Digitally Guided Frontal Sinus Fracture Fixation: A Point-of-Care "In-House" Biomodel Protocol with Cyanoacrylate-Assisted Fragment Stabilization.

Background/Objectives: Frontal sinus fractures are uncommon injuries that may cause persistent aesthetic deformity when the anterior wall is comminuted, as small irregular fragments are difficult to stabilize with conventional osteosynthesis alone. Methods: We describe a point-of-care digital workflow combining 3D planning/printing and cyanoacrylate-assisted fixation for an isolated comminuted anterior frontal sinus wall fracture. A young adult presented with a depressed forehead contour after assault; computed tomography confirmed at least four displaced fragments. Results: A two-part 3D-prin

Tomographic assessment of anatomical boundaries in endoscopic frontal sinusotomy: retrospective radiologic analysis of Draf IIA and IIB approaches.

OBJECTIVES: The aim of this study was to evaluate the anatomical dimensions and boundaries involved in Draf IIA (DIIA) and Draf IIB (DIIB) frontal sinusotomies, and to determine how complete frontal beak removal affects the frontal sinus drainage pathway. Specifically, we compared the proportional increase in drainage area between DIIA and DIIB performed with and without complete frontal beak removal. METHODS: This retrospective radiological anatomical study analyzed 296 hemifaces (from 148 patients) who underwent paranasal sinus Computed Tomography (CT). Individuals over 18-years of age with

Transorbital Approach to Surgical Resection of a Far-Lateral Frontal Sinus Osteoma.

A 21-year-old male presented with chronic left-sided rhinosinusitis and was found to have a left frontal sinus mass on maxillofacial computed tomography (CT). The mass measured approximately 1.9 × 1.3 × 2.3 cm in size and was contiguous with the intrasinus septum and anterior table of the frontal sinus. Imaging features were consistent with osteoma. An endoscopic endonasal approach resulted in subtotal resection of the mass. Eleven months later, the patient underwent a combined left transorbital and endoscopic surgical resection with oculofacial plastic surgery and otolaryngology. The mass was

Diagnostic and Therapeutic Approach to External Frontal Lesions: A Report of Two Cases.

Benign frontal masses represent a diagnostic and therapeutic challenge due to their aesthetic impact and the wide range of possible differential diagnoses. We present a comparative case series of two patients with benign frontal masses: a frontal lipoma and a frontal osteoma. Both cases highlight the importance of careful clinical evaluation, appropriate use of imaging studies, and tailored surgical planning to achieve optimal functional and aesthetic outcomes. Frontal osteomas are benign, slow-growing bone tumors arising from the cranial vault and represent the most common benign tumors of th

Steroid-Eluting Stents for Maintaining Frontal Sinus Patency After Endoscopic Sinus Surgery: A Systematic Review and Meta-Analysis.

BackgroundSteroid-eluting stents (SES) are used alongside functional endoscopic sinus surgery (FESS) to maintain sinus ostium patency and reduce Scar tissue formation and inflammation. Their effectiveness in minimizing oral steroid use and revision surgeries is debated. This review assesses the efficacy and safety of SES in maintaining patency of the frontal sinus ostium (FSO) and improving postoperative outcomes after endoscopic sinus surgery (ESS).MethodsThis review followed PRISMA guidelines and searched multiple databases, including PubMed and Web of Science, for studies published up to 20

Lateral Mucosal Flap With Steroid-Eluting Implant in Revision Draf Type III Frontal Sinus Surgery.

This technical note introduces a novel endoscopic strategy that integrates a laterally based endonasal mucosal flap with a bioabsorbable steroid-eluting implant to prevent restenosis following revision of Draf type III frontal sinus surgery. The vascularized flap is stabilized and maintained in close apposition to the underlying bone by a self-expanding bioabsorbable stent, which simultaneously provides sustained local corticosteroid delivery to reduce postoperative inflammation. By combining mechanical stabilization with targeted anti-inflammatory therapy, this technique represents a potentia

Open and Endoscopic Open-Assisted Repair of Anterior Skull Base Defects.

Although endoscopic surgery has become more prevalent and favorable than open approaches, open and combined approaches remain relevant. Such situations include extensive tumors involving potential flap donor sites, visualization and instrumentation limitations of the frontal sinus, and prior surgery/radiation requiring greater access and alternative flap reconstruction. Combined approaches use the foundation of open craniotomy and endoscopic techniques to preserve the frontal outflow tract and frontal sinus mechanism while repairing cerebrospinal leaks caused by anterior skull base defects.

Computed Tomography of the Nasal Cavities and Paranasal Sinuses of Cattle at Different Ages.

BACKGROUND: The nasal cavities and paranasal sinuses of cattle are complex anatomical structures that undergo significant changes throughout growth and maturation. Computed tomography (CT) is increasingly used as a routine imaging modality in small animals but remains less common in cattle due to technical limitations. However, it provides valuable diagnostic insight into the nasal cavities and sinuses, which are difficult to assess and often involved in disease processes. OBJECTIVES: This study aimed to provide a detailed description of these structures using CT in cattle of different ages i

Radiology in ragtime: Seeing beyond the air.

