: a case report and comprehensive literature review.

BACKGROUND: Proximal or "downhill" esophageal varices are a rare complication of superior vena caval (SVC) obstruction. Few reports describe downhill varices in dialysis patients with catheter-related SVC occlusion. METHODS: We studied a case of downhill esophageal varices in a dialysis patient from our center and reviewed the published literature on presentation, evaluation and treatment in other dialysis patients (MEDLINE database search). RESULTS: Including our current case, we identified eight reports of dialysis patients with downhill varices. All cases were recognized after presentatio
