Multicomponent exercise with virtual reality for preventing functional decline in hospitalized older adults: a randomized, three-arm, parallel clinical trial.

BACKGROUND: Acute hospitalization often triggers functional decline in older adults. While multicomponent exercise programs (MEP) are effective, the role of immersive Virtual Reality (VR) remains underexplored in acute geriatric wards. This study evaluated the efficacy of a VR-enhanced MEP program compared with conventional MEP and usual care (UC). METHODS: Randomized, three-arm, parallel clinical trial in an acute geriatric ward. Patients aged ≥75 years were randomized to receive 1) MEP, 2) MEP-VR, or 3) UC. The primary outcome was the change in functional capacity, as measured by the Short

Polygenicity shapes distinct brain patterns and clinical profiles of bipolar disorder.

BACKGROUND: Bipolar disorder (BD) manifests both genetic predispositions and brain imaging abnormalities. Genetic analyses provide a powerful approach to disentangling the associations between BD and brain imaging-derived phenotype (IDP). However, in East Asian (EAS) populations, the genetic basis of these neuroimaging changes and their clinical relevance remains underexplored due to limited integrative evidence. METHODS: We investigated the genetic architecture and clinical relevance of BD and brain IDPs in East Asian populations using individual-level data from the Chinese Bipolar Disorder

Caregiver-implemented standardized-to-individualized behavior-analytic toilet-training package for preschoolers with autism.

Children with autism often face challenges with incontinence and may benefit from more structured toilet training relative to neurotypical children. In the current study, we evaluated a standardized-to-individualized behavior-analytic toilet-training package with caregivers as the interventionists. We used telehealth to train caregivers remotely to support their implementation of the procedures and obtain additional data on implementation and outcomes. Four preschool-aged children with autism and their caregivers participated. Three of four participants met mastery criteria with a standardized

Cardiopulmonary Exercise Testing in Patients with Cardiac Pacemakers: From Device Optimization to Individualized Cardiac Rehabilitation.

Permanent cardiac pacing abolishes symptomatic bradycardia, yet a substantial proportion of recipients remain limited on exertion. Chronotropic incompetence is present in about half of pacemaker-wearing patients when assessed by the Wilkoff index, and the rate-responsive function intended to correct it is frequently left at the settings applied at implantation. Read across the whole cardiopulmonary exercise test rather than at peak effort, the relationship between heart rate and oxygen consumption distinguishes an inadequate sensor slope, an activity threshold set too high, a reaction time out

Long-term follow-up of limbic leucotomy for self-injurious behavior: Report of 2 cases.

Introduction Severe self-injurious behavior (SIB) is a potentially life-threatening condition that is often treatment refractory, particularly in patients with severe psychiatric illness. Limbic leucotomy, which combines anterior cingulotomy and subcaudate tractotomy, has shown benefit in select patients with severe, treatment-refractory SIB as an intervention of last resort. However, long-term outcome data beyond a few years is essentially absent from present literature. We report the longest follow-up to date in two patients after limbic leucotomy for severe refractory SIB. Case Presentation

Thromboembolic and Mortality Risk in Patients with Postoperative Atrial Fibrillation After Coronary Artery Bypass Graft-The TRIP-AF Study.

Background/Objectives: Postoperative atrial fibrillation (POAF) is a common complication of coronary artery bypass grafting (CABG) and may recur during cardiac rehabilitation (CR); its long-term prognostic value remains unclear. We evaluated the impact of POAF on a composite endpoint of all-cause mortality and non-fatal stroke after isolated CABG and assessed POAF duration and timing, AF recurrence, and other predictors of adverse outcomes. Methods: This multicentre retrospective study enrolled 1139 patients who underwent CR after isolated CABG between 2011 and 2015 across 17 Italian centres.

Auditory Biofeedback Gait Training in Individuals With Chronic Ankle Instability: Protocol for a Randomized Controlled Trial.

BACKGROUND: Chronic ankle instability (CAI) develops in a large majority of patients with lateral ankle sprain (LAS), who often experience recurring injuries, residual feelings of instability, and abnormal neuromuscular patterns. Specifically, gait alterations are common in those with CAI and considered a contributing factor to poor long-term ankle joint health. Yet, there are no evidence-based gait-training protocols targeted to improve biomechanics, which may improve long-term outcomes related to ankle joint health and patient satisfaction. OBJECTIVE: This study protocol introduces a progre

Symbolic ableism in health professions education: development and initial psychometric evaluation of the German symbolic ableism scale.

