Artificial intelligence-based approaches for advance care planning: a scoping review.

BACKGROUND: Advance Care Planning (ACP) empowers individuals to make informed decisions about their future healthcare. However, barriers including time constraints and a lack of clarity on professional responsibilities for ACP hinder its implementation. The application of artificial intelligence (AI) could potentially optimise elements of ACP in practice by, for example, identifying patients for whom ACP may be relevant and aiding ACP-related decision-making. However, it is unclear how applications of AI for ACP are currently being used in the delivery of palliative care. OBJECTIVES: To explo

Narrative identity at the end of life: a qualitative analysis of dignity therapy interviews from an existential perspective.

BACKGROUND: Confronting terminal illness and the prospect of death and dying can severely undermine an individual's sense of identity, meaning and purpose in life, leading to existential suffering, increasing hopelessness, depressive symptoms, and death anxieties. OBJECTIVES: This study explored how narrative approaches can alleviate existential suffering, promote emotional healing, and consolidate identity for individuals confronted with death and dying. Specifically, the study aimed to (1) analyze dimensions of identity reflected in Dignity Therapy (DT) legacy documents from a randomized co

Baseline characteristics of people experiencing homelessness in the PHOENIx community pharmacy multicentre pilot randomised controlled trial.

BACKGROUND: People experiencing homelessness (PEH) are amongst the most marginalised populations in the society. Despite facing extreme health inequalities and poorer outcomes than the general population, they are under-represented in clinical trials. Little is hence known about the physical, mental health and social characteristics of PEH as clinical trial participants. Cross-sectoral complex interventions aiming to mitigate health, wellbeing and wider inequalities faced by PEH are urgently needed. METHODS: This paper reports on baseline characteristics of PEH from the PHOENIx Community Pharm

Fasting at Life's End: Exploring the Spiritual and Medical Tensions in Palliative Care.

Voluntary fasting at the end of life raises complex spiritual, ethical, and clinical challenges for palliative care providers. This qualitative study explored healthcare professionals' experiences in managing patient-initiated fasting within a culturally and religiously diverse palliative care context in Saudi Arabia. Sixteen participants-including nurses, physicians, and allied health professionals-were interviewed using a phenomenological-hermeneutic approach and analyzed through reflexive thematic analysis. Four interrelated themes emerged: (1) between respect and responsibility, reflecting

Online Role-Play in Palliative Care Education for Early-Career Nurses in Japan: A Pre-Post Evaluation of Perceived Impact and Feasibility.

BACKGROUND: As the global incidence of cancer rises, high-quality palliative care is increasingly needed. Early-career nurses often find symptom management and communication challenging. Although role-playing is a promising educational approach, its effectiveness in fully online formats remains underexplored. OBJECTIVE: This study aimed to examine the feasibility and preliminary effectiveness of a fully online, role-play-based palliative care education program and to explore participants' experiences with this program. METHODS: This prospective observational study included 31 nurses with 2-4

A mixture of postbiotics/tyndallized probiotics reduces trimethylamine (TMA) in trimethylaminuria models: Evidence from in vitro and in vivo studies.

INTRODUCTION: Trimethylaminuria (TMAU), also known as "fish-odor syndrome," is a rare metabolic disorder characterized by a body malodor that smells like a decaying fish. This syndrome is caused by a FMO3 liver enzyme malfunction, leading to trimethylamine (TMA) accumulation. To date, there is no definitive therapeutic treatment but only palliative care for TMAU, such as a controlled diet, taking antibiotics, or using acidic soaps to capture sweat-released TMA. METHODS: Here, we describe an innovative approach for the treatment of this disorder, where the use of postbiotics/tyndallized probio

Outcomes and prognostic insights in primary tracheal cancer: a multicenter retrospective study.

BACKGROUND: Primary tracheal tumors are exceedingly rare, comprising less than 0.1% of all respiratory system cancers and only 0.02-0.04% of all malignancies. Their rarity and clinical heterogeneity present significant challenges for diagnosis and treatment. This study aimed to evaluate treatment outcomes, identify prognostic factors, and compare recent findings with historical data to assess changes in clinical practice. METHODS: We conducted a retrospective analysis of 31 patients treated between 2017 and 2022 at Maria Skłodowska-Curie National Research Institute of Oncology, Warsaw and Gli

Barriers and Facilitators for Implementation of Palliative Care for Patients with Heart failure - a Rapid Synthesis of Reviews.

PURPOSE OF THE REVIEW: While the benefits and need for integrating palliative care (PC) services into heart failure (HF) care have been well recognized, integration has been slow. Currently, only a small proportion of patients with HF utilize PC services, meaning there is a discrepancy between the knowledge about optimal care and the implementation of care. In recent years researchers have attempted to understand the barriers and facilitators of implementing PC models to patients with HF which has been captured in both primary, empirical research and secondary literature review papers. We aime

Developing a programme theory of implementing patient-reported outcome measures for older people living with severe frailty: a mixed methods study using the consolidated framework for implementation research.

BACKGROUND: Older people with severe frailty (OPWSF) have palliative needs but typically do not receive specialist palliative care (SPC). Patient-Reported Outcome Measures (PROMs) may offer valuable means to capture these needs. There is a limited understanding of what to include and how to implement PROMs in settings where this group receive most care. The study aimed to: (1) Critically examine how existing PROMs are currently implemented with OPWSF within a SPC setting. (2) Understand how the items with the PROMs are used (3) Develop a programme theory to determine how PROMs can be optimally

Heart Failure with Reduced Ejection Fraction.

Heart failure (HF) is no longer centered on the failing ventricle. Various salutary treatment discoveries now support substantially improved survival with lesser likelihood for urgent care or hospitalization. Careful assessment of comorbidities and early recognition of symptoms further support the care for patients with HF. Advanced care strategies are effective, but not only includes mechanical circulatory assist and heart transplantation, but also clinical trial participation, palliative care, and hospice. With this contemporary view of HF, failure is supplanted with opportunity. At the pati
