Pneumosinus dilatans (PSD) is an abnormal expansion of the paranasal sinus beyond the normal anatomical boundaries of the sinus and the associated bone, with normal thickness of the sinus wall. It is a rare clinical condition, with a relatively small number of cases documented in the literature. Numerous theories have been proposed to explain the etiology of PSD, of which only the ball valve mechanism and Genetic predisposition with hormonal trigger have some scientific evidence. Usually detected incidentally by imaging, however, it may present with symptoms such as swelling of the forehead (w

BACKGROUND AND PURPOSE: Facial injury consults often receive dual coverage by plastic and reconstructive surgery (PRS) and otolaryngology (ENT) services, with reported variations in management patterns between specialties. However, studies have not separated fracture patterns or reported variations in care specific to ENT or PRS. This single-institutional study evaluates differences in management and outcomes of frontal sinus fractures treated by PRS and ENT.
METHODS: A retrospective chart review identified adult patients with frontal sinus fractures from 2010 to 2020. Wilcoxon rank sum and F

OBJECTIVE: Feminizing frontal cranioplasty (FFC) is an essential technique in feminization of the upper third, often involving the violation of the anterior table of the frontal sinus. Though case reports document frontal sinus complications years after surgery, no studies to date examine the presence of delayed frontal sinus disease on long-term imaging after FFC.
STUDY DESIGN: Case series.
SETTING: Tertiary care center.
METHODS: Adult patients who underwent FFC with type III forehead classification between August 2016 to December 2019 were offered postoperative computerized tomography (CT

Frontal sinus (FS) and mandibular ramus morphology are valuable components of forensic identification because these structures are resilient to trauma, fire, decomposition, and fragmentation, and they exhibit high individual variability. This study aimed to evaluate the diagnostic performance of computed tomography (CT)-based FS morphology and mandibular ramus length (MRL) for sex estimation and age-group classification. CT scans of 150 adults (75 males and 75 females) aged 20 to 72 years were retrospectively analyzed. Measurements included frontal sinus height, width, depth, volume, and MRL.

BACKGROUND: This video illustrates a minimally invasive neuroendoscopic transorbital technique used for the removal of a right orbital cyst accompanied by frontal sinus reconstruction. The approach enables a direct and efficient corridor to orbital and anterior skull base lesions, reducing the need for brain retraction while ensuring both functional and aesthetic benefits.
CASE DESCRIPTION: A 38-year-old male patient presented with progressive exophthalmos, double vision, and elevated intraocular pressure caused by a right orbital cyst. Radiological evaluation reveals a thick-walled cystic fo

INTRODUCTION: Endoscopic frontal sinus surgery frequently poses significant technical challenges due to the complex anatomy of the frontal recess, and carries a substantial risk of restenosis and revision surgery. Removable frontal sinus stents have been used to maintain postoperative patency, but their limitations and complications are well recognised. Bioabsorbable steroid-eluting implants have emerged as an alternative designed to support wound healing and reduce postoperative scarring and restenosis. The aim of this study was to describe real-world utilisation patterns and clinical outcome

The Draf III procedure may serve as a surgical option for patients with frontal sinusitis that is refractory to standard treatment or prone to recurrence. However, postoperative restenosis caused by neo-osteogenesis and granulation tissue formation remains a major clinical problem. Coverage of exposed bone using mucosal grafts or pedicled flaps has been advocated to prevent restenosis, but adequate flap reconstruction may be difficult in revision cases with marked bony overgrowth. We report a case of a 62-year-old woman with refractory frontal sinusitis associated with IgG4-related disease and

Large B-cell lymphoma (LBCL) with IRF4/MUM1 rearrangement is a rare and newly recognized entity predominantly involving Waldeyer's ring of lymphoid tissue in the oropharynx. Unusual clinical presentations can result in delayed diagnosis of LBCL with IRF4 rearrangement, resulting in increased patient morbidity. We present a case involving a pregnant woman with a frontal sinus swelling and clinical diagnosis of chronic sinusitis and possible Pott's puffy tumor. An endoscopic procedure and subsequent histologic examination revealed diffuse LBCL with diffuse IRF4/MUM1 staining, supported by fluore

