Purpose: To clarify the existence and characteristics of the latency period in the improvement course of fusional ability in patients with intermittent exotropia (IXT) treated with orthoptic training, and to contribute to clinical decision-making regarding the optimal timing for training termination. Study design: Retrospective observational case series. Methods: Fusional ability was assessed in 13 patients with IXT using the ratio of absolute fusional range divided by ocular deviation (FR/D ratio) and graphed to analyze the shape of the improvement course. The latency period was defined as th

To evaluate the effects of orthoptic training using a virtual reality (VR) head-mounted display game in patients with intermittent exotropia (IXT), a prospective, randomized, double-blind study was performed in 62 patients ≥ 13 years with IXT of ≥ 8 prism diopters (∆) and with a near ≥ distance angle, i.e., basic type and convergence insufficiency-type IXT. Patients were randomly assigned to a video-game group that induced convergence (exercise group) and a placebo-game group not inducing convergence (control group), and completed the program 15 min/day for 4 weeks. Subjective symptom scores,

PURPOSE: To determine the effect of low hypermetropia correction and orthoptic exercises on binocular visual function in children with symptomatic convergence insufficiency.
METHODS: This was a prospective, randomized intervention with a 3-month follow-up. Consecutive pediatric patients with convergence insufficiency and hypermetropia who met inclusion criteria were randomly assigned to one of three treatment groups: spectacle correction and orthoptic training (group 1), spectacle correction alone (group 2) and orthoptic training alone (group 3). The correction of hypermetropia ranged from +0

Purpose: To compare the effect of combinational office-based and home-based orthoptic training regimens with part-time patching in participants with intermittent exotropia. Methods: In a randomized clinical trial study, patients with a diagnosis of IXT were involved and randomly assigned to three groups. Patients in the control group were followed with part-time patch therapy. For the Intervention 1 group, in-office and home-based orthoptic training was prescribed along with playing with a video game software (PIVOT). For Intervention 2 group the same management protocol was incorporated excep

BACKGROUND: Intermittent exotropia (IXT) is the most common type of exotropia in China. Surgery is usually required to align the eye deviation to maintain or obtain better binocular visual function. However, there is a high rate of exodrift or recurrence in surgically treated patients. Orthoptic therapy is sometimes recommended for IXT patients after surgery. However, there is a lack of high-quality randomized controlled trials to prove that orthoptic therapy could be an effective supplement to surgical treatment for IXT patients. The main purpose of this study is to test the clinical effectiv

PURPOSE: To investigate the effects of orthoptic training for residual diplopia after blowout fracture (BOF) surgery.
METHODS: We retrospectively reviewed the medical records of 14 (average age, 22.9 ± 13.1 years) patients with residual diplopia, who had undergone orthoptic training after BOF surgery at the Department of Ophthalmology, Kindai University Hospital, between August 2013 and September 2019. The orthoptic training included exercises for eye movement, convergence, and fusional area expansion. We assessed the training effects by scoring patients' Hess screen (Hess) test results and f

BACKGROUND: Dyslexia affects 5%-8% of the population of the Western world. While reading, different eye movements are required. Compared to other persons, dyslexics have more and longer fixations, shorter saccade amplitude, a higher percentage of regression, and more fixation disparity when reading. In non-reading situations, dyslexics do not have more binocular problems than others. The aim of the present study was to investigate whether computerized orthoptic vergence training could improve reading ability for dyslexic children.
METHODS: The study was conducted at Ängkärrskolan, Solna, an e

Prior studies have pointed toward a link between the saccadic and vergence systems, coordinating binocular saccadic movements. Recent studies have shown that vergence deficits in children induce poor binocular coordination during saccades, but none of them have studied ocular motility in children during a daily task such as reading. The present study tests whether vergence deficits in children perturb binocular coordination of saccades and fixation during reading. Our second objective was to explore whether vergence training could improve the quality of binocular coordination. Twelve patients

BACKGROUND: Treatment for intermittent exotropia X(T) aims to keep the eye in a phoric position and to maintain the phoria. However, maintenance of phoria is difficult even after treatment, and the cause is unclear. The aim of this study was to investigate the presence of suppression during tropia and/or phoria in X(T), and to determine how the suppression affected patient's ability to maintain phoria.
METHODS: Medical records of 89 children with X(T) (mean age, 9.8 ± 2.7 years) were reviewed retrospectively. According to their previous treatment for X(T), the patients were divided into four

We report a rare case of double disinsertion of inferior and medial rectus muscles of the left eye. After injury, the patient presented ocular ascension. The eye's position made the clinical examination difficult. The patient underwent two surgical treatments to find and reattach the deficient muscles. In spite of a laborious search, we obtained a good anatomic result. Oculomotor disorders require orthoptic training. To avoid diagnostic errors and to reduce oculomotor after effects, vigilance is required during examination.

Vergence abnormalities could lead to inappropriate vestibulo-ocular reflex (VOR), causing vertigo and imbalance (Brandt 1999). Indeed, a recent study by Anoh-Tanon et al. (2000) reported the existence of a population of children with symptoms of vertigo in the absence of vestibular dysfunction but with abnormal vergence findings in orthoptic tests. The purpose of this study was to examine in such children the accuracy, duration and mean velocity of vergence and saccades; additionally, for a few subjects, the effect of orthoptic vergence training on these parameters was also investigated. BACKGROUND: In the evaluation of therapies aiming at binocular vision, for instance by the use of prisms or orthoptic training in the case of heterophoria, stereoacuity is often the primary outcome measure. To assess therapeutic effects it is necessary to separate them from perceptual learning with repeated testing. Learning stereoacuity has been investigated only in a few studies with up to six subjects.
METHODS: To ascertain the interindividual variability of learning in stereoacuity we examined 24 subjects, 12 with and 12 without experience in psychophysical experiments. In a two-alternati

This paper discusses the use of vertical fixation disparity (VFD) measurements to determine a patient's ability to adapt to vertical prism. Vertical vergence adaptation for orthophoric patients, hyperphoric patients, and patients with vertical deviations associated with superior oblique paresis is reported. The effect of orthoptic training on the ability to adapt to vertical prism is also reported.

Accommodative spasm (AS) is rarely reported in the literature. We studied 17 patients with accommodative spasm. Most patients were clinically emmetropic. Ten patients also manifested a spasm of the near reflex (SNR). The probable etiology of the accommodative disorder for most patients was psychogenic as revealed by case histories and visual field analysis. Treatment consisted primarily of plus reading lenses and, in some instances, orthoptic training. Some patients also underwent psychological counseling. Follow-up ranged from 2 months to 30 months. Although visual symptoms improved for most

Twelve exotropes of various types received oculomotor biofeedback therapy at State College of New York (SUNY) University Optometric Center. Feedback of a variable pitch tone which reflected changes in ocular vergence reinforced motor control of eye posture. Patients were trained to achieve and sustain alignment in a variety of viewing situations. The six intermittent exotropes in the study who did not have amblyopia or prior history of unsuccessful surgical or orthoptic therapy achieved the highest recovery rating after training. The amblyope and those who had orthoptic training learned to vol
