Moving Beyond the Checklist: A Three-Tier Model for Integrating Geriatrics Expertise into the CMS Age-Friendly Era.

As the Age-Friendly Health Systems (AFHS) movement enters a new phase of policy integration, hospitals face a pivotal moment. The U.S. Centers for Medicare and Medicaid Services (CMS) Age Friendly Hospital Measure has created regulatory conditions necessary for hospital systems to institute high-quality, personalized care for older adults. As the CMS is expected to transition from pay-for-reporting to pay-for-performance for age-friendly care in the coming years, health systems must urgently move beyond reporting compliance toward true care redesign. The authors highlight three interrelated st

End-of-Life Pathways and Limitation of Life-Sustaining Treatment in Children with Pre-existing Neurological Disorders: A 10-Year Retrospective Cohort Study.

BACKGROUND AND OBJECTIVE: Limitation of life-sustaining treatment (LSTL) is an important component of pediatric end-of-life care, but its use in children with pre-existing neurologic disorders remains insufficiently described. We aimed to characterize end-of-life pathways, LSTL practices, and decision-making processes in this population. METHODS: We conducted a retrospective observational cohort study of children with pre-existing neurologic disorders who died in a tertiary pediatric hospital between 2015 and 2024. Clinical characteristics and end-of-life care were compared between children w

TIRELESS study protocol: non-inferiority study of nurse-led web-based cognitive behavioral therapy for cancer-related fatigue in patients with incurable cancer.

BACKGROUND: Fatigue is a prevalent and debilitating symptom for patients with incurable cancer. Cognitive behavioral therapy (CBT) can significantly reduce cancer-related fatigue in this patient group. However, implementation of CBT into clinical care is difficult as the intervention is time-intensive, costly, and requires face-to-face consultations with a specialized CBT therapist. Since nurses already support patient with symptom management, including fatigue, internet supported CBT delivered by nurses could be a promising alternative for patients with incurable cancer. The primary aim of th

Understanding symptom occurrence and treatment in the palliative phase for people with intellectual disabilities: a protocol for a mixed-methods study.

BACKGROUND: Despite advances in palliative care, people with intellectual disabilities (ID) still face major barriers in accessing timely and appropriate care. They often experience complex symptoms that may be expressed in atypical ways, in combination with multimorbidity and communication challenges. These factors complicate both symptom recognition and treatment. There is limited knowledge on how symptoms evolve and are managed in people with ID during the palliative phases. Better understanding of symptom occurrence and treatment responses can support tailored care. AIM/RESEARCH QUESTION

Interprofessional education in palliative care: a theory-informed evaluation of a storyline-based curriculum for health and social care students.

BACKGROUND: Interprofessional education (IPE) in palliative care is essential for fostering collaborative, patient-centred practice. However, existing programs often underrepresent non-medical professions and insufficiently address spiritual care, ethical reflection, and experiential learning, despite the relevance of these aspects to comprehensive, holistic palliative care. This study evaluates an innovative interprofessional curriculum integrating the Canadian Interprofessional Health Collaborative (CIHC) framework, Social Identity Theory (SIT), and Transformative Learning Theory (TLT), desi

Problems and needs in palliative care questionnaire - caregiver form (PNPC-c): a cross-cultural adaptation and validation into Spanish.

BACKGROUND: Caregivers are the essential mainstay of care for patients under palliative treatment, and they experience problems and needs for which they require support, but these are rarely assessed. The Problems and Needs in Palliative Care questionnaire - caregiver form (PNPC-c) evaluates these issues, but it has not been adapted into Spanish. The aim of this study was to translate, cross-culturally adapt, and evaluate the psychometric properties to validate the PNPC-c into Spanish (PNPC-c SV®). METHODS: The translation and a cross-cultural adaptation of the PNPC-c comprised direct and rev

Factors Associated With Clinical Trial Participation in Adult Hematopoietic Stem Cell Transplantation Recipients: A Center for International Blood and Marrow Transplant Research (CIBMTR) Analysis.

BACKGROUND: Only 3%-5% of patients with cancer nationwide enroll in clinical trials. Although participation among patients with solid tumors has been well studied, data in the hematopoietic stem cell transplantation (HSCT) setting remain limited. We sought to (1) describe clinical trial participation rates by HSCT type; (2) examine associations between patient, disease, and transplant characteristics and trial participation; and (3) explore the association between trial participation and overall survival (OS). METHODS: We conducted a retrospective study using Center for International Blood an

Firstline treatment with ruxolitinib versus best available therapy in patients with polycythemia vera: results from the randomized RuxoBEAT trial.

Ruxolitinib (RUX) is approved as second-line treatment for HU-intolerant/-resistant patients with polycythemia vera (PV). Here, we assessed RUX vs. best available therapy (BAT) as first-line treatment in PV in the randomized multicenter RuxoBEAT clinical trial (NCT02577926). Crossover from BAT to RUX was possible after 6 months. Baseline characteristics in the RUX (n = 44) and BAT (n = 34) arms were comparable. The primary endpoint, the clinico-hematologic complete response (CR) rate at month 6, was not met (2.3% vs. 2.9%, p = 1.000). Analyses of secondary and exploratory endpoints showed that

Added value of a specialized mobile palliative care team compared to the usual care in the primary care setting of Croatia: a 3-month prospective cohort study.

