Multimodal Machine Learning-Based Prediction of Relapse in Major Depressive Disorder.

IMPORTANCE: Early identification of patients at high risk of relapse is key to personalized preventive strategies in major depressive disorder (MDD). However, current predictive models do not adequately account for the multimodal factors contributing to MDD relapse and fail to provide clinically meaningful predictions. OBJECTIVE: To predict relapse within 2 years in patients with MDD by using a comprehensive machine learning pipeline that integrates an unprecedented set of multimodal data. DESIGN, SETTING, AND PARTICIPANTS: This naturalistic longitudinal prognostic study used data from the M

Reduced Somatomotor Cortical Thickness in Women With PTSD After Childhood Sexual Abuse: A Structural Correlate Distinct From Adult Sexual Dysfunction.

BACKGROUND: Childhood sexual abuse (CSA) is a major risk factor for posttraumatic stress disorder (PTSD) and adult sexual dysfunction in women. Although adverse childhood experiences have been linked to altered brain structure, cortical alterations specifically associated with CSA and PTSD in women remain poorly characterized, and their relationship to adult sexual functioning is unknown. METHODS: Women with CSA and current PTSD (n = 52), women with CSA without current PTSD (n = 32), and control women without CSA or PTSD (n = 48) underwent structural magnetic resonance imaging. Cortical thick

Balancing Innovation and Evidence: Reflections on Structured Protocols in MDMA-Assisted Therapy Versus a Principle-Guided, Patient-Directed Approach.

There is growing interest in incorporating MDMA (3,4-Methylenedioxymethamphetamine) into structured, evidence-based cognitive-behavioral treatment protocols for post-traumatic stress disorder. Yet this risks overlooking the distinctive qualities of MDMA-assisted therapy, which has already demonstrated strong clinical effects, high response rates, and low dropout using a principle-guided, patient-directed therapeutic model. This model integrates MDMA administration into a broader therapeutic process, including nondrug preparatory and integrative sessions, designed to support the unique, nonline

A Case of Complex Post-Traumatic Stress Disorder Manifesting With Psychogenic Non-Epileptic Seizures.

In cases where complex post-traumatic stress disorder (CPTSD) co-occurs with psychogenic non-epileptic seizures (PNES), it is essential to emphasize that severe interpersonal trauma is strongly associated with PNES. Failure to recognize this association may result in misdiagnosis, inappropriate treatment (including polypharmacy), and potential iatrogenic harm.

Trauma-focused cognitive behavioral therapy for peripartum-onset post-traumatic stress disorder: a narrative review.

INTRODUCTION: Peripartum-onset post-traumatic stress disorder is a serious condition that warrants prompt diagnosis and treatment due to its adverse health outcomes for mothers and infants. When the trauma is associated with pregnancy and childbirth, it is known as childbirth-related post-traumatic stress disorder. Currently, psychotherapy is considered a first-line treatment for post-traumatic stress disorder and is also recommended for the peripartum population. One of the psychotherapeutic modalities whose effectiveness has been tested in the perinatal population is trauma-focused cognitive

Predicting cognitive-behavioral therapy outcomes in obsessive-compulsive disorder from inhibitory control neural activity: A mega-analysis and machine learning study from the ENIGMA-OCD consortium.

OBJECTIVE: Cognitive behavioral therapy (CBT) is an effective treatment for obsessive-compulsive disorder (OCD), yet it remains difficult to predict who will respond. This study investigates associations between neural activity during inhibitory control tasks and CBT outcomes, and its value as a predictive marker of CBT outcome. METHODS: Using fMRI data from individuals performing an inhibitory control task across five ENIGMA-OCD samples (n = 130, age range=8-57, 54% female), mass-univariate associations were analyzed between response inhibition and error processing activity and three CBT out

Anankastic personality in historical and contemporary perspective: a literature review.

Obsessive-compulsive personality disorder (OCPD), rooted in Janet's psychasthenia, Freud's "anal character," and Schneider's notion of anankastia, is among the most common yet least frequently diagnosed personality disorders. Contemporary classifications emphasize its dimensional nature. The central features are perfectionism, rigidity, and excessive need for control. Prevalence in the general population is estimated at 6.5%. In clinical populations, rates are higher: 16% among outpatients and up to 30% in inpatients. OCPD most often co-occurs with obsessive-compulsive disorder (OCD) (17‒45%),

Concentrated Exposure Therapy vs Internet-Based Cognitive Behavioral Therapy for Obsessive-Compulsive Disorder: A Randomized Clinical Trial.

IMPORTANCE: Given the high prevalence, chronicity, and associated disability of obsessive-compulsive disorder (OCD), optimizing treatment is a global priority. The Bergen 4-day treatment (B4DT), a concentrated exposure-based intervention, may reduce treatment burden and improve access to evidence-based care. OBJECTIVE: To evaluate the efficacy of B4DT compared with internet-based cognitive behavioral therapy (iCBT) in reducing OCD symptom severity. DESIGN, SETTING, AND PARTICIPANTS: Single-center randomized controlled trial (1:1 allocation). Between May 8, 2024, and August 8, 2025, adults (a

Shared and Differential Psychological Processes Associated With Treatment Response in Mindful Self-Compassion and Cognitive-Behavioural Therapy for Chronic Pain: An Exploratory Secondary Analysis of a Randomized Controlled Trial.

