Manualized versus treatment-as-usual cognitive behavioral therapy for social anxiety disorder: A comparative study.

OBJECTIVE: This study aims to compare treatment outcomes of manualized cognitive behavioral therapy (MAN-CBT) with CBT treatment-as-usual (TAU-CBT) in patients with social anxiety disorder across two separate investigations. It was examined whether treatment effects differ between conditions after the total number of sessions, and whether manualized therapies would show better outcomes with equivalent dose of sessions. METHODS: All patients underwent CBT at the same outpatient treatment center. This secondary analysis utilizes data from two randomized controlled trials for the manualized CBT

Immunoglobulin subclass profiles in depression: Insights from a general population study.

The immune system plays a crucial role in defending the body against harmful invaders, with immunoglobulins being key players in pathogen recognition and neutralization. Immunoglobulin dysregulation is linked to various diseases, including autoimmune disorders, immunodeficiencies, and allergies. Previous studies suggest that depression can alter immune function by modifying immunoglobulin production, which may contribute to increased vulnerability to infections and autoimmune diseases. In this study, we examined the association between the immunoglobulin (Ig) subclasses IgA, IgG, IgM, and IgE

Effects of prospective mental imagery on approach motivation and behavioral engagement in social anxiety.

Exposure therapy is considered the leading evidence-based treatment for anxiety disorders. Yet, a significant number of individuals fail to benefit from its efficacy due to their reluctance to confront themselves with the feared situation. With evidence indicating a link between approach motivation and reward processing in socially anxious individuals, prospective mental imagery may serve to enhance anticipatory reward processes and promote approach motivation for exposure to feared situations and subsequently behavioral engagement with feared social situations. The present study tested the im

Limited Efficacy of 3 mA Intensified tDCS of the Right Inferior Frontal Cortex for OCD Treatment: A Randomized, Double-Blind, Sham-Controlled Study.

OBJECTIVE: Transcranial direct current stimulation (tDCS) has been proposed for treating obsessive-compulsive disorder (OCD) with advantages such as affordability and home-use application. Despite a moderate therapeutic effect, previous findings have been mixed. This registered, randomized, double-blind, sham-controlled trial investigated the efficacy of an intensified tDCS protocol targeting the inferior frontal cortex (IFC), a relevant brain region for inhibitory control for OCD treatment. METHODS: Forty patients with OCD received 20 sessions of 3 mA intensified tDCS or sham tDCS in paralle

Co-occurrence of dissociation and anxiety and depressive disorders in adults: a scoping review.

BACKGROUND: Dissociation, including depersonalization, derealization, dissociative identity disorder or dissociation in the context of functional neurological disorder, is classically associated with trauma-related disorders but is increasingly reported in anxiety and depressive disorders. Despite growing evidence linking dissociation to affective symptom severity, functional impairment, and treatment outcomes, its role in anxiety and depression remains insufficiently synthesized. OBJECTIVE: This scoping review aimed to map the extent, nature, and characteristics of dissociation in adults wit

One-Session Versus Three-Sessions of Virtual Reality Exposure Therapy for Public Speaking Anxiety: Protocol of a Randomized Controlled Trial.

BACKGROUND: A 1-session virtual reality (VR) exposure for public speaking anxiety has been found to be effective in reducing public speaking anxiety. Previous meta-analyses comparing 1-session to several sessions of in vivo exposure for specific phobias did not find a difference in effectiveness. However, due to the specificity of VR exposure and public speaking anxiety, it is unclear how dividing the same amount of exposure tasks into 3 shorter sessions instead of one long session would affect the effectiveness of VR exposure for public speaking. OBJECTIVE: The aim of this study is to compar

Comparative Analysis of Metacognition and Mindfulness Across Social Anxiety Disorder, Panic Disorder and Healthy Controls.

