Emergency department management of mild traumatic brain injury in the USA.

OBJECTIVE: To describe the emergency department (ED) management of isolated mild traumatic brain injury (TBI) in the USA and to examine variation in care across age and insurance types. METHODS: A secondary analysis of ED visits for isolated mild TBI in the National Hospital Ambulatory Medical Care Survey 1998-2000 was performed. Mild TBI was defined by International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9) codes for skull fracture, concussion, intracranial injury (unspecified), and head injury (unspecified). Available ED care variables were analysed by patient

Predictors of non-significant endoscopic findings in patients with suspected upper gastrointestinal tract hemorrhage.

OBJECTIVES: The objective of the present study was to determine pre-endoscopic predictive factors of nonsignificant endoscopic findings in patients with suspected upper gastrointestinal tract hemorrhage (UGIH). MATERIAL AND METHOD: Medical records of 187 patients admitted with the primary diagnosis of UGIH were reviewed. Non-significant endoscopic findings were defined as "normal": "mild gastritis" or unspecified gastritis with a hospital stay of two days or less. Possible predictors of non-significant endoscopic findings included pertinent history, physical examination, nasogastric tube aspi

Age-related changes in the relationship between visual evoked potentials and visually evoked cerebral blood flow velocity response.

BACKGROUND: Several reports have been published about the impact of coronavirus disease 2019 (COVID-19) vaccines on human health, and each vaccine has a different safety and efficacy profile. The aim of this study was to reveal the nature and classification of reported adverse drug reactions (ADRs) of the two COVID-19 vaccines (tozinameran and ChAdOx1) among citizens and residents living in Saudi Arabia, and show possible differences between the two vaccines and the differences between each batch on the health of populations. METHODS: A cross-sectional study was conducted in Saudi Arabia betw

Parental acceptance of adolescent vaccines within school-based health centres.

Parental acceptability of genital herpes (HV) and meningococcal (MV) vaccines within school-based health centres (SBHCs) was examined. A convenience sample of 85 parents completed a telephone survey. Eighty-five per cent of respondents supported offering the HV within SBHCs; 87% the MV. Sixty-two per cent of respondents would allow HV to be administered to their adolescent in the SBHC, 75% the MV. When location of vaccination was unspecified, 42% of parents would allow vaccination of their adolescent for HV, and 71% would permit MV vaccination. Parents supported offering HV via SBHCs; many wou

Learning with computerized guidelines in general practice?: A randomized controlled trial.

BACKGROUND: Evidence-based guidelines are seen as an important instrument to transfer scientifically generated knowledge into daily clinical practice and to ensure high standards of clinical care. Despite wide promulgation, clinical guidelines so far have a limited impact on individual professional learning and on changing daily medical practice. OBJECTIVES: Our aims were (i) to study a potential knowledge increase among German GPs after implementation of web- and evidence-based guidelines and (ii) to identify and analyse potential barriers to individual professional learning with computerize

Colorectal cancer screening and treatment: review of outcomes research.

BACKGROUND: Colorectal cancer is the second leading cause of cancer deaths in the United States each year. Screening is effective in reducing colorectal cancer mortality; however, compliance with screening is poor, and factors associated with its compliance are poorly understood. The outcomes of treatment of colorectal cancer (surgery, radiation therapy, and chemotherapy) may have profound effects on quality of life (QOL). Furthermore, colorectal cancer screening and treatment may be expensive, and the costs are important from a policy perspective. This review examines patient-centered outcome

Emergency nurses' utilization of ultrasound guidance for placement of peripheral intravenous lines in difficult-access patients.

OBJECTIVES: Emergency nurses (ENs) typically place peripheral intravenous (IV) lines, but if repeated attempts fail, emergency physicians have to obtain peripheral or central access. The authors describe the patient population for which ultrasound (US)-guided peripheral IVs are used and evaluate the success rates for such lines by ENs. METHODS: This was a prospective observational study of ENs in a Level I trauma center with a census of 75,000, performing US-guided IV line placement on difficult-to-stick patients (repeated blind IV placement failure or established history). ENs were trained o

Trends in inpatient pediatric trauma care in new England.

BACKGROUND: The annual number of childhood injury admissions to New England hospitals decreased by more than half from 1991 through 2000. This study was undertaken to determine whether this decrease was caused by decreased injury incidence or changes in the diagnosis and/or management of childhood injury. METHODS: Patients younger than 16 years of age with an injury diagnosis code were extracted from the New England Pediatric Trauma Database and an Injury Severity Score and hospital-specific variables were assigned. U.S. Census Bureau age-specific population estimates were used to generate po

Gastrointestinal disorders: are health care professionals missing zoonotic causes?

Acute gastrointestinal disorders (AGIDs) of presumed infectious origin are common illnesses and among the most frequent infective diseases referred to hospital. Much is known about the diagnosis and aetiology of AGIDs, but relatively little work appears to have been done regarding the assessment of relevant general practitioner (GP) history-taking and the potential for under-recognition of sources such as zoonoses. Established sources of zoonosis (for example exotic pet-keeping, farmyards and zoological parks) seem either rarely considered or annotated. Surveillance of food-borne illness and i

Prevalence of the chronic sinusitis diagnosis in Olmsted County, Minnesota.

BACKGROUND: Commonly cited estimates from the National Health Interview Survey rank chronic sinusitis as one of the most prevalent chronic diseases in the United States. These data rely on patient self-reporting of the disease. However, chronic sinusitis is difficult to diagnose because its symptoms overlap those of many other disease processes. As such, these prevalence data may be unreliable. OBJECTIVE: To provide an estimate of the prevalence of chronic sinusitis based on assigned International Classification of Diseases, Ninth Revision (ICD-9) codes for the year 2000 in a population-based

Trends in emergency department antibiotic prescribing for acute respiratory tract infections.

BACKGROUND: Injudicious use of antibiotics is associated with the reported rise in antibiotic-resistant bacteria. With an estimated 26 million antibiotics being prescribed annually in the emergency department (ED), the ED represents an important setting for targeting interventions. OBJECTIVE: To provide national estimates of potentially inappropriate antibiotic prescribing during ED visits for acute respiratory tract infections (ARTIs) and examine associations between patient, provider, visit characteristics, and antibiotic prescribing patterns. METHODS: A cross-sectional study was conducted

Respiratory tract infections: diagnosis and use of antibiotics by family physicians in north-eastern Poland.

The aim of our study was to describe the pattern of antibiotic prescribing for respiratory tract infections. Forty-four family physicians from Białystok (north-eastern Poland) filled in questionnaires regarding 1315 patients with respiratory tract infections. Antibiotics were prescribed in 803 cases (61.1%). The four principle diagnoses associated with prescribing an antibiotic were: bacterial pharyngitis/tonsillitis, acute bronchitis, unspecified bacterial superinfection and acute sinusitis for which a total of 572 antibiotic treatments were given (71.2% of all antibiotic prescriptions). Prevalence of hospital-acquired infections in a tertiary referral hospital in northern Tanzania.

Hospital-acquired infections (HAI) are a major and largely preventable cause of morbidity and morbidity worldwide. Very few reports on the prevalence of HAI in sub-Saharan Africa have been published and most of those that have appeared in the press have focused on surgical-wound infection. In the present, questionnaire-based, point-prevalence study, in which the doctor on the ward round was used as the primary informant, the prevalences of all HAI among all the inpatients at a tertiary referral hospital in northern Tanzania were estimated. On the day of the study, there were 412 inpatients (in
