An Anti-Amyloidogenic Approach to Specifically Block Memory Consolidation in Mice for Therapeutic Intervention.

Post-traumatic stress disorder (PTSD) is a mental health disorder triggered by exposure to a traumatic event, which manifests with anguish, intrusive memories, and negative mood changes. So far, there is no efficient treatment for PTSD other than symptomatic palliative care. However, the cytoplasmic polyadenylation element binding protein-3 (CPEB3) has been recently associated with a potential risk gene for PTSD. Considering that the CPEB3 protein is a functional amyloid whose importance for long-term memory consolidation in mammals is well established, we propose the active (amyloid) state of

Terminal agitation in dementia: Challenges and opportunities.

Terminal agitation (TA), also referred to as terminal restlessness or terminal delirium, is a complex, multifactorial clinical syndrome that occurs at the end of life. While TA is evident in dementia (including Alzheimer's disease), it is not exclusive to the condition and also occurs in other terminal illnesses, e.g., cancer. Currently, no clinical tools exist or have been validated to assess TA in dementia. This may hinder the correct identification of TA and further contribute to the incorrect estimation of TA prevalence. Due to the complex diagnostic profile of TA, it may be frequently und

Rapid-Onset Opioid-Induced Neurotoxicity During Morphine Therapy.

PurposeOpioid therapy is central to cancer pain management and aims to provide effective analgesia while minimizing neurotoxic adverse effects; however, cognitive dysfunction, delirium, and hallucinations continue to be significant complications.SummaryOpioid-induced neurotoxicity (OIN) has been associated with higher opioid doses, poor functional status, and advanced disease burden, and is frequently an underrecognized contributor to delirium. We describe an unusual case of visual and auditory hallucinations shortly after morphine exposure, with prompt reversibility after opioid rotation and

Machine learning uncovers previously undetectable genetic deletions in cancer panel sequencing.

Missed genetic deletions can prevent cancer patients from receiving effective targeted therapy. Standard pipelines, such as TSO500, use bin count averaging for copy number variation detection but often overlook focal events. To address this, we developed a machine learning (ML) pipeline for deletion detection and zygosity evaluation within the MASTERsg precision oncology program. We focused on the tumor suppressor gene PTEN, whose loss confers sensitivity to mTOR/Akt inhibition. An Extreme Gradient Boosting model trained on PTEN-deleted and wild-type tumor samples identified focal deletions mi

Public Health Pharmacology: a framework for population-level immunologic monitoring (PLIM) to optimize vaccine performance in Africa.

Despite high administrative immunization coverage across Sub-Saharan Africa, unexpected infectious disease outbreaks pose significant public health challenges. Traditional tracking relies heavily on logistical inputs and reactive surveillance, leaving an unmonitored biological gap in real-world host immunity. This paper presents a novel systems-design conceptual framework for Population-Level Immunologic Monitoring (PLIM) as a core operational tool of Public Health Pharmacology (PHP) to systematically bridge this gap. PLIM operates across three interconnected pillars: (1) biological sentinel s

Improving inpatient palliative care delivery in COPD: a mixed-methods study.

BACKGROUND: There is limited evidence to guide implementation of recommended collaborative palliative care (PC) models for hospitalized patients with chronic obstructive pulmonary disease (COPD). We sought to characterize palliative needs and identify barriers to PC delivery to inform inpatient collaborative care models. METHODS: We conducted a sequential explanatory mixed-methods study of hospitalized adults with COPD and physicians (hospitalists and pulmonologists) at two urban teaching hospitals in the United States. Patients rated their palliative needs in a survey, followed by a semi-str

Management of Heart transplant recipients for the non Heart Transplant health care provider. A Clinical Consensus Statement of the Heart Failure Association (HFA) and the Association of Cardiovascular Nursing & Allied Professions (ACNAP) of the ESC.

