Extraperitoneal Single-Port Robotic-Assisted Radical Cystectomy with Orthotopic Ne-obladder: Technique and Surgical Considerations.

INTRODUCTION: Radical cystectomy (RC) is the standard therapy for muscle-invasive bladder cancer (MIBC) and refractory high-risk non-muscle-invasive bladder cancer (1-3). Single-port robot-assisted radical cystectomy (RARC) offers notable minimally invasive advantages (4-6), whereas conventional transperitoneal approaches are associated with intestinal and gastrointestinal complications (7). This study evaluated an optimized extraperitoneal single-port RARC with orthotopic neobladder reconstruction for improved surgical safety and clinical outcomes. MATERIALS AND METHODS: A 57-year-old male p

Efficacy and Safety of Tranexamic Acid in Endoscopic Surgery for Benign Prostatic Hyperplasia: A TURP-Dominated Systematic Review and Meta-Analysis.

BACKGROUND: Hemorrhaging continues to be a significant issue in endoscopic procedures for benign prostatic hyperplasia (BPH). Tranexamic acid (TXA) has garnered heightened scrutiny for its ability to mitigate bleeding-associated problems during transurethral prostate procedures. We performed a comprehensive meta-analysis to assess the effectiveness and safety of TXA in individuals having endoscopic procedures for BPH, acknowledging that the existing data primarily originated from transurethral resection of the prostate (TURP). METHODS: PubMed, Web of Science, Cochrane Library, Embase, CNKI, W

The antifungal and antibiofilm effects of isobavachalcone on Candida glabrata (Nakaseomyces glabratus) are associated with intracellular ROS production and cell membrane damages.

Millions of fungal infections are reported every year, while the current antifungal drugs are so few that the clinical choices are limited. The opportunistic fungal pathogen Candida glabrata (reclassified as Nakaseomyces glabratus) caused more infections than any other Candida species except C. albicans. This research sought to explore the antifungal effects of isobavachalcone (IBC, a bioactive chalcone from Psoralea corylifolia) on C. glabrata planktonic growth and biofilms. Our results showed that IBC has potent antifungal activity against C. glabrata (MIC: 4-5 μg/mL), including both the sta

Tonsillar metastasis from hepatocellular carcinoma: a case report and literature review.

Tonsillar metastasis from hepatocellular carcinoma (HCC) is extremely rare. We report an 84-year-old man with chronic hepatitis B who presented with a right-lobe HCC and portal vein tumor thrombosis. Three months after diagnosis, intrahepatic metastases were treated with transarterial chemoembolization and lenvatinib. Twenty-three months later, he developed blood-tinged sputum; imaging and laryngoscopy revealed a left tonsillar mass with concurrent pulmonary and hepatic progression. Tonsillectomy yielded an undifferentiated carcinoma lesion. Compared with the primary tumor, the tonsillar metas

Double Trouble: Dual Positivity for Antiglomerular Basement Membrane Antibody and proteinase-3-antineutrophil Cytoplasmic Antibody Presenting as Rapidly Progressive Glomerulonephritis.

Antiglomerular basement membrane (anti-GBM) disease and antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis are uncommon causes of rapidly progressive glomerulonephritis. Their coexistence, termed double-positive disease, is rare and associated with severe renal and pulmonary manifestations. A 27-year-old male came with complaints of inflammatory polyarthralgia followed by progressive dyspnea and hemoptysis. Investigations revealed azotemia, active urinary sediment with proteinuria, bilateral pulmonary infiltrates, and dual seropositivity for anti-GBM antibodies and proteinase-3-A

Rouleaux mimicking anti-H in molecularly confirmed para-Bombay siblings: a diagnostic pitfall.

The para-Bombay phenotype is a rare H-deficient blood group variant characterized by markedly reduced or absent expression of H on red blood cells (RBCs) because of mutations in FUT1, with variable secretor status influenced by FUT2. The Bombay phenotype is relatively more common in the Indian population, but the para-Bombay phenotype remains exceedingly rare and is frequently underrecognized because of diagnostic challenges during routine serologic testing. We report molecularly confirmed para-Bombay siblings from India in whom rouleaux-induced pseudoagglutination mimicked anti-H reactiv ity.

D-- phenotype and anti-Rh17 alloimmunization: a comprehensive review of identification and clinical management.

The D-- phenotype is an exceptionally rare Rh blood group phenotype characterized by expression of D in the complete absence of C, c, E, and e. Individuals with this phenotype are at risk of developing anti-Rh17 (anti-Hr0), a clinically significant alloantibody that can severely complicate transfusion support and pregnancy management because compatible blood is exceptionally difficult to obtain. This review critically synthesizes the available published evidence regarding the D-- phenotype and anti-Rh17 alloimmunization, integrating data from case reports, case series, family investigations, a

Healthcare Providers' Practices in Postpartum Hemorrhage Management: A Multi-Centre Cross-Sectional Study in Tanzania.

BACKGROUND: Postpartum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality in Tanzania. Accurate diagnosis, proper documentation, and effective patient communication including disclosure and informed consent are essential for quality PPH care. However, evidence on these practices among healthcare providers (HCPs) is limited. This study assessed HCPs practices related to PPH diagnosis, documentation, disclosure and consent in Comprehensive Emergency Obstetric and Neonatal Care (CEmONC) facilities. METHODS: A facility-based cross-sectional study was conducted between Oc

IRF4 p.T95R combined immunodeficiency: Clinical features and transplant outcomes.

IRF4 is a transcription factor within the interferon regulatory factor family that controls key aspects of lymphocyte differentiation and adaptive immunity. The heterozygous IRF4 p.Thr95Arg (p.T95R) variant affecting the DNA-binding domain was recently identified to cause a new inborn error of immunity, but the longitudinal clinical course and therapeutic implications remain incompletely defined. We performed detailed clinical characterization and follow-up of all 10 known patients. Each presented with early-onset combined immunodeficiency characterized by hypo- or agammaglobulinemia and marke

Future-proofing thalassaemia care: harnessing scalable therapies, artificial intelligence, and global collaboration.

Advances in the treatment of thalassaemia have led to major improvements in survival and quality of life over the past 30 years. In addition to developments in stem-cell transplantation, transfusion safety, iron overload monitoring, and iron-chelation therapy, novel disease-modifying and gene therapies are also emerging. However, substantial disparities in access to specialised care and novel therapies persist, particularly in low-income and middle-income countries, where most people with thalassaemia reside. This Series paper explores complementary strategies aimed at improving global thalass
