At three community mental health centers (CMHCs) in Indiana, 167 clients at risk for rehospitalization were randomly assigned to experimental groups receiving assertive case management (ACM) or to control groups eligible to receive all other aftercare services at the centers. During a six-month follow-up period, experimental clients received an average of one visit a week from the ACM team, usually in the client's home or in community settings. Overall, ACM clients were rehospitalized an average of 9.2 days, significantly less than the 30.8 days for controls. In two of the three centers, signi

A mail survey of clinics in the Federal Republic of Germany revealed that aphasia assessment is performed primarily by speech & language pathologists, and medical specialists. Psychologists, linguists, and speech educators, on the other hand, are involved to a much lesser degree in the process of aphasia diagnosis. Diagnosticians primarily rely on psychometric aphasia testing. Utilization of these procedures appears to be motivated by their high degree of formalization, which facilitates diagnostic objectivity, and by the good selection, syndrome classification, and severity determination this

The paper discusses the results of treatment of 102 cases of locally-advanced malignant tumors of the head and neck. Myocutaneous flaps including m.m. platysma, sternocleidomastoideus, trapezius and pectoralis major were used to rectify primary postexcisional defects. Preoperative radiation treatment (30-100 Gy) was given to 92 patients. Primary reconstruction for defects of the head and neck failed to stimulate recurrence development or affect survival as compared to controls. Frequency of complications involved in the said method proved lower while stay at the hospital shorter as compared to

A survey of all American Veterinary Medical Association members in Minnesota and Wisconsin was conducted by questionnaire to document injuries resulting from animal treatment. Of 995 respondents, 64.6% had sustained a major animal-related injury. Seventeen per cent were hospitalized within the last year. Of those hospitalized, 25.3% required a surgical procedure. Hand injuries were most common in a veterinarian's career (52.6% of respondents), followed by trauma to the arms (27.6%), and the head (20.8%). The thorax (8.3%), genitalia (3.9%), and intra-abdominal viscera (2.8%) were injured less

More than 4,000 human heart transplants have been performed worldwide since the inception of this procedure in 1967. More than half have occurred in the last two years, and current survival rates at one and five years are 81% and 78%, respectively. This increased incidence and survival has been attributed to the advent of cyclosporine as an immunosuppressive agent used to prevent graft rejection. As the obstacles to cardiac transplantation are overcome, more patients will require rehabilitation due to related neurologic sequelae. In the case reported, a 56-year-old man was admitted for rehabil

This study investigated the degree of adherence to different aspects of the diabetic treatment regimen (insulin injections, glucose testing, diet, and exercise), the consistency of adherence across different regimen areas, and relationships between adherence and glycemic control. A predominantly adult community sample of 93 insulin dependent outpatients participated in the prospective study that involved two series of home interviews separated by a 6-month interval. Multiple measures of adherence (e.g. self-report, interview/recall, self-monitoring, and objective indices) were collected for ea

This article describes the Role Activity Performance Scale (RAPS) and presents data from a study of its reliability and validity. The RAPS evaluates an individual's functional level, using 12 subscales that represent a range of life roles. It was developed as an instrument for evaluating the impact of occupational therapy and other treatment modalities on the functioning of psychiatric patients. It is also useful in the diagnostic, treatment, and discharge planning processes. Validity and interrater reliability testing involved a sample of 30 patients in two major diagnostic groups. Today, patients who have a diagnosis of cancer often face the same concerns as persons who have other types of chronic illnesses. The effects of the disease, the treatment, or both, may leave patients with residual impairments that necessitate sometimes prolonged periods of rehabilitation. Rehabilitation may be preventive, restorative, supportive, or palliative, depending on the type and stage of the disease and characteristics of the patient. Rehabilitation is most effectively planned and carried out by a team composed of various disciplines, as appropriate, but always with the patient and fa

The state of the art in laryngeal cancer treatment in the RSFSR is outlined on the basis of an analysis of morbidity and other statistical data. Measures which can be taken to considerably improve the effectiveness of treatment are discussed. The importance of a complex of diagnostic procedures to be employed in therapy planning is shown. Preoperative HBO-mediated gamma therapy and mean fractionation of radiation were used to improve combined treatment. The following function-saving procedures were suggested for radical removal of stage III laryngeal malignancies: an endoprosthetic procedure,

Social skills training methods represent a major strategy for psychiatric rehabilitation. Building skills in patients with schizophrenic and other major mental disorders is based on the assumption that coping and competence can override stress and vulnerability in reducing relapses and improving psychosocial functioning. For maximum efficiency, skills training needs to incorporate procedures and principles of human learning and information processing. Several models for skills training have been designed and evaluated, each of which has proved to be effective in raising the social competence o