BACKGROUND: We aimed to determine paranasal sinus volumes using 3D volumetric measurements and to evaluate the effect of anatomical variations on these volumes, ensuring balanced age and sex distribution during childhood.
METHODS: Thirteen age groups (0-16 years), each including 10 males and 10 females, were formed. After excluding sinus pathologies, a total of 260 subjects were randomly selected from CT head examinations. Right and left frontal, maxillary, and sphenoid sinus volumes were calculated using 3D Slicer software (version 5.6.2) following manual segmentation of axial CT slices. Als

Background and Objectives: Frontal recess cells (FRCs) are key anatomical variants influencing frontal sinus drainage and disease. The International Frontal Sinus Anatomy Classification (IFAC) provides a standardized system for their identification. The baseline data on FRC prevalence and its association with sinusitis in Middle Eastern populations remain limited. This study aimed to assess the prevalence, laterality, and clinical relevance of FRCs in an adult Omani population using IFAC criteria. Materials and Methods: Computed tomography (CT) scans of 488 sides from 244 adult Omani patients

BACKGROUND: Post-traumatic headache (PTH) affects up to 80% of patients with mild-to-moderate traumatic brain injury (TBI), contributing to significant morbidity and healthcare burden. While early diagnosis and treatment can help prevent persistent headaches, neurology referral rates remain low. This study aims to (1) identify risk factors for PTH >1 year and (2) assess the frequency of neurology referrals in patients with craniofacial trauma.
METHODS: We retrospectively reviewed adult trauma patients who presented with frontal sinus, orbital, and mandibular fractures between (2018-2019). Our

INTRODUCTION: Skeletal maturity evaluation plays a critical role in orthodontic treatment planning. While cervical vertebral maturation index (CVMI) and dental calcification stages are commonly used, the potential of frontal sinus dimensions as an additional indicator has gained attention. This study evaluated the correlation of frontal sinus development with skeletal maturation of cervical vertebrae and calcification stages of permanent teeth.
MATERIALS AND METHODS: The sample consisted of 60 subjects, with a mean age of 13 ± 3.4 (range 8-20) years. Standardized pre-treatment lateral cephalo

BACKGROUND: Breast cancer is the most common malignancy in women, with invasive ductal carcinoma being the predominant subtype. Although metastases commonly affect the bones, liver, lungs, and brain, involvement of the paranasal sinuses is exceedingly rare and poses significant diagnostic challenges.
CASE PRESENTATION: A 70-year-old postmenopausal woman from Pakistan with a history of left-sided mastectomy and chemotherapy for invasive ductal carcinoma presented with an altered sensorium, aphasia, and reduced oral intake. Imaging revealed an aggressive, destructive lesion in the left frontal

PURPOSE: There has been concern regarding the applicability of the Carolyn’s Window Approach (CWA) to Asian patients because the nasal corridors in Asians are typically narrower externally with lower radix height compared to Western noses. This study evaluated the success rates and frontal sinus patency outcomes of the CWA in a cohort of Asian patients. METHODS: Patients (≥ 18 years) who underwent frontal sinus surgery via CWA were consecutively enrolled. Patients were followed at 3 and 6 months after surgery. Anthropometric data on nasal appearance were obtained from CT scans. The outcomes we

BACKGROUND: Retained surgical materials are a preventable source of postoperative complications. This study evaluates the radiographic detectability of retained neuropatties in the paranasal sinuses to support safer postoperative imaging and surgical practice. METHODS: Sixteen previously dissected cadaveric heads were labeled and assigned to predefined sinonasal or nasopharyngeal site using an allocation table (e.g., frontal sinus anterior table, maxillary sinus, sphenoid sinus floor, nasal cavity, nasopharynx). For each head–site combination, a large neuropatty (1 × 3 in) was first inserted a

Giant frontal sinus mucoceles, caused by ostial obstruction, can lead to severe complications such as headaches, vision impairment, and brain compression. Early diagnosis through imaging and prompt surgical intervention, such as craniotomy, is critical to remove mucopurulent debris and necrotic bone, preventing vision loss and intracranial complications. For giant frontal sinus mucoceles with extensive bony necrosis and intracranial extension, open craniotomy for complete debridement of mucopurulent debris and necrotic bone may be necessary over endoscopic marsupialization to prevent recurrenc