Metacognition and mindfulness are central to contemporary accounts of anxiety, yet their comparative profiles across social anxiety disorder (SAD) and panic disorder (PD) remain insufficiently defined. The objective of this study is to compare metacognition and mindfulness across SAD, PD and healthy controls (HC) and to examine how these processes predict disorder specific severity. Adults with SAD (n = 47), PD (n = 50) and HC (n = 50) participated, for a total of 147 individuals. Participants completed the Metacognitions Questionnaire 30 (MCQ-30), the Five Facet Mindfulness Questionnaire Shor

Good morning or good evening: Daytime and cortisol as modulators of exposure therapy outcomes.

BACKGROUND: Although exposure therapy is considered the gold standard treatment for anxiety disorders, many patients do not respond sufficiently to it. This study investigated the effects of the time of day at which exposure therapy is delivered and endogenous cortisol levels during exposure on treatment success in patients with fear of spiders. It was hypothesized that individuals undergoing exposure therapy in the morning would experience greater symptom reductions compared to those undergoing therapy in the evening, and that higher endogenous cortisol levels would predict greater symptom re

Addressing tobacco use in the context of opioid use disorder: A systematic review of smoking cessation interventions.

BACKGROUND AND OBJECTIVES: Rates of tobacco use disorder (TUD) are high among individuals with opioid use disorder (OUD), who consistently show lower response rates to evidence-based treatment approaches for TUD relative to the general population. This systematic review aims to provide a comprehensive and updated examination of the current evidence regarding TUD treatments for individuals with OUD. We first summarize potential mechanisms driving TUD and treatment nonresponse among those with OUD, given the likely roles of overlapping reward and withdrawal processes in co-occurrence and treatme

Neurofeedback as an emerging rehabilitation strategy for Parkinson's disease: a narrative review.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by motor and non-motor symptoms associated with dysfunction of cortico-basal ganglia-thalamo-cortical networks. Because current treatments remain largely symptomatic, there is increasing interest in adjunctive rehabilitation strategies capable of targeting disease-relevant neural activity more directly. Neurofeedback (NF), delivered through electroencephalography (EEG), functional magnetic resonance imaging (fMRI), or deep brain stimulation (DBS)-guided platforms, has emerged as a potential self-regu

Differential effects of cognitive behavioral therapy and pharmacotherapy on cognitive function in patients with major depressive disorder: A propensity-weighted longitudinal study.

BACKGROUND: Neurocognitive dysfunction is a core feature of major depressive disorder (MDD), predicting relapse, persisting after remission, and hindering functional recovery. Although cognitive behavioral therapy (CBT) and pharmacotherapy are both established treatments, their differential effects on specific neurocognitive domains remain poorly characterized. We investigated the distinct neurocognitive effects of CBT and pharmacotherapy in patients with MDD. METHODS: This study assessed 69 patients with MDD treated with either manual-based CBT combined with pharmacotherapy (n = 41) or pharm

Brain Ageing in Social Anxiety Disorder: An ENIGMA-Anxiety Mega-Analysis Across 26 International Cohorts.

Social anxiety disorder (SAD) is among the most prevalent anxiety disorders, and it has been associated with signs of advanced biological ageing. Despite this, brain age research on anxiety disorders remains limited. This mega-analysis investigated brain ageing in adults with SAD within the ENIGMA-Anxiety Working Group. Structural MRI scans from 576 participants with SAD and 1 355 non-affected healthy controls (HCs) across 26 international samples were included. Brain age was estimated from 77 cortical and subcortical regions using a publicly available ENIGMA brain age model. The brain-predict

Delay discounting correlates with depression but does not predict relapse after antidepressant discontinuation.

Approximately one third of people with Major Depressive Disorder (MDD) experience a relapse within six months of discontinuing antidepressant medication (ADM), however, reliable predictors of relapse following ADM discontinuation are currently lacking. A putative behavioural predictor is delay discounting, which measures a person's impatience to receive reward. Previous studies have linked delay discounting to both MDD and reduced serotonergic function, rendering it a plausible candidate predictor. In this multi-site study we measured delay discounting in participants with remitted MDD (N = 97