BACKGROUND: Ableist attitudes may undermine equity, quality, and safety of care and may also shape the inclusion of learners and professionals with disabilities. Brief, context-sensitive instruments for health professions education are lacking. This study aimed to develop the German-language Symbolic Ableism Scale (SAS-G) and obtain initial exploratory psychometric evidence among medical and dental students. METHODS: A three-phase instrument-development study was conducted between 2022 and 2024. From the outset, people with disabilities participated directly as experts by experience in constr

Clinical Performance and Survival Outcomes of All-on-4 Full-Arch Implant-Supported Prostheses in Patients With a Completely Edentulous Arch: A Retrospective Cohort Study.

INTRODUCTION: All-on-4 full-arch implant-supported prostheses have become a predictable treatment option for rehabilitating completely edentulous patients by providing improved function, esthetics, and patient satisfaction while minimizing the need for extensive bone grafting. However, long-term evidence regarding implant survival, prosthetic success, and biological and mechanical complications is essential for validating their clinical effectiveness. This study aimed to evaluate the clinical performance, survival outcomes, biological and mechanical complications, and marginal bone loss associ

Key skills and challenges in one-on-one peer support for older adolescents and young adults with serious mental health conditions.

BACKGROUND: Peer support for older adolescents and young adults has become increasingly available as a formal mental health service. Alongside this increasing availability has been a steady accumulation of studies examining the potential for peer support to lead to positive outcomes for peer support users. However, a consistent refrain from the literature on peer support is that research and practice are hampered by a lack of theory-based models that specify the activities and skills of specific peer support roles, and describe how skillful practice is linked to outcomes. As part of a larger s

Voice Therapy in Patients with Cleft Lip and Palate: A Narrative Review.

OBJECTIVES: Cleft lip and palate (CLP), as one of the most common congenital anomalies in children, can result in voice disorders. Incidence of CLP and its consequential impact may lead to laryngeal pathologies of the vocal cords, and symptoms may include hoarseness, harshness, breathiness, decreased loudness, and abnormal pitch. Considering the limited number of findings on the efficacy of voice therapy in patients with CLP, the purpose of this narrative review is to aggregate the results of previous studies, explore their findings and restrictions, and identify gaps to present more appropria

TIRELESS study protocol: non-inferiority study of nurse-led web-based cognitive behavioral therapy for cancer-related fatigue in patients with incurable cancer.

BACKGROUND: Fatigue is a prevalent and debilitating symptom for patients with incurable cancer. Cognitive behavioral therapy (CBT) can significantly reduce cancer-related fatigue in this patient group. However, implementation of CBT into clinical care is difficult as the intervention is time-intensive, costly, and requires face-to-face consultations with a specialized CBT therapist. Since nurses already support patient with symptom management, including fatigue, internet supported CBT delivered by nurses could be a promising alternative for patients with incurable cancer. The primary aim of th

Questioning the cost of convenience: a mixed methods study of the impact of a telehealth service delivery on pharmacists' work.

OBJECTIVES: Telehealth represents a fundamental change to the health care delivery process. Despite the burgeoning literature on the rapid expansion of telehealth services during the COVID-19 pandemic, the consequences of these changes for clinicians' workload are seldom examined. This study addresses that gap by describing the interactions between patient activity, work processes and pharmacists' experience of an Australian hospital outpatient telepharmacy service that was rapidly upscaled during the pandemic to ensure all patients continued to access pharmaceutical care. METHODS: A three-ph

Healthcare providers' use and acceptance of translation technologies in multilingual healthcare settings: a cross-sectional study.

BACKGROUND: Language barriers remain a major challenge to equitable healthcare delivery. While qualified interpreters are considered the preferred standard, their availability is often limited. Translation technologies are therefore increasingly used in practice. However, quantitative evidence on how healthcare providers use these tools, how they evaluate them, and which factors are associated with their acceptance remains limited. METHODS: A cross-sectional online survey was conducted among healthcare and psychosocial service providers across a range of clinical settings. Inferential analyse

Temporal attentional bias to food stimuli in adolescent anorexia nervosa - an ERP study.

BACKGROUND: Anorexia nervosa (AN) is associated with substantial psychological and physical impairment and low recovery rates. A deeper understanding of its underlying mechanisms is therefore crucial. Cognitive models of eating disorders emphasize the role of attentional biases in the processing of food stimuli. However, only a few studies to date have examined the temporal dynamics of food-related attentional bias in adolescents with AN. METHODS: The present study combined the high temporal resolution of event-related potentials with the behavioral measure of the attentional blink in a rapid
