When Febrile Seizures Are Not Benign: An Unusual Cause of Dyke-Davidoff-Masson Syndrome.

A 14-year-old girl presented with a history of recurrent seizures over the past year, varying in type-generalized tonic-clonic and right-sided focal seizures involving both upper and lower limbs. Birth and family history were unremarkable, and early developmental milestones were appropriate. However, her academic performance declined over the past 2 years, eventually leading her to drop out of school. At 7 months of age, she had an upper respiratory tract infection followed by febrile status epilepticus lasting 20 minutes and requiring 5 days of hospitalization. Neuroimaging, cerebrospinal flu

Anaesthetic Management of a Patient with Dyke-Davidoff-Masson Syndrome Undergoing Robotic Thermocoagulative Hemispherotomy: A Case Report.

Dyke-Davidoff-Masson Syndrome (DDMS) is a rare neurological disorder characterized by cerebral hemiatrophy and compensatory calvarial changes, typically resulting from early-life cerebral insults. Patients may present with drug-resistant epilepsy (DRE), hemiparesis, and developmental delay. Due to its rarity and heterogeneous clinical presentation, diagnosis is often delayed or missed. We present the anaesthetic management of a 36-year-old male with DDMS undergoing Robotic Thermocoagulative Hemispherotomy (ROTCH) for DRE. The patient had a longstanding history of focal seizures since infancy a

Developmental Morphometry of the Paranasal Sinuses in Subjects Aged 1-25 Years: A Computed Tomography Study.

The present study aimed to evaluate the developmental morphometry of the paranasal sinuses using computed tomography (CT) in subjects aged 1-25 years and to characterize age-related changes in sinus dimensions, volume, and surface area throughout childhood, adolescence, and young adulthood. This retrospective CT-based study included 250 subjects (125 females and 125 males) without radiologic evidence of sinonasal pathology. Bilateral morphometric analyses of the frontal, sphenoid, maxillary, and ethmoid sinuses were performed using multiplanar CT images and three-dimensional reconstructions. M

Diagnostic Evaluation and Surgical Management of Self-Inflicted Trans-Frontal Sinus Penetrating Brain Injury in a Patient with Schizophrenia.

Background: Non-missile penetrating traumatic brain injury (pTBI) is a rare but life-threatening condition. In self-inflicted cases involving the frontal sinus, a rigorous preoperative diagnostic workup is crucial to assess foreign body fragmentation, trajectory, and potential vascular compromise. The complexities of such cases demand a highly structured approach to prevent severe secondary brain injury and long-term infectious sequelae. Case Presentation: A 36-year-old male with schizophrenia presented after attempting suicide by stabbing a kitchen knife through his frontal sinus. Multimodal

High Sinus, Lower Risk: A Paradigm Shift in Frontal Fracture Patterns.

OBJECTIVE: The morphology of the frontal sinus (FS) has long been debated as a determinant of fracture type and severity. This study aimed to evaluate the association between sinus dimensions, fracture patterns, and clinical outcomes using real-world data. STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary trauma referral center. METHODS: We retrospectively reviewed the records of 48 frontal sinus fractures treated at a tertiary trauma center. Volumetric and dimensional measurements (volume, height, width, and anteroposterior depth) were obtained from CT scans. Each sinus was segme

An Unusual Presentation of Frontal Pneumosinus Dilatans as Flight-Related Headache With Favorable Response to Carbamazepine.

Pneumosinus dilatans (PSD) is an uncommon disorder marked by an abnormal hyperpneumatization of a paranasal sinus with intact bony walls. Although it is usually asymptomatic, it can cause headaches or other pressure-related symptoms, especially during changes in barometric pressure. We report a case of isolated frontal PSD in a 28-year-old man who complained of recurring, pressure-like frontal headaches that only occurred during air travel. The clinical examination, diagnostic nasal endoscopy, and neurological assessment were unremarkable. High-resolution computed tomography indicated hyperaer

Management of two osteomas in the frontoethmoidal region using an osteoplastic flap, endoscopic approach and 3D CT imaging.

