Novel use of telitacicept in primary immune thrombocytopenia: a case report.

BACKGROUND: Primary immune thrombocytopenia (ITP) is generally a self-limiting disorder in children; however, a subset of cases becomes refractory to conventional treatments or progresses to chronic disease. Managing such refractory instances remains a significant clinical challenge. CASE PRESENTATION: We report the case of a 5-year-old child with refractory ITP that was not adequately controlled with standard therapies, including intravenous immunoglobulin, corticosteroids, and rituximab. Following disease recurrence and limited response to these interventions, the novel biologic agent telita

Case Study: HLA incompatible platelet infusion to allow emergency salvage HCT in a patient with primary graft failure due to donor specific HLA antibody.

A patient with myelodysplasia received a haploidentical hematopoietic stem cell transplant (HCT). Pre-transplant HLA antibody screening showed no detectable donor-specific antibodies (DSAs). Post-transplant, the patient developed an anti-HLA-A2 antibody, likely due to a memory B-cell response. This immune response led to platelet transfusion refractoriness and graft failure. The patient engrafted following a second stem cell infusion from the same haploidentical donor, using both HLA compatible platelets, to support initial platelet transfusion refractoriness (PTR) following primary stem cell

Association of Hemostatic Blood Product Transfusion With Clinical Outcomes in Esophageal Variceal Bleeding.

BACKGROUND/AIMS: Variceal bleeds occur due to portal hypertension and not primary hemostatic dysfunction. However, these patients frequently have low platelet counts and elevated international normalized ratio (INR) values, for which they often receive transfusions upon admission despite guidance from multiple liver societies. METHODS: We performed a retrospective analysis using the TriNetX United States Collaborative Network. Cohorts were defined based on transfusion of fresh frozen plasma (FFP), platelets, or cryoprecipitate within 72 h of variceal bleeding diagnosis. Propensity score match

Prophylactic Donor Lymphocyte Infusions in Pediatric Patients With High-Risk Hematological Malignancies.

INTRODUCTION: Allogeneic stem cell transplantation (allo-SCT) and donor lymphocyte infusions (DLI) can elicit a graft-versus-leukemia (GvL) effect in pediatric patients with hematological malignancies. We report our single-center experience with prophylactic DLI in high-risk pediatric patients with leukemia or lymphoma, focusing on feasibility, safety, and efficacy. METHODS: In total, 10 high-risk patients received prophylactic DLI following allo-SCT. Donors were either matched (n = 5) or haploidentical (n = 5). CD3+ T-cell doses of up to 1 × 107 cells/kg body weight were administered, in som

Fetal and neonatal alloimmune thrombocytopenia (FNAIT): From reactive transfusion to proactive prevention.

Fetal and neonatal alloimmune thrombocytopenia (FNAIT) is a major cause of severe thrombocytopenia, intracranial hemorrhage, and long-term neurological impairment in otherwise healthy neonates. Current management strategies are largely reactive, relying on maternal intravenous immunoglobulin (IVIG), intrauterine platelet transfusions, and postnatal supportive care. Although these interventions can be lifesaving, they are associated with considerable risks, inconsistent efficacy, and significant maternal and healthcare burdens, underscoring the need for a paradigm shift toward prevention. Deceased donor ABO discrepancy due to a rare AwB subgroup: Implications for organ allocation.

BACKGROUND: Accurate ABO determination is critical for safe deceased donor organ allocation. Weak or variant ABO subgroups may not be reliably detected by routine serologic testing, especially under urgent conditions or following transfusion. Such limitations can result in discrepant ABO assignments with potential implications for transplant safety. STUDY DESIGN AND METHODS: We describe a deceased organ donor with discordant ABO typing results reported by two laboratories. Serologic testing was performed using gel agglutination methods on independently collected specimens. Due to persistent d

Tranexamic acid in spinal surgery: a stratified protocol from the surgeon's point of view.

INTRODUCTION: Intraoperative bleeding in spinal surgery remains a major concern, given its association with increased morbidity, prolonged hospitalization, and greater transfusion requirements. Tranexamic acid (TXA), a synthetic antifibrinolytic, has shown consistent efficacy in minimizing blood loss across diverse surgical contexts. Nevertheless, its application in spinal surgery must be individualized, taking into account bleeding risk, surgical complexity, and patient comorbidities. RESEARCH QUESTION: Following the creation of a dedicated Spine Unit, our objective was to design and validat

Pelvic fracture related hematoma - emergency treatment and bleeding control.

PURPOSE: Unstable pelvic fractures present significant challenges in trauma care due to the risk of massive haemorrhage from arterial, venous, and osseous sources. Rapid identification of the bleeding origin is critical for targeted intervention. This multicentre retrospective study evaluated the use of the Pelvic Vascular Injury Score (P-VIS) and CT-based volumetric assessment (Kothari's method) for early detection and classification of pelvic vascular injury (PVI) in severely injured patients. METHODS: Data from January 2021 to December 2022 were collected from three German Level I Trauma C

Warm Acupuncture for Refractory Thrombocytopenia Induced by Cancer Treatment: Two Case Reports.

INTRODUCTION: Cancer treatment-induced thrombocytopenia (CTIT) is a common adverse complication during antineoplastic treatment. Although standardized western medical treatment is available, CTIT cannot be totally improved in some patients due to the severe degree of myelosuppression or low sensitivity to thrombopoietic agents. CASE PRESENTATION: We report two cases of CTIT that demonstrated insufficient response to conventional platelet-raising treatment, but achieved better curative effect after the addition of warm acupuncture therapy. Case 1: A 50-year-old man was diagnosed with right-sid

Whole blood storage time impairs clot strength with minimal change in functional fibrinogen concentration.

