Odontogenic Maxillary Sinusitis With Sequestrum-Like Bone Formation Mimicking Fungal Sinusitis: A Case Report.

Calcified materials in an opacified maxillary sinus often suggest fungal sinusitis, particularly aspergillosis. However, odontogenic maxillary sinusitis may rarely be associated with a bone-like calcified mass, creating diagnostic difficulty. We report a case of chronic odontogenic maxillary sinusitis with sequestrum-like bone formation that mimicked fungal sinusitis. An 88-year-old man was referred for mobility and occlusal pain of the maxillary left first molar. He had no nasal obstruction, rhinorrhea, headache, facial pain, or history of maxillary sinus surgery. Intraoral examination showed

Case Report: Sticky bone-assisted autogenous tooth transplantation for compromised alveolar recipient sites: a case series.

BACKGROUND: Autogenous tooth transplantation is one of the treatment options for replacing missing or non-restorable teeth in appropriately selected patients, depending on patient-related factors, donor-tooth availability, and recipient-site conditions. Sticky bone was prepared by combining particulate deproteinized bovine bone mineral with autologous fibrin glue and was applied to the recipient-site osseous defects at the time of transplantation. We report a case series of autogenous tooth transplantation using sticky bone-assisted recipient-site management in four complex scenarios. CASE PR

Deciphering the mycetoma 'grain': a comprehensive look at its chemical composition.

Paranasal sinus mycetoma, also known as a fungus ball, is a non-invasive fungal infection characterized by a dense conglomeration of hyphae within a sinus cavity, most commonly the maxillary and sphenoid sinuses. Diagnosis is primarily based on the typical finding of hyperdensities on computed tomography, but their precise chemical composition remains incompletely understood. The objective of our study was to investigate the detailed chemical composition of central compartments of mycetomas (grains) and the relationship between these chemical components and clinical and radiological findings.

Paresthesia and Facial Bone Destruction due to Chronic Granulomatous Invasive Aspergillosis in the Left Maxillary Sinus.

We report a case of chronic granulomatous invasive aspergillosis in a 56-year-old male teacher from South Kordofan, Sudan, who underwent surgery in 2007 to remove his left orbital aspergillus granuloma. The patient developed progressive facial swelling beginning in 2017 and paraesthesia in the left zygomatico-maxillary region, despite initial treatment. A soft tissue mass in the left maxillary sinus and widespread bone loss affecting the zygomatic arch, maxillary antrum, and fronto-temporal lytic lesions were seen on computed tomography. Histopathological analysis confirmed the recurrence of a

Quantitative PCR-Based Analysis of Bacterial Profiles in Periapical Lesions and Maxillary Sinus in Odontogenic Sinusitis.

Odontogenic sinusitis (ODS) is a common cause of unilateral maxillary sinusitis arising from periapical lesions (PALs) or other dental sources. The infection is typically polymicrobial and dominated by anaerobic bacteria, which are often under detected by routine culture. Molecular approaches such as quantitative polymerase chain reaction (QPCR) and next-generation sequencing (NGS) may provide improved characterization of the microbial burden and community structure. This study aimed to compare culture-based methods, targeted quantitative PCR, and 16S rRNA sequencing in paired samples to chara

Prosthetic-surgical integration and robot-assisted implant placement for maxillary anterior fixed rehabilitation in the predoctoral clinic: A case report.

This clinical report describes a prosthetically directed execution to maxillary anterior rehabilitation in a patient with congenitally missing lateral incisors, retained primary molars, and anterior segment spacing. The definitive tooth morphology and eccentric guidance were established before surgery through a diagnostic wax-up, intraoral mock-up, and conservative direct bonding to confirm esthetic and functional parameters. Implant placement at sites #5 and #12 was then completed using robotic-assisted intervention, allowing versatile mesial-angulation correction of the osteotomies to avoid

Implant-supported fixed prosthodontic treatment for ectodermal dysplasia: a case report.

BACKGROUND: Ectodermal dysplasia (ED) often results in congenital tooth agenesis and severe developmental alveolar ridge defects, leading to functional impairments, reduced masticatory efficiency, and psychosocial challenges. Limited local blood supply and rapid bone resorption in ED patients pose significant challenges for predictable implant rehabilitation, highlighting the need for biologically guided, long-term restorative strategies. CASE PRESENTATION: An 18-year-old male patient with hypohidrotic ED (HED) presented with multiple congenitally missing teeth (FDI 12–16, 22–26, 34–35, 42, 44

Anatomical Variations Favouring Antrochoanal Polyp Genesis.

BACKGROUND: The antrochoanal polyp (ACP) is a rhinosinusal pseudo-tumour. Its etiopathogenesis is controversial. The antral cyst is thought to be the precursor of ACP. Certain rhinosinusal anatomical variants would favor its transformation into ACP. However, these anatomical variants remain undetermined. The aim of this study was to determine which rhino-sinusal anatomical variants are significantly associated with ACP presence. METHODS: This was a retrospective, descriptive and comparative study of 54 patients operated on for ACP. Clinical and paraclinical data were collected from medical re

Isolated oncocytic sinonasal papilloma of the sphenoidal sinus.