BACKGROUND: Psychological treatments for chronic pain are effective, but the processes associated with therapeutic change, and whether these are shared across treatments or treatment-specific, remain unclear. This exploratory secondary analysis examined whether changes in pain acceptance, pain catastrophizing facets and self-compassion were associated with clinical outcomes in Mindful Self-Compassion (MSC) versus Cognitive-Behavioural Therapy (CBT). METHODS: Data were drawn from a randomized trial comparing MSC (n = 61) and CBT (n = 59) in adults with chronic pain and elevated emotional distr

Incorporating Data From Text Messages and Email Into Psychotherapy: Randomized Controlled Trial.

BACKGROUND: As digital communication becomes central to daily life, psychotherapy increasingly has access to patients' electronic media data. Although digital phenotyping has been widely studied, less is known about whether incorporating personal communication data, such as text messages, improves clinical outcomes in psychotherapy. OBJECTIVE: This study aimed to determine whether integrating personalized text message data into psychotherapy improves depression, anxiety, health-related quality of life, and therapeutic alliance and to examine whether increased access to collateral information

Outcomes of evidence-based psychotherapy for healthcare workers: a single-arm prospective cohort study.

BACKGROUND: Healthcare workers (HCWs) experience high rates of depression and anxiety, yet face substantial barriers to accessing mental health care, including limited availability, scheduling constraints, cost, and concerns about confidentiality and stigma. Despite widespread efforts to support HCWs well-being, there is limited research on improving access to high-quality mental health care for this population. OBJECTIVE: To evaluate engagement and clinical outcomes associated with a scalable, employer-sponsored digital psychotherapy program implemented within a large health system to improv

Case Report: Panic attacks after tirzepatide injection in two female patients with recurrent depressive disorder.

Tirzepatide was introduced several years ago for the management of obesity. Its side-effect profile includes gastrointestinal symptoms and hormonal shifts similar to other glucagon-like peptide-1 receptor agonists (GLP-1RAs). This class of medications is currently being investigated for its potential utility in treating depressive and anxiety disorders. While safety data generally indicate a low risk for adverse psychiatric events - such as suicidal ideation or depressive episodes - and some evidence suggests a beneficial impact on mood, anecdotal reports have linked these agents to panic atta

Dynamic gene expression signatures predict psychotherapy response in PTSD.

Static blood-based biomarkers provide limited insight into psychiatric illness and recovery, largely because they capture only a single, unperturbed snapshot of inherently dynamic biological systems. In this study, we applied a dynamic, longitudinal dual-state approach to characterize peripheral blood mononuclear cell (PBMC) transcriptional trajectories associated with psychotherapy outcomes in post-traumatic stress disorder (PTSD). PBMC transcriptional profiles were generated for veterans with PTSD (n = 46, 21, and 17, respectively) and controls (n = 26, 26, and 23) at pre-treatment, post-tre

Pharmacologically altered androgens, brain activation and response inhibition in a stop-signal task in male heavy drinkers.

UNLABELLED: Heavy episodic drinking (HED) is highly prevalent in men and a major risk factor for Alcohol Use Disorder (AUD). Previous studies indicated an influence of testosterone (T) on AUD, and dihydrotestosterone (DHT) was linked to alcohol consumption, impulsivity, and response inhibition. As the role of androgens in AUD and the modulation thereof is not fully understood, this study investigated the effect of a pharmacological reduction in DHT by inhibition of the 5-alpha reductase II with finasteride on brain activation and response inhibition during a stop signal task. Fifty males with

A nurse-led digital nudging strategy to enhance home-based cardiac rehabilitation exercise adherence among patients with chronic heart failure: development of a complex intervention informed by the Medical Research Council framework.

BACKGROUND: Home-based cardiac rehabilitation offers a cost-effective alternative model. However, long-term exercise adherence among patients remains suboptimal due to insufficient motivation. Self-Determination Theory and Nudge Theory provide the theoretical foundation for understanding behavioral drivers and designing low-burden intervention strategies. Therefore, this study aimed to develop a nurse-led, theory-driven, digitally platform-delivered nudge intervention program to improve long-term rehabilitation behavior in patients with chronic heart failure. METHODS: A multi-stage mixed-meth

Characterizing Behavioral Engagement in Smartphone Cognitive Behavioral Therapy for Obsessive Compulsive Disorder: Secondary Analysis of a Randomized Controlled Trial.

BACKGROUND: Digital mental health interventions (DMHIs) are increasingly popular for their ability to disseminate evidence-based treatments in a scalable, accessible way. Engagement with digital treatments is critical for effectiveness but remains poorly defined and measured. Most studies rely on subjective self-reports or 1 or 2 objective metrics, overlooking the multidimensional nature of how users interact with DMHIs. OBJECTIVE: This exploratory secondary analysis applied a road map for comprehensive behavioral engagement measurement, developed for a prior smartphone cognitive behavioral t
