Disease Burden in Transfusion-Dependent β-Thalassemia (TDT) Patients and Caregivers in Italy: Results of a Cross-Sectional Descriptive Online Survey.

BACKGROUND: Patients with transfusion-dependent thalassemia (TDT) require lifelong blood transfusions; data on the psychosocial and economic impact on patients and their caregivers remain limited. The study aims to address this gap by describing the burden of TDT on pediatric, young, and adult patients, as well as their caregivers, within real-world settings in Italy. METHODOLOGY: This is a cross-sectional, descriptive study conducted between May and December 2021. TDT patients and their caregivers from Italian sites were invited to complete an online survey with the Ratip and Modell question

Breakthrough invasive pulmonary Coprinopsis cinerea (Hormographiella aspergillata) infection successfully managed with antifungal therapy and surgical resection enabling cord blood transplantation.

Coprinopsis cinerea (C. cinerea, also known as Hormographiella aspergillata) is a rare basidiomycete that causes invasive fungal infections in immunocompromised patients and is often difficult to diagnose microbiologically. We report a case of breakthrough proven invasive pulmonary fungal infection caused by C. cinerea during antifungal prophylaxis in a patient with acute myeloid leukemia undergoing induction chemotherapy. During the neutropenic phase, chest computed tomography (CT) revealed a nodular lesion in the right lower lobe that progressed despite antifungal prophylaxis, raising suspic

Transfusion-related alpha-gal syndrome: Two new cases expanding the demographic and geographic spectrum, and evidence of a diagnostic gap in allergic transfusion reaction evaluation.

BACKGROUND: Transfusion-related alpha-gal syndrome (TRAGS) has recently been proposed as a cause of allergic transfusion reactions (ATRs) in which alpha-gal-specific IgE in sensitized group O (or potentially group A) recipients reacts with epitopes on group B or AB plasma-containing components. Fewer than 10 cases have been reported, all from the Northeast and mid-Atlantic United States, and alpha-gal-specific ATR evaluation practices are unstudied. STUDY DESIGN AND METHODS: Two patients with ATRs consistent with TRAGS at a large academic medical center in Nashville, Tennessee are reported al

Highly effective suppression of herpes simplex virus type 1 by titanium dioxide nanoparticles.

BACKGROUND AND AIMS: Herpes simplex virus type 1 (HSV-1) is an important clinical problem due to its drug resistance and reactivation of infections, and new antiviral agents are required. Titanium dioxide (TiO2) nanoparticles, due to their high photocatalytic activity, lack of toxicity and biocompatibility, were claimed possible candidates in antiviral therapy. The purpose of this work was to investigate the antiviral effect of TiO2 nanoparticles in HSV-1 infection, and evaluate their biocompatibility on Vero cells. METHODS: TiO2 nanoparticles were characterized for their structure by X-ray d

Validation and implementation of the FLHASc score for risk stratification of patients with hemoptysis in the emergency department.

BACKGROUND: Hemoptysis is a potentially life-threatening symptom that often prompts emergency department (ED) evaluation and hospitalization. Reliable prognostic tools to guide clinical decision-making and optimize resource use are currently lacking. OBJECTIVE: To prospectively validate the Florence Hemoptysis Assessment Score (FLHASc) in patients presenting with hemoptysis to the ED, and to eventually derive and validate an improved version of the score (FLHASc2). METHODS: We analyzed data from the POPEIHE study (NCT06067997), a multicenter prospective cohort of 546 consecutive adult patien

Long-Term Clinical and Molecular Dynamics in Hypoplastic Myelodysplastic Neoplasia Treated With Immunosuppressive Therapy.

Myelodysplastic neoplasia (MDS) comprises heterogeneous clonal hematologic disorders characterized by peripheral cytopenia, bone marrow dysplasia, and a risk of leukemic transformation. A hypoplastic variant (MDS-h) shares features with aplastic anemia and responds to immunosuppressive therapy (IST). We report three low-risk MDS-h cases treated with IST and monitored over 9-17 years using serial cytogenetic and molecular analyses. All patients achieved transfusion independence after IST, and two experienced durable, long-term remissions. One patient developed late clonal evolution culminating

Prognostic value of severe hypocalcemia in predicting mortality and transfusion in moderate-to-severe trauma.

