Network analysis of kinesiophobia and fear of progression and their association with cardiac rehabilitation adherence in post-CABG patients.

BACKGROUND: Coronary heart disease is the major disease burden of cardiovascular diseases in China, and coronary artery bypass grafting (CABG) is the preferred surgical procedure for complex coronary artery lesions. Postoperative cardiac rehabilitation can improve patient outcomes; however, kinesiophobia and fear of progression are associated with lower adherence to rehabilitation. This study aimed to identify core symptoms within the fear network, explore hypothesized directional relationships among symptoms, and analyze the associations between the two types of fear symptoms and adherence to

Derivation of a blood count-based machine learning model to prioritise sequencing for high-risk clonal haematopoiesis: a retrospective cohort study.

BACKGROUND: Clonal haematopoiesis is a precursor of myeloid neoplasia and common in older adults, Precursor risk stratification is critical to early cancer detection, guiding surveillance and prevention efforts to high-risk populations. Existing clonal haematopoiesis risk scores use molecular features to estimate risk of myeloid neoplasia, but only around 1% of adults with detectable clonal haematopoiesis are at high risk of myeloid neoplasia transformation. Moreover, sequencing based molecular diagnostics are expensive with limited global availability highlighting the need for pre-sequencing

Phase Angle at Completion of Cardiac Rehabilitation Predicts Unplanned All-cause Readmission: A Retrospective Cohort Study.

OBJECTIVES: Phase angle (PhA) reflects cellular integrity and systemic physiological resilience. This study investigated whether PhA measured at the completion of late-recovery-phase cardiac rehabilitation (CR) predicts unplanned all-cause readmission in patients with cardiovascular disease (CVD), comparing its predictive value with baseline PhA and exercise capacity. METHODS: We analyzed 83 patients with CVD. PhA, peak oxygen uptake (peak V̇O2), and anaerobic threshold (AT) were assessed at both CR initiation and completion. The primary endpoint was unplanned all-cause readmission. RESULTS:

Uptake and Inequalities in Exercise-Based Cardiac Rehabilitation After Decentralization.

IMPORTANCE: Despite clinical benefits and guideline recommendations, uptake of exercise-based cardiac rehabilitation (CR) following coronary artery disease remains suboptimal, with barriers including transportation issues, travel distance, and geographic location. To overcome these barriers, decentralization strategies that move health care to local settings have been developed across several countries. OBJECTIVE: To determine whether CR uptake improved following decentralization, and whether geographic and social inequalities were reduced. DESIGN, SETTING, AND PARTICIPANTS: This register-ba

One-Year Systolic Blood Pressure and Subsequent Cardiovascular Outcomes After Acute Coronary Syndrome: Insights from a Swiss Prospective Cohort.

AIMS: Current guidelines recommend targeting systolic blood pressure (SBP) below 130 mmHg in patients at high cardiovascular risk, including after acute coronary syndrome (ACS), although evidence specific to the post-ACS setting remains limited. We evaluated the prognostic value of SBP measured 1 year after ACS on subsequent cardiovascular outcomes. METHODS: The ELIPS cohort prospectively enrolled ACS patients across four Swiss tertiary-care centers (2009-2017). Patients alive at 1 year with available SBP measurements were included. The association between SBP at 1 year and 3-point major adve

Home-Based Programs and Cardiac Rehabilitation Participation Among Veterans With Ischemic Heart Disease.

IMPORTANCE: Cardiac rehabilitation (CR) is a guideline-recommended secondary prevention program that reduces morbidity and mortality among patients with ischemic heart disease (IHD) yet remains underutilized. Over the past decade, the Veterans Health Administration (VHA) has implemented strategies to increase CR participation, including expansion of virtual home-based CR (HBCR) programs as alternatives to traditional center-based CR (CBCR). OBJECTIVE: To examine temporal trends and factors associated with CR participation among veterans with IHD. DESIGN, SETTING, AND PARTICIPANTS: This cohor

Improving access to cardiac rehabilitation in Australia: a policy reflection on equitable secondary prevention.

Cardiac rehabilitation (CR) is an evidence-based secondary prevention intervention associated with reduced cardiovascular mortality and hospital readmissions and improved quality of life. Despite strong guideline recommendations, participation remains low in Australia and internationally, particularly among rural and remote populations, Aboriginal and Torres Strait Islander peoples and other underserved populations. This reflects a service delivery challenge rather than an evidence gap. Current CR models remain largely centred on metropolitan hospital-based programs that may not adequately mee

The Future of Cardiac Care: A Review on the Role for Non-Invasive, Home-Based Monitoring and Rehabilitation after Hospitalisation for Acute Decompensated Heart Failure.

PURPOSE OF REVIEW: Heart failure (HF) is characterised by recurrent decompensation leading to worsening of symptoms and unplanned hospitalisations. Patients often experience poor post-hospitalisation recovery, fail to maintain functional capacity, and demonstrate a high risk for re-admission. In this review we discuss these challenges, and explore how non-invasive, digital solutions may improve health outcomes. RECENT FINDINGS: In contrast to current episodic surveillance, advances in wearable sensor technologies enable continuous, non-invasive monitoring to facilitate physiology-guided manag

A Bayesian-Optimized Weighted Fuzzy Rank-Based Ensemble for Coronary Stenosis Detection in X-ray Angiography.

X-ray coronary angiography is the gold standard for diagnosing coronary artery disease, a leading cause of global mortality. Existing artificial intelligence methods typically detect stenosis from individual frames or rely on manually selected key frames, despite angiography being inherently a video-based examination in which diagnostically relevant information appears only in a subset of frames. Moreover, conventional ensemble methods generally employ fixed classifier weights, limiting their ability to account for uncertainty and variations in classifier performance. To address these challeng