The ectodermally derived mucosa of the nose and paranasal sinuses may give rise to three morphologically different papillomas referred to individually as exophytic papilloma, inverted papilloma (IP) and oncocytic sinonasal papilloma (OSP). OSP is the rarest of the three different types, frequently affects the maxillary sinus, ethmoid sinus and nasal cavity while sphenoidal sinus involvement is exceedingly rare. The tumour has no gender predilection and has a wide age range with the majority above 50 years of age. Clinically, its behaviour parallels IP due to local recurrence and tendency to ma

Piezosurgery Versus Conventional Osteotomy in Prelacrimal Approach for Maxillary Sinus.

INTRODUCTION: The prelacrimal recess approach (PLA) is a minimally invasive approach to maxillary sinus lesions, especially those of the anterior and inferior wall. Innovative advances, for example, the piezosurgery ultrasonic device (PZD), have been developed aiming at facilitating surgical procedures in terms of efficient hemostasis, dissection, safety, and reduction in surgical time. The current study aimed to evaluate the efficacy of piezosurgery versus conventional osteotomy while performing a prelacrimal approach. METHODS: A prospective randomized controlled study was conducted in the O

Complications and Clinical Outcomes Following Modified Endoscopic Denker's With Pyriform Aperture Preservation: A Prospective Analysis.

BACKGROUND: The modified endoscopic Denker's (MED) approach provides access to all maxillary sinus (MS) walls for resection of various MS and retromaxillary pathologies, but the anteromedial maxilla has historically been resected. This study determined complication rates and outcomes following MED with pyriform aperture (PA) preservation (MEDPAP). METHODS: A prospective study was conducted on all adults who underwent MEDPAP for different MS or retromaxillary pathologies over 7.5 years. The following complications were collected at all postoperative visits: epiphora, hypesthesia or complete nu

Translacrimal Antrostomy: An Augmented Endoscopic Approach to the Maxillary Sinus.

Conventional middle meatal antrostomy has limitations in surgical accessibility and may be associated with postoperative stenosis. Alternative approaches that improve access to the maxillary sinus while maintaining long-term patency are therefore needed. This study aimed to evaluate the safety and efficacy of a novel technique, translacrimal antrostomy (TLA). Unlike standard middle meatal antrostomy (MMA), TLA involves the removal of the inferior lacrimal bone to facilitate access to the anterior maxillary recesses. A retrospective review was conducted of 64 patients (81 sinuses) who underwent

Endoscopic Prelacrimal Approach for the Surgical Management of a Late Silicone Foreign Body and an Odontogenic Maxillary Cyst: A Case Report.

Foreign bodies and odontogenic lesions of the maxillary sinus can lead to chronic rhinosinusitis, particularly when located in compartments that are difficult to access surgically. We report two cases managed using the endoscopic prelacrimal recess approach (EPLA). The first case involved a 76-year-old woman presenting with chronic facial pain and purulent discharge caused by a retained silicone fragment following bilateral inferior turbinectomy performed 20 years earlier. The second case concerned a 28-year-old woman with recurrent left maxillary sinusitis secondary to a 2.1-cm pericoronal cy

When Fatigue Was Vasculitis: A Case of Limited PR3 ANCA-Positive Granulomatosis With Polyangiitis.

Granulomatosis with polyangiitis (GPA) is a rare multisystem necrotizing vasculitis involving small to medium-sized vessels and is commonly associated with proteinase 3 antineutrophil cytoplasmic antibodies (PR3 ANCA). While pulmonary and renal manifestations are classically recognized, atypical presentations with predominant constitutional, sinonasal, ophthalmological, and endocrine manifestations may result in substantial diagnostic delay. We present the case of a 28-year-old woman with a two-year history of profound fatigue, generalized myalgia, anosmia, chronic sinonasal symptoms, headache

Different Approaches to the Treatment of Radicular and Related Cysts Associated with Nasal Mucosa in the Maxilla: A Case Series.

Radicular cysts (RCs) represent the most frequent inflammatory cystic lesions of the jaw, typically arising from non-vital teeth. While standard management via enucleation is well-documented, complex cases involving the anterior maxilla present significant surgical challenges due to their proximity to the nasal cavity floor (NCF) and the maxillary sinus floor (MSF). This report provides a comprehensive revision of a clinical case series involving seven patients (ages 17-50) treated with multimodal surgical and regenerative protocols. The patients were stratified into five distinct anatomical r

Aggressive Central Giant Cell Granuloma: A Case Report.

Central giant cell granuloma (CGCG) is an uncommon benign intraosseous lesion of the jaws that typically affects the anterior mandible in young females. Aggressive variants may demonstrate rapid growth, cortical expansion, pain, root resorption, and recurrence. This report presents an unusual case of aggressive CGCG involving the posterior maxilla in a 20-year-old female patient who presented with progressive swelling and facial asymmetry. Radiographic evaluation revealed a well-defined expansile lytic lesion involving the right maxilla and maxillary sinus. Differential diagnoses included aden