This study was conducted over a 14-month period to assess the physical therapy intervention needs of 201 patients who attended monthly spinal cord injury (SCI) outpatient clinics conducted in three outlying local communities of northern California. Methods to identify and provide appropriate physical therapy services for the patients were explored. An experienced physical therapist from a regional SCI center and other clinic staff members screened the patients and identified 66 patients (33%) who needed physical therapy services, including evaluation (82%), patient education (62%), and referra

Patient and family education is essential in all settings, not only the rehabilitative setting. The patient may be in a variety of settings: the intensive care unit, a general medical or surgical unit, an outpatient clinic, or at home. The health care professional will need to determine what the patient has to learn. Teaching can be done by all members of the multidisciplinary team, and the education that has been completed should be communicated to other members through a good record keeping system. It can never be assumed that the patient already knows something. For example, many patients w

A workplace health education program aimed at reducing unnecessary outpatient visits was offered through 22 California employers. A total of 5,200 employees attended a presentation, received self-help books, and completed self-administered questionnaires. All visits for their households were monitored for up to 15 months. The study utilized a quasi-experimental staggered intervention design involving eight different cohorts, before/after comparisons, with statistical adjustment for seasonality and other intervening variables. Visit rates for households insured only by Blue Cross of California

A structured, goal-oriented format for enhancing the involvement of activity therapy disciplines in the multidisciplinary treatment planning process has been developed in a large private psychiatric teaching hospital. The format, an adaptation of the problem-oriented record, encompasses formal procedures for identifying and recording relevant problems, goals, methods, and objectives for activity therapy treatment. The benefits of this approach include the development of specific, measurable, attainable functional goals; increased accountability in treatment planning and delivery; less time spe

During 1979-1984, 240 outpatients aged 30 to 65 years suffering from primary osteoarthrosis deformans with primary involvement of the knee joints were followed up. One of the dispensary groups consisted of 180 patients with osteoarthrosis deformans without synovitis, the other one of 60 patients with associated osteoarthrosis and synovitis. Prolonged follow up and repeated multiple modality treatment inhibited the progression of the pathological process, reduced the clinical and functional disease manifestations permitting the patients' work fitness to be preserved.

The number of aged persons in the United States is rising steadily. Rehabilitation medicine is becoming increasingly involved in providing the care necessary to keep aging individuals in good health so that the onset of late life disability can be delayed and social independence be maintained for as long as possible. For this reason, education in geriatric rehabilitation must become an essential part of training programs for residents in physical medicine and rehabilitation. The curriculum of these programs must be broad based and must include many of the general aspects of geriatric medicine

Infection control surveillance in a rehabitation setting is essential to quality assurance because of the high number of infection risk factors found in this patient population. These factors include longer length of stay, high incidence of urinary tract instrumentation, and high incidence of skin lesions, including decubital and stump wounds. A surveillance process is described which offers timely access to each infected patient's diagnosis, treatment, and follow-up, and results in simplified hospital-wide antibiotic monitoring. Quality assurance problems identified through this surveillance

Outcome assessment attempts to measure the long-term impacts of illness in order to select management strategies and public policies which minimize these impacts. Disability is an important dimension of outcome, particularly in conditions such as rheumatoid arthritis, but cannot be considered apart from other dimensions such as mortality, pain, iatrogenic problems, and economic impact since decisions frequently involve comparing positive effects in one dimension with negative effects in another. Modern techniques of survey research have yielded self-administered patient questionnaire instrumen

Kinetic modeling, as the name implies, involves a study of the dynamics or rate of change of process or system to either gain enhanced understanding or predict an outcome. In applying kinetics to extracorporeal treatment both goals are encompassed. For example, in dialysis treatment the question of mortality has been replaced by questions of morbidity and rehabilitation. To improve the latter, it is essential to study the kinetics of interaction between the patient and the treatment process; mathematical models can provide both useful insights and better patient management. The application of

The health professional needs to make an educational diagnosis before he selects a specific approach to patient education. Tailoring a patient education program to the patient's belief regarding his health and the patient's expectations regarding his diagnosis and care is useful. Other factors that influence the pulmonary rehabilitation team's selection of educational methodology are the patient's age, sex, educational level, cultural background, and prior experience with his disease. An educational program oriented to patients who rely on self-motivation or self-reward should provide choice o