BACKGROUND: Walter Dandy's immense contribution to radiological diagnosis of cerebral disease in the form of intraventricular air is well known. His pivotal role in neuroradiological evaluation from 1918 to the 1970's was not without precedent, but how much the unexpected radiological diagnosis of traumatic pneumoventricle before 1918 influenced radiodiagnostics is the question this paper examines in detail. METHODS: A critical review of the first radiographically documented case of pneumoventricle, seen in 1912 by Drs. William H Stewart and William H. Luckett of New York City. Discussion is

Radiation-Induced Sphenoid Wing Meningioma After Childhood Medulloblastoma: A Case Report.

Radiation-induced meningiomas (RIMs) are a potential late sequela of pediatric cranial radiotherapy and may arise after a prolonged latency, often 10-30+ years after exposure. We report a 28-year-old male patient treated for medulloblastoma with surgical resection and cranial radiotherapy at age three, who later underwent resection of a right sphenoid wing meningioma at age 16. Following a period of stability, surveillance imaging revealed progressive regrowth at the prior craniotomy site, extending into the superior right orbit. He subsequently presented with dizziness, headaches, and visual

Persistent Forehead Sinus After Traditional Cautery: Frontal Mucocele Complicated by a Fronto-Mucocutaneous Fistula.

Frontal mucocutaneous fistula is a rare complication of chronic frontal sinus disease and may remain unrecognized because of its slow, insidious progression. This case report presents the clinical course, diagnostic process, and surgical management of this uncommon condition. A 56-year-old Ethiopian woman presented with a 5-year history of frontal swelling, nasal obstruction, and headache. Following cautery by a traditional healer, she developed a persistent draining sinus over the right frontal region. Examination revealed a mucocutaneous fistula discharging seropurulent fluid, while laborato

The Chinese carpet flap for anterior cranial fossa reconstruction in cases of depressed frontal fractures: 10 years' experience at a tertiary referral center.

Frontal sinus fractures account for 5–15% of facial injuries and endanger adjacent vital structures. We retrospectively analyzed 89 depressed frontal sinus fractures reconstructed with a pericranial flap (PCF) harvested in 29 of them at a tertiary center between 2014 and 2024 (MFM.IRB.ID code: R.25.08.3314). Most patients were young men (mean 18.3 ± 4.1 years); mechanisms included falls (42.7%) and motor-vehicle collisions (39.3%). Fracture types were isolated anterior table (n = 54), combined anterior/posterior tables (n = 35), and nasofrontal outflow tract (NFOT) involvement (n = 8). Using a

Improving Resident Frontal Sinusotomy Education With Three-Dimensional Surgical Planning Software.

OBJECTIVES: Frontal sinusotomy remains a technically challenging component of resident training in endoscopic sinus surgery. We hypothesize that the use of three-dimensional (3D) surgical planning software improves resident performance, as measured by blinded supervising surgeon evaluations. This study aims to assess whether 3D planning software enhances resident performance during frontal sinusotomy in live surgical cases. METHODS: This is a prospective trial involving unblinded self-evaluations and blinded evaluations from supervising surgeons. Participants alternated between an experimenta

Mixed Reality-Assisted Osteotomy Guidance for Single-Stage Skull Tumor Resection and Cranial Reconstruction: A Brief Clinical Study.

Single-stage cranial reconstruction using a patient-specific implant (PSI) requires precise intraoperative replication of the preoperative osteotomy plan. Minimal deviations may lead to implant mismatch and the need for intraoperative trimming. The authors report a case of meningioma with skull invasion treated with simultaneous tumor resection and cranial reconstruction using an ultra-high-molecular-weight polyethylene custom implant under mixed reality (MR)-assisted osteotomy guidance. The final 3-dimensional data set used for the implant fabrication was directly imported into the MR device

Current Concepts in Frontal Sinus Fracture Management.

Frontal sinus fractures typically reflect high-energy trauma and must be evaluated and treated carefully to avoid long-term problems including contour deformity, sinus dysfunction, cerebrospinal fluid (CSF) leakage, chronic sinusitis, and mucocele formation. This article outlines frontal sinus anatomy, diagnostic pathways, and evolving treatment concepts in detail. An anatomically driven treatment algorithm is emphasized, with a focus on preservation of sinus function whenever possible and preference for conservative management. Advanced procedures, such as endoscopic sinus surgery and cranial

Closed Reconstruction of Depressed Frontal Sinus Fractures: An Innovative Technique.

This article describes a novel technique for reconstructing a frontal sinus fracture that minimizes the risks associated with conventional approaches. In this study, we report a patient presented with a depressed frontal sinus fracture following a car accident. Through a small (5 mm) sub-brow incision, a freer elevator was inserted beneath the fractured fragments to elevate them. A 16-gauge venous catheter was then introduced horizontally from the medial aspect of the bone, exiting at the lateral side. Following this, a 24-gauge wire was threaded through the catheter, which was subsequently re

3D modeling of the frontal sinus as a contribution tool for human identification - a case report.

UNLABELLED: Forensic identification in advanced decomposition body is challenging when traditional methods become unreliable. This case report demonstrates the utility of 3D modeling of the frontal sinus for human identification in forensic anthropology and odontology. A decomposed body found in southern Brazil was successfully identified using 3D-2D superimposition technique, which overlays a three-dimensional model derived from antemortem computed tomography (CT) data onto postmortem radiographs. The frontal sinus, with its unique morphological patterns, served as a reliable anatomical marke