BACKGROUND: Whole blood (WB) is a preferred resuscitation strategy, yet how storage affects hemostasis remains unclear. Prior studies have shown decreased fibrinogen function during storage; however, these were limited by incomplete removal of platelet contributions. Additionally, leukoreduced (LR) versus non-leukoreduced (NLR) status in preserving fibrinogen remains unknown. STUDY METHODS: WB units (n = 15, 7 NLR, 8 nLR) were evaluated at serial time points through day 35. ROTEM EXTEM was performed. Platelet-poor plasma (PPP) from WB at day 0, day 21, and day 35 underwent western blot for fi

Severe aplastic anemia concurrent with lymphoplasmacytic lymphoma/Waldenström's macroglobulinemia: A case report.

A 79-year-old female presented with progressive dyspnea. A bone marrow biopsy revealed hypoplastic marrow with abnormal lymphoid cells. A genetic analysis revealed a MYD88 p.V204F mutation, supporting the diagnosis of lymphoplasmacytic lymphoma/Waldenström's macroglobulinemia (LPL/WM). Additional evaluations established a concomitant diagnosis of aplastic anemia (AA). Treatment prioritized AA with cyclosporine and eltrombopag. Subsequently, the LPL/WM was treated with rituximab monotherapy. This sequential treatment resulted in a symptomatic improvement. Although AA is a diagnosis of exclusion

Detection of human antibody responses to tick-borne Neoehrlichia mikurensis.

Tick-borne Neoehrlichia mikurensis is the cause of neoehrlichiosis, an infectious disease that features fever and vascular events. Compromised B-cell immunity is a risk factor for severe neoehrlichiosis, indicating the importance of antibodies in host defense. The development of serological assays has been hampered by the difficulty of culturing these intracellular bacteria. Here we present the first serological test for N. mikurensis, an ELISA for human IgM and IgG antibody responses to a P44/Msp2 protein of N. mikurensis. Serum or plasma from immunocompetent (n = 44) and immunosuppressed (n

High-Risk Chronic Lymphocytic Leukemia in a Young Adult Treated With Reduced-Dosage, Fixed-Duration Ibrutinib and Venetoclax.

Chronic lymphocytic leukemia (CLL) is a disease primarily of the elderly; however, in rare cases, it can occur among adolescents and young adults (AYAs). While evidence-based guidelines are established, the guidelines include elderly with very limited data among young adults. CLL in AYA is deemed as high-risk disease. They experience longer survival than their older age counterparts, but this longevity increases their susceptibility to secondary cancers, CLL-related complications, and treatment-related adverse events. Thus, effective treatment requires a careful balance of efficacy and safety.

Use of plasmapheresis during cardiopulmonary bypass in a pediatric heart transplant of a patient with Failing Fontan Physiology: first case in Argentina.

BACKGROUND: A 17-year-old male patient diagnosed with a single ventricle, in a failed Fontan stage, was evaluated prior to heart transplantation. The patient had a panel-reactive antibody (PRA) for human leucocyte antigen (HLA) I of 18% and for HLA II of 37%, so the decision was made to administer three doses of immunoglobulin while waiting for a donor heart. METHODS: Once extracorporeal circulation was initiated, the apheresis machine extracted blood from the patient's venous drainage and returned it to the oxygenator reservoir. A total of 8278 mL of blood was processed, and 4224 mL of plasm

Beyond HLA: An adaptive nanopore sequencing assay for simultaneous HLA and blood group profiling in transplantation.

AIM: Transplantation is a life-saving therapy but carries significant risks, including rejection, graft-versus-host disease, infection, and malignancy, which can lead to graft failure and death. These complications arise from the interaction between the recipient's immune system and the transplanted organ, requiring long-term toxic drugs to prevent immune injury. Traditional genetic pre-transplant assessment focuses on ABO and 11 human leukocyte antigen (HLA) genes, but advancing technologies now allow for the assessment of many more genes relevant to transplant success. METHODS: Using Oxford

Hemostatic resuscitation in patients with trauma-induced coagulopathy: a narrative review.

Hemorrhage remains a leading cause of preventable death in trauma, emphasizing the importance of early bleeding control. In addition to mechanical hemostasis, effective management of trauma-induced coagulopathy (TIC) plays a critical role in improving outcomes. TIC is a multifactorial condition with diverse phenotypes, involving complex pathophysiology. These variations complicate early diagnosis and targeted treatment. In the prehospital setting, phenotype-based management is not feasible; thus, empirical strategies have been adopted. Administration of tranexamic acid and prehospital whole bl

Simultaneous Branch Retinal Artery and Central Retinal Vein Occlusions following Blood Transfusion in a Young Man: A Case Report.

INTRODUCTION: Simultaneous branch retinal artery occlusion (BRAO) and central retinal vein occlusion (CRVO) is an exceptionally rare cause of acute vision loss, particularly in young individuals without systemic vascular risk factors. Blood transfusion has been implicated in rare thromboembolic and hyperviscosity-related complications but is infrequently associated with retinal vascular occlusions. CASE PRESENTATION: A 20-year-old man presented with sudden painless unilateral vision loss shortly after receiving a packed red blood cell transfusion for severe anemia. Ophthalmic examination reve