BACKGROUND: Hypocalcemia is a common metabolic disturbance in trauma and may contribute to coagulopathy, hemodynamic instability, and adverse outcomes. This study aimed to investigate whether admission ionized calcium (iCa) levels are associated with in-hospital mortality and transfusion requirements in moderate-to-severe trauma patients. METHODS: This retrospective cohort study included adults (≥ 18 years) with moderate-to-severe trauma, defined as an Injury Severity Score (ISS) ≥ 9, admitted to the emergency department of a tertiary university hospital between January 2019 and December 2024.

Robotic Caval Replacement for Leiomyosarcoma of the Inferior Vena Cava.

BACKGROUND: Primary leiomyosarcoma of the inferior vena cava (IVC) is a rare malignant smooth muscle tumor, representing fewer than 1 in 100,000 adult cancers and less than 0.5% of soft tissue sarcomas. Despite its rarity, it is the most common primary tumor of the IVC and typically aff ects women in their 50s to 60s. Clinical presentation is often silent or nonspecific, resulting in late diagnosis and poor long-term outcomes. Surgical resection remains the only potentially curative treatment, and IVC reconstruction may be necessary depending on tumor location and extent. METHODS: We present

Efficacy and Safety of Tranexamic Acid Use in Bariatric Surgery: An Updated Systematic Review and Meta-Analysis.

Intravenous tranexamic acid (TXA) is increasingly used to reduce perioperative bleeding. Its safety in bariatric surgery, a population with chronic inflammation and hypercoagulability, remains unclear. Following PRISMA guidelines, 11 comparative studies (five RCTs, four retrospective cohorts, two prospective cohorts; n = 1,959) were analyzed. TXA significantly reduced transfusion risk (RR 0.29, 95%CI 0.11–0.80), bleeding events (RR 0.46, 95%CI 0.32–0.67), postoperative hemoglobin reduction (MD 0.24 g/dL, 95%CI 0.04–0.44), length of stay, and operative time. Intraoperative blood loss was unchan

Thermal anomaly of erythrocyte osmotic fragility and aquaporin function at body temperature.

The recently proposed Interfacial Water Quantum Transition (IWQ) model presents a novel framework for functional changes in proteins at critical temperatures. Building on this, we hypothesized that red blood cell (RBC) osmotic fragility (OF) and aquaporin (AQP) function might exhibit similar transitions. We assessed the osmotic resistance of human and chicken RBCs across 24-50 °C. Mean corpuscular fragility (MCF50) was obtained by logistic curve fitting. In human RBCs, MCF50 consistently decreased with increasing temperature in whole blood, washed RBCs, and HgCl2-treated (known to reduce AQP w

Association Between Transfusion Status, Hemoglobin Levels, and Patient-Reported Outcomes in Myelofibrosis: A Post Hoc Clinical Trial Analysis.

BACKGROUND: Quality of life and symptom burden of patients with myelofibrosis are well recognized and compounded in those with anemia; however, the effects of transfusion burden or anemia severity on quality of life have not been comprehensively characterized. This post hoc descriptive analysis explored the association between transfusion status or hemoglobin improvement and patient-reported outcomes (PROs). METHODS: The analysis used pooled populations across treatment arms from 3 clinical trials (SIMPLIFY-1, SIMPLIFY-2, MOMENTUM); sample sizes for each PRO measure were dependent on the tria

A case report of soluble A antigen confounding the blood type in a haplo-identical hematopoietic stem cell transplant patient with graft-versus-host disease.

BACKGROUND: ABO-incompatible (ABOi) hematopoietic stem cell transplantation (HSCT) is often associated with ABO discrepancies post-HSCT. Re-emergence of the patient's original type post-HSCT may signal graft loss, disease relapse, recent transfusion, or soluble ABO antigen. We present a group A pediatric patient with leukemia who underwent a minor ABOi HSCT from a group O donor with persistent and increasing A reactivity 2 years post-HSCT. METHODS: The patient's electronic medical and laboratory records were reviewed. ABO, Lewis typing, and direct antiglobulin (DAT) tests were performed by co

Utilization and safety outcomes following the use of low titer group O whole blood in non-trauma patients.

BACKGROUND: Low Titer Group O Whole Blood (LTOWB) use has expanded to include patients with non-traumatic bleeding etiologies. While a large body of evidence supports a potential survival benefit of LTOWB in trauma patients, data demonstrating the safety and efficacy of LTOWB in non-trauma patients are lacking. STUDY DESIGN AND METHODS: Non-trauma adult patients at two hospitals who received at least one unit of LTOWB were included in the study. Patient demographics, transfusion and laboratory data, diagnoses, mortality, and hospital length of stay were collected between January 1, 2018, and