BACKGROUND: Croatia has been implementing a Mobile multidisciplinary specialist palliative care team (MMSPCT), a home-based specialized palliative care service at the primary healthcare level since 2011. This study aimed to evaluate whether a MMSPCT performs better than usual care, in terms of health/well-being, experience with care, and cost outcomes. METHODS: A 3-month prospective cohort study was performed during 2017-2019. Two cohorts were compared: MMSPCT vs. usual care cohort. Sample size was based on the literature data, ranging 150-200 patients per group. A SELFIE questionnaire with s

Perineural versus Intravenous Dexmedetomidine for Ultrasound-guided Costoclavicular Brachial Plexus Block: A Randomized Controlled Trial.

BACKGROUND: Dexmedetomidine is a sought-after adjunct to prolong the effects of local anesthetics in regional anesthesia, administered through both perineural and intravenous (IV) routes. However, there is still an ongoing debate to ascertain the preferred route of administration. We conducted this trial because there is a lack of literature about the comparative advantages of these approaches in costoclavicular brachial plexus block (CC-BPB). METHODS: Sixty subjects were randomized to Group A (received 0.5 μg/kg dexmedetomidine plus 20 mL of 0.5% ropivacaine administered perineurally, preced

A Web-Based Registry Framework Linking Transfusion Practice to Long-Term Patient Outcomes: Design and Single-Center Feasibility Study.

Blood transfusions are among the most common medical interventions performed worldwide. Several comprehensive databases link transfusions to routinely collected health data but do not include the indication and the decision-making basis for a particular administration, nor patient-reported long-term outcomes. We developed a web-based registry for the standardized assessment of transfusion practice and patient outcomes, designed for an observation period of up to 30 years. The framework combines clinical data captured at the point of care with patient-reported outcome measures collected through

The prognostic value of KRAS, NRAS, and BRAF genes mutations in patients with newly diagnosed multiple myeloma.

BACKGROUND: KRAS, NRAS, and BRAF (RAS/RAF) genes mutations play a role in the pathogenesis of multiple myeloma (MM). We conducted this study to investigate whether RAS/RAF genes mutations have a prognostic value in patients with MM. MATERIAL AND METHODS: 117 newly diagnosed MM patients underwent the FISH technique to detect high-risk cytogenetic abnormalities and next-generation sequencing (NGS) to detect KRAS, NRAS, BRAF and TP53 mutations. RESULTS: In the study patient group, patients with the KRAS mutation had significantly longer OS and PFS compared to those with the TP53 mutation; patie

Factors predicting blood transfusion in endoscopic metopic strip craniectomy-analysis of 102 consecutive cases.

PURPOSE: Endoscopic strip craniectomy (ESC) provides a safe surgical approach for the management of metopic craniosynostosis. However, it has a higher reported blood transfusion requirement than ESC for other sutures. We sought identify factors that may be related to greater blood transfusion use. METHOD: Retrospective consecutive case series. All metopic ESC cases were identified from the institutional database. Case notes reviewed following pre-drafted proforma, with regression analysis to identify risk factors for transfusion. RESULTS: One hundred two consecutive cases of ESC for metopic

Efficacy and safety of mitapivat in patients with sickle cell disease (RISE UP): a phase 3, double-blind, global, randomised, placebo-controlled trial.

BACKGROUND: Persistent haemolytic anaemia and recurrent vaso-occlusion drive complications and accelerate mortality in sickle cell disease. Mitapivat, an oral allosteric activator of pyruvate kinase, increases ATP and decreases 2,3-DPG with the potential to improve haemolytic anaemia and reduce sickling. We aimed to evaluate the efficacy and safety of mitapivat in patients with sickle cell disease. METHODS: We report results from RISE UP, a phase 3, double-blind, global, randomised, placebo-controlled trial from 69 sites across 15 countries. Patients older than 16 years with anaemia and recur

Biological markers of grief in bereaved family members: A systematic review of cortisol, inflammatory and cardiovascular studies.

Bereavement is among life's most stressful events, and a minority of mourners develop prolonged and disabling grief. Biological markers have been proposed as signals of this risk, but no systematic review has examined them across bereaved family members or asked how well bereaved parents (parents who have lost a child) are represented. We systematically reviewed observational studies linking a cortisol/HPA-axis, inflammatory/immune, or cardiovascular/autonomic biomarker to a validated measure of coping or grief in bereaved family members of any age or relationship to the deceased (PROSPERO CRD

Paediatric palliative care in advanced cystic fibrosis: from prognosis-based to needs-based care across the disease trajectory.

Advances in cystic fibrosis care, particularly highly effective cystic fibrosis transmembrane conductance regulator modulator therapies, have transformed disease trajectories for many children and adolescents, improving health and survival. However, advanced disease, symptom and treatment burden, psychosocial distress, and uncertainty persist, requiring continued attention to palliative care needs. This narrative review examines palliative care in children and adolescents with advanced cystic fibrosis, focusing on multidimensional symptom burden, communication, shared decision-making, advance
