Serving Vietnam Veterans Hospitalized Outside the VA System: A Scoping Review of Presumptive Service-Related Illnesses and Presentations.

BackgroundRecent legislation, the MISSION Act and the PACT Act, expanded access to and utilization of non-Department of Veterans Affairs (VA) health care; more Veterans now receive care from non-VA healthcare providers. Hospitalists outside the VA may be less familiar with Veterans' service-related exposures and presumptive service-related conditions. We aimed to summarize research findings on service-related exposures and conditions among Vietnam War Veterans.MethodsUsing Arksey and O'Malley's methodological framework for scoping reviews, we searched PubMed, EMBASE, and Web of Science databas

Severe mental illness, chest pain, and emergency response.

AIMS: How the prehospital chest pain triage contributes to inequities in cardiovascular outcomes remains poorly understood in patients with severe mental illness (SMI). We aimed to examine the association between SMI, emergency response, and cardiovascular health disparities in chest pain-related calls. METHODS: This registry-based cohort study included first-time chest pain-related calls to the Copenhagen Emergency Medical Services (emergency number and out-of-hours service) during the study period 2014-2018. We compared patients with SMI to patients without SMI (controls). We examined emerg

Global, regional, and national burden of chronic kidney disease and its underlying etiologies from 1990 to 2021: a systematic analysis for the Global Burden of Disease Study 2021.

BACKGROUND: We aimed to investigate global, regional, and national burden of chronic kidney disease (CKD) and its underlying etiologies from 1990 to 2021. METHODS: We summarized the results of the Global Burden of Disease (GBD) 2021 to derive the disease burden of CKD by considering four distinct types of epidemiological data, namely incidence, prevalence, mortality, and disability-adjusted life years (DALYs). The Joinpoint regression analysis, which is skilled in calculating annual percentage change (APC) and average annual percentage change (AAPC), was used to estimate global trends for CKD

Invasive Fungal Infection Increases Mortality Risk After Burn Injury.

While invasive fungal infections (IFIs) can be deadly for any hospitalized patient, a high-risk subset are those with burn injuries due to their immunocompromised state, and an increasing number of non-Candida infections have been on the rise. This novel study captures data from across the United States using a multi-institutional dataset derived from electronic health record data and is the largest study to date evaluating mortality associated with fungal infections in burn injury. Inclusion criteria identified all patients with burn injury from 2002 to 2024; IFI was defined as a diagnosis of

Development of a machine learning-based prediction model for acute kidney injury associated with respiratory failure in the intensive care unit.

Acute kidney injury (AKI) is a frequent and severe complication in intensive care unit (ICU) patients with respiratory failure, associated with high mortality, prolonged hospitalization, and substantial healthcare burden. Conventional risk scores, such as SOFA and APACHE II, are not optimized for AKI prediction in this heterogeneous population. This study aimed to develop and validate an early AKI prediction model using machine learning. We analyzed 10,780 adult ICU patients with unspecified respiratory failure from the MIMIC-IV database, of whom 53.96% developed AKI according to KDIGO criteri

Nomograms applicability in clinical toxicology - enhancing precision in clinical decision-making: a systematic review.

Nomograms represent powerful predictive tools that could be easily applied to guide managing acutely intoxicated patients. Thus, several nomograms were developed and validated in the last few decades to predict various outcomes following acute poisoning. However, the adopted nomograms remain sporadic efforts of researchers that limited their usefulness in clinical settings. We aimed to bridge the gap between theoretical formulation and hands-on application of the developed nomograms to benefit acutely poisoned patients. In this context, this systematic review was conducted to be a reference gu