The aim of this study was to demonstrate the application of preoperative three-dimensional (3D) computed tomography (CT) reconstruction in the surgical management of paranasal sinus (PNS) osteomas.Osteomas are the most common benign PNS tumors and can lead to significant symptoms, including chronic rhinosinusitis, frontal pressure, and orbital or intracranial complications. Surgical excision is recommended for symptomatic cases or those with potential complications, with endoscopic and external approaches being the primary techniques. A 74-year-old patient presented with symptoms of chronic rh

Bipedicled Laterally Based Pericranial Flap for Anterior Skull Base Reconstruction.

Frontal sinus fractures with posterior-wall disruption, dural injury, and cerebrospinal fluid (CSF) leakage require durable separation of the sinonasal cavity from the intracranial space. An anteriorly based pericranial flap is commonly used for this purpose, but its supraorbital and supratrochlear pedicles may be unreliable after frontal soft-tissue trauma or orbital rim fractures. A 67-year-old man sustained complex craniofacial trauma after a fall, including fractures of the anterior and posterior walls of the frontal sinus, a forehead laceration reaching the bone, CSF rhinorrhea, and midfa

Nontraumatic subperiosteal orbital hematoma secondary to frontal sinus mucocele: Case report.

Nontraumatic subperiosteal orbital hematoma (NTSOH) secondary to a paranasal sinus mucocele is rare and may be mistaken for an orbital abscess. A 40-year-old man presented with acute proptosis, periorbital pain, diplopia, and reduced vision in the right eye. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated a mass in the superior region of the right orbit and another mass in the right frontal sinus, hinting at NTSOH. An external approach to the frontal sinus combined with endoscopic sinus surgery (ESS) was performed for treatment. Intraoperative drainage of brown hemor

Small ethmoid mucocele with intracranial extension associated with fibrous dysplasia: A rare case report.

Ethmoid mucoceles are expansile, benign cystic lesions characterized by slow growth, predominantly affecting the frontal sinus, followed by the ethmoid sinus, and infrequently the sphenoid and maxillary sinuses. Intracranial involvement is rare and is typically associated with large or longstanding lesions. We present a case involving a 34-year-old immunocompetent woman who experienced non-specific facial pain subsequent to an influenza infection without exhibiting typical sinonasal or neurological symptoms. Computed tomography and magnetic resonance imaging revealed a small ethmoid sinus lesi

Atypical Inflammatory Laryngopharyngeal Web With Concurrent Nasopharyngeal Stenosis in an Older Cat.

An older neutered male Korean Shorthair cat was evaluated for chronic respiratory distress, stertor, dysphagia, and severe weight loss. Radiography and computed tomography revealed nasopharyngeal narrowing and a thin membranous web-like obstructive structure at the laryngopharynx, resulting in near-complete airway obstruction, with concurrent fluid-to-soft tissue attenuating material within the frontal sinus and tympanic bulla. Surgical exploration confirmed a membranous lesion, and histopathology demonstrated chronic ulcerative palatitis without evidence of neoplasia. This case suggests that

Surgical considerations in post-programmed cell death-1 therapy for cutaneous squamous cell carcinoma.

Cutaneous squamous cell carcinoma (cSCC) can present as advanced disease requiring systemic therapy. Programmed cell death-1 (PD-1) inhibitors such as cemiplimab enable significant tumor regression. We report a patient with advanced cSCC who achieved near-complete remission after cemiplimab, followed by Mohs surgery revealing unexpected frontal sinus communication. This case underscores the importance of considering underlying structural changes when operating in previously treated areas.

Recurrent Post-traumatic Frontal Mucocele Following Previous Endoscopic Management: Alternative Surgical Reconstruction With Partial Capsule Resection.

Frontal sinus mucoceles are benign but locally aggressive expansile lesions that may occur years after trauma or previous sinus surgery. Their progressive enlargement can lead to bone remodeling, facial deformity, orbital involvement, and intracranial complications. Endoscopic marsupialization is currently considered the standard treatment in most cases; however, recurrence remains possible, particularly in patients with altered anatomy secondary to trauma or previous surgical manipulation. This report describes the case of a 55-year-old man with a history of severe frontal trauma after a moto