Heart transplantation (HTx) remains the gold standard treatment for selected patients with advanced heart failure and survival after transplantation has substantially improved over recent decades. As a consequence, the number of long-term HTx recipients continues to increase, and these patients are now frequently encountered in routine practice outside specialized transplant centers. Their management remains complex due to the use of chronic immunosuppression, transplant-specific complications, and a growing burden of comorbidities in the perspective of increasing longevity. Non-transplant car

Healthcare professionals and relatives in shaping the preferred place of end-of-life care and death for patients with advanced cancer: the WhereToCare qualitative study.

BACKGROUND: Although most people in France wish to die at home, the majority of deaths still occur in hospitals. The choices of end-of-life care for patients with advanced cancer and how these preferences are shaped by interactions with relatives and healthcare professionals, remains poorly understood. This study aimed to explore how patients with advanced cancer develop and adjust these preferences, and to examine the roles of relatives, general practitioners, and oncologists in this process. METHODS: We conducted a single-centre longitudinal qualitative study at a French university hospital

Associations between gastrointestinal symptoms and coexisting symptoms amongst patients in a palliative care unit: a PCOC-based longitudinal study.

BACKGROUND: Gastrointestinal (GI) symptoms are both common and distressing in patients receiving palliative care, however their relationships with coexisting symptoms and clinical factors, including functional status, clinician-rated problem severity, and palliative care phase, remain unclear in hospice ward settings. The Palliative Care Outcomes Collaboration (PCOC) framework enables for standardized daily symptom assessment and longitudinal evaluation. This study aimed to longitudinally investigate the associations between GI symptoms and related symptoms and clinical domains among patients

Evaluation of hospice and palliative care policies in China: a Theme-Instrument-Effectiveness (TIE) framework based on text analysis.

BACKGROUND: China is undergoing rapid population aging, which has substantially increased the demand for hospice and palliative care (HPC). Systematic and quantitative evaluation of China's HPC policy corpus remains limited. This study addresses this gap through an integrated three-dimensional analytical framework examining policy themes, policy instruments, and text-based policy effectiveness. METHODS: A corpus of 95 national-level, provincial-level, and selected sub-provincial pilot HPC policy documents (January 2011-May 2026) was analyzed using a Theme-Instrument-Effectiveness (TIE) framew

Needs among persons with advanced COPD and their informal caregivers: a multi-perspective qualitative study focusing on the potential role of palliative care.

OBJECTIVES: To explore needs among persons with advanced COPD and informal caregivers, and to explore how palliative care can address those needs. BACKGROUND: COPD is a common, progressive disease affecting lung function and represents a leading cause of death. Advanced COPD is associated with a high symptom burden and poor quality of life. Persons with advanced COPD may benefit from palliative care, but its use among persons with COPD remains limited, also in Germany. METHODS: A qualitative, multi-perspective interview study was conducted with persons suffering from advanced COPD, informal

Parkinson's disease mortality in the United States, 1999-2025: a national multiple-cause-of-death analysis with projections to 2030.

BACKGROUND: Parkinson's disease (PD) is the second most common neurodegenerative disorder in the United States and a growing driver of late-life disability and death. We characterized U.S. PD-related mortality from 1999 to 2025 and projected it to 2030. METHODS: In this serial cross-sectional analysis of CDC WONDER Multiple Cause of Death data, PD-related deaths in adults aged ≥45 years were identified using ICD-10 code G20 recorded anywhere on the death certificate. Age-adjusted mortality rates (AAMRs) per 100,000 were decomposed by joinpoint regression and projected to 2030 using a log-scal

Developing a Double Awareness Model Among Patients With Advanced Cancer in China: A Constructivist Grounded Theory Study.

OBJECTIVE: Double awareness, defined as the capacity to engage with life while acknowledging mortality, has been conceptualized primarily within Western cultural contexts. This study aimed to develop a contextually informed interpretive model of double awareness among patients with advanced cancer in China. METHODS: A constructivist grounded theory approach was adopted. Semi-structured interviews were conducted with 10 patients with stage IV or recurrent/metastatic cancer recruited from an oncology department and a hospice care facility in China. Data were analyzed iteratively using initial,
