Magnetic drug targeting in post-operative human paranasal sinuses: a CFD-DPM study.

BACKGROUND: Topical drug delivery to the paranasal sinuses remains challenging in chronic rhinosinusitis (CRS), even after functional endoscopic sinus surgery (FESS). This study evaluated magnetic drug targeting (MDT) for enhancing aerosolized drug delivery to post-operative sinuses. RESEARCH DESIGN AND METHODS: A computational fluid dynamics-discrete phase model was developed using two patient-specific post-operative sinonasal geometries: frontal drill-out (FDO) and full-house FESS (FHFESS). Sixty-six simulations assessed the effects of magnetic field strength, magnetic nanoparticle volume f

Odontogenic Sinusitis: The Selective Role of a Modified Caldwell-Luc Approach in Two CT/CBCT-Documented Cases.

Tooth-bearing structures are commonly involved in the spread of various odontogenic lesions. Radicular and dentigerous cysts are among the most common odontogenic cysts, exhibiting significant variations in size and morphology within the jawbones. On the other hand, the influence of dental caries, periodontal diseases, and periapical lesions extends beyond tooth-bearing bone and also affects adjacent anatomical structures, especially the maxillary sinuses. In addition to untreated odontogenic lesions, treatment-related complications in the posterior maxilla may also disturb the sinus environme

Diagnostic Accuracy of Panoramic Radiography for Assessing Maxillary Posterior Root Protrusion into the Maxillary Sinus: A Cluster-Adjusted Prediction Model Using CBCT as Reference.

Background/Objectives: The proximity of the maxillary sinus floor to the tooth apices is critical for dental procedures, so preoperative evaluation is crucial for preventing complications. This study aims to examine the accuracy of panoramic radiographs in determining the relationship between the maxillary sinus and teeth, in comparison with cone-beam computed tomography (CBCT), which serves as the reference standard, and to develop a cluster-adjusted clinical prediction model to assist clinicians in identifying patients who may benefit from CBCT imaging. Methods: A total of 3436 teeth were ev

Azithromycin in Dentistry: From Systemic Antibiotic to a Candidate for Local Therapeutic Delivery.

Azithromycin is widely used in dentistry as a systemic antibiotic, particularly for odontogenic infections and as an alternative in patients with β-lactam hypersensitivity. Beyond its antimicrobial activity, azithromycin possesses unique pharmacokinetic, anti-inflammatory, immunomodulatory, and anti-biofilm properties. Together with growing concerns regarding antimicrobial resistance and antibiotic stewardship, these characteristics have stimulated interest in local drug-delivery strategies that may reduce systemic antibiotic exposure while maintaining therapeutic efficacy. This narrative revi

Precautions When Extracting Impacted Third Molars With Impacted Enamel Pearls in the Maxillary Sinus Floor: A Case Report.

Cone-beam computed tomography (CBCT) is routinely used before mandibular wisdom tooth extraction to determine its three-dimensional relationship with the mandibular canal. In contrast, the extraction of a maxillary third molar is often planned based on panoramic radiographs alone. Enamel pearls are common at the maxillary molar sites, with prevalence varying by ethnicity; however, their detection based on panoramic radiographs alone is challenging. We herein present the novel case of a 27-year-old woman whose maxillary sinus floor was being elevated and partially perforated by enamel pearls, t

Orthodontic Microscrew for Ectopic Maxillary Canine Extraction in an Adolescent: A Case Report.

Extraction of a permanent maxillary canine in a skeletally immature patient creates a rehabilitative interval during which definitive implant therapy is contraindicated and conventional alternatives have well-known limitations. This case report describes the use of a titanium orthodontic microscrew as a transitional prosthetic support in this clinical situation. A 13-year-old female patient was referred with an impacted right maxillary canine in a severely ectopic position adjacent to the floor of the maxillary sinus. Orthodontic traction was considered unfeasible based on the tooth position,

Co-relation between periodontitis and maxillary sinusitis: A cross sectional CBCT based study.

Periodontitis is a potential but often underdiagnosed cause of maxillary sinusitis, complicating accurate diagnosis due to overlapping clinical features. Therefore, it is of interest to evaluate the correlation between alveolar bone loss and maxillary sinus mucosal thickening in 50 patients with periodontitis. Measurements were performed in coronal and sagittal sections to assess the extent of periodontal destruction and sinus involvement. A significant association was observed, with greater mucosal thickening in patients exhibiting moderate to severe alveolar bone loss compared to mild cases.

Impact of hypertension and antihypertensive medications on early implant failure following sinus augmentation.

BACKGROUND: Hypertension (HTN) is a prevalent condition affecting a significant proportion of the population. It requires careful management due to its potential systemic complications. While the overall effects of HTN are well documented, its specific impact on implant dentistry procedures, such as sinus augmentation, remains less clear. OBJECTIVES: The present study aimed to investigate the influence of HTN and antihypertensive medications on early implant failure (EIF) following sinus augmentation. MATERIAL AND METHODS: A retrospective cohort study was conducted at a tertiary referral cen

Fungal ball-type maxillary sinusitis associated with Penicillium species: A case report and management perspective.

Penicillium is a diverse genus of fungus that widely exists in the environment and is often non-pathogenic. Only a handful of literature have reported its association as a cause of infection in humans. Here, we report a case of a Penicillium maxillary sinus fungal ball in an elderly man complaining of chronic headache, progressive unilateral nasal blockage and post-nasal drip. The role of Penicillium in a fungal ball and management are further discussed.

Management of a Symptomatic Maxillary Implant Adjacent to the Schneiderian Membrane Using Cone Beam Computed Tomography (CBCT)-Guided Explantation and Reimplantation: A Case Report.

Dental implants are a predictable treatment option for the replacement of missing teeth; however, complications related to implant positioning and proximity to adjacent anatomical structures may result in discomfort and compromise long-term outcomes. Cone beam computed tomography (CBCT) plays an important role in the diagnosis and management of implant-related complications by providing detailed three-dimensional evaluation of implant position and surrounding anatomy. This case report describes the management of a symptomatic maxillary implant in a patient who presented with intermittent pain

Evaluations of maxillary sinus pathologies using CBCT imaging.

The detection of maxillary sinus pathologies is often challenging due to their asymptomatic nature and the limitations of conventional imaging techniques. Therefore, it is of interest to use cone-beam computed tomography (CBCT) to examine maxillary sinus issues. They reviewed 120 CBCT scans to identify both disease and anatomical differences. Mucosal thickening was the most common finding, followed by sinus opacification and retention cysts. These issues appeared more often in older patients. Thus, CBCT proved to be a reliable way to detect sinus problems and can support better clinical decisi

Global anatomical variability of the posterior superior alveolar artery: a CBCT-based systematic review with AI-assisted risk stratification.

The posterior superior alveolar artery (PSAA) is a clinically important anatomical landmark in the maxilla and is frequently encountered during surgical procedures such as maxillary sinus augmentation and posterior dental implant placement. Owing to considerable anatomical variations in its course, diameter, branching pattern, and position, accurate preoperative assessment is essential to minimize the risk of intraoperative bleeding and vascular injury. The present systematic review analyzed data extracted from 49 published studies, including cone-beam computed tomography (CBCT), computed tomo
