Treated and untreated suicide attempts in substance abuse patients.

Suicidal behavior is a relatively common and serious problem in those with alcohol and drug problems. We compared demographic and clinical characteristics of 123 alcohol rehabilitation patients with and without a history of suicide attempts, using the Schedule for Affective Disorders and Schizophrenia-Lifetime version. Younger age was significantly related to suicide attempts, as were drug disorders, panic attacks, antisocial symptoms, and alcohol-related problems such as violence, withdrawal symptoms, and personal or occupational loss. Untreated suicide attempts were characterized by less ser

Lifetime psychiatric comorbidity in hospitalized alcoholics: subject and familial correlates.

We classified 123 alcohol rehabilitation patients by their histories of lifetime psychiatric comorbidity, and examined the demographic, social, occupational, treatment, and familial variables characterizing the groups. Diagnostic assessments were made with the SADS-L/RDC. High lifetime prevalences of major depressive disorder and drug use disorder were found. Aspects of treatment history distinguished between patient groups with and without lifetime major depression, but not other personal or familial variables. Patients with lifetime drug use disorders were younger and experienced an earlier

Severity of alcohol dependence and social/occupational problems: relationship to clinical and familial history.

We examined the relationship of major depression, aspects of antisocial personality disorder, family history of alcoholism, treatment history, and onset of alcohol problems to the severity of alcohol dependence and severity of social/occupational problems due to alcohol. Subjects were 111 randomly sampled patients in alcohol rehabilitation. The predictor variables formed different patterns of relationships with alcohol dependence and with social/occupational problems. Implications of the findings for future research are discussed.

Alcoholism among homeless adults in the inner city of Los Angeles.

The prevalence, course, and severity of alcoholism were examined in a probability sample of homeless adults in Los Angeles' inner city and in a household sample (drawn from two communities in Los Angeles), matched to the demographic characteristics of the homeless sample. Both lifetime and current prevalence of alcoholism (as measured by the National Institute of Mental Health's Diagnostic Interview Schedule) was substantially higher among homeless individuals. Moreover, compared with household alcoholics, homeless alcoholics were characterized by (1) a substantially higher prevalence of other

Predictors of successful alcohol rehabilitation in an Air Force population.

In order to find predictors of success in alcohol treatment, the authors conducted a 1-year follow-up of 71 active duty military personnel treated in a 28-day inpatient program. Success at 1 year follow-up was then compared with variables collected on a patient questionnaire completed at the beginning of rehabilitation. A family history of emotional disorders, together with a single incident precipitating treatment, was associated with a positive alcohol rehabilitation outcome.

Characteristics of patients who continue with alcohol outpatient treatment.

Demographic data were collected on 121 patients who were screened and accepted by the outpatient selection of the West Side VA alcohol rehabilitation program. Patients who engaged in active outpatient treatment (26%) were more likely to be: employed at night, sober at the time of the screening, and living within 40 blocks of the hospital. Referring the patient to another facility for detoxification and admitting him to the inpatient section of the rehabilitation program decreased the chances that he would continue with outpatient care.

Evaluation of a residential AA program.

The purpose of the study was to evaluate the effectiveness of the Serenity Lodge alcohol rehabilitation program by comparing a group of 137 men who entered the program with the same number of similar men who did not enter the program after an admission to a detoxification hospital. The residential program, for which people must pay board, is based on the AA philosophy and reality therapy. Follow-up interviews were conducted with three-quarters of the men in each group. Even after allowing for spontaneous remission as measured by the improvement in the control group, it was apparent that for a

Lithium orotate in the treatment of alcoholism and related conditions.

The subjects were 42 alcoholic patients (33 males and 9 females) who were treated with lithium orotate during an alcohol rehabilitation program in a private clinical setting for at least six months. They derive from a total number of 105 patients who received this treatment initially, while the remainder discontinued the treatment within six months. The data were collected from a private practice record and the follow-up varied between six months and 10 years. The 42 patients studied displayed a multitude of complaints in addition to chronic alcoholism. These included liver dysfunction, seizur

Evaluation of routine screening physicals by emergency room personnel for an alcohol rehabilitation program.

A study of the University of Maryland Hospital emergency room's medical screening program for alcohol rehabilitation revealed that the screening exam works well, but that it may not be necessary in a resource-intensive ER. Cost-effective, rational approaches for supplying appropriate services are suggested.

Mood-altering effects of disulfiram in alcoholics.

The effects of disulfiram on depression and anxiety were examined. In a 3-week double-blind study, 40 inpatients in al alcohol rehabilitation unit (ARU) were randomly assigned to receive placebo, 250 mg/day of disulfiram or 500 mg/day of disulfiram. During their first week in the ARU and prior to beginning medications, all subjects were administered the Zung self-rating depression scale, the Hamilton observer rating scale for depression, the Zung self-rating scale for anxiety and the Hamilton observer rating scale for anxiety. All subjects were rescored on these instruments at the end of their

Effects of alcohol rehabilitation treatment on health and performance of Navy enlisted men.

Changes in medical care utilization (rates of hospital admission and days hospitalized) and performance (rates of unauthorized absence and desertion) from pre- to posttreatment for alcohol abuse were determined for a group of career Navy enlisted men. Comparisons were made with two control groups matched with the abusers on year of entering the service and age at enlistment. Treatment favorably affected health and performance during the 2-year posttreatment period. While the alcohol abuse group showed no change in the mean number of hospital admissions from pre- to posttreatment, significant i

Evaluation of an inpatient alcohol rehabilitation programme.

A retrospective matching procedure was used to form a control group of 145 men who underwent detoxification, but unlike the treatment group did not enter the Quo Vadis Hospital rehabilitation programme. On a wide range of variables for which data was obtained at follow-up interview, respondents in the two groups were very similar. By contrast, official records data showed that the treatment group had significantly more readmissions to the detoxification hospital during the follow-up period than the control group. A number of methodological issues are briefly discussed.

Ward atmosphere as a factor in irregular discharge from an alcohol rehabilitation unit.

A 20-bed alcohol rehabilitation unit with a high patient retention rate had the experience of a sudden increase in irregular discharges. Administration of the Ward Atmosphere Scale developed by Rudolph Moos showed a drop in various therapeutic aspects of the ward environment that paralleled the increase in irregular discharges. These results were discussed with staff, and soon after the number of irregular discharges diminished. The Ward Atmosphere Scale was readministered and showed a move toward a more therapeutic atmosphere. The reasons for these fluctuations are discussed.

Psychological severity and response to alcoholism rehabilitation.

This study examines the problem of effective treatment for alcoholic patients with concurrent psychopathology. The subjects were 131 male veterans admitted to the Coatesville Veterans Administration Medical Center. They remained in an alcohol rehabilitation program at least five days following detoxification and were successfully followed up six months after admission. Subjects were divided into LOW, MID, and HIGH groups based on their psychological severity score. The Addiction Severity Index (ASI) was then used to assess problem severity in six areas (medical, legal, family, substance abuse,
