Voluntary and non-remunerated blood donation by 2025? A comparative study of blood donor eligibility in 19 healthcare institutions in Lebanon.

Eligibility criteria in blood donation are essential to protect both donor and recipient health. This study examines variability in donor selection criteria and donor health questionnaires across 19 healthcare institutions in Lebanon amid its transition toward a voluntary and non-remunerated blood donation (VNRD) system. Notable discrepancies were identified across medical, behavioral, demographic, and geographic deferral categories. A comparative analysis against Lebanese Committee for Blood Transfusion guidelines and EU-based standards reveals a lack of centralization and inconsistent adhere

Implications of occult hepatitis B infection for blood donor management: a five-year evaluation from Western Saudi Arabia.

BACKGROUND: Occult hepatitis B infection (OBI), defined by detectable HBV DNA in the absence of hepatitis B surface antigen (HBsAg), remains a diagnostic challenge and a potential source of transfusion-transmitted infection. In intermediate-endemic settings such as Saudi Arabia, serology-based screening alone may miss low-level viremia. This study evaluated the prevalence, serologic patterns, and demographic correlates of OBI among blood donors. METHODS: We performed a five-year retrospective analysis of 48,241 blood donors screened between November 2019 and November 2024 at a tertiary-care ho

Alpha-gal sensitization and allergic blood transfusion reactions: a scoping review.

BACKGROUND: Tick bites may expose individuals to a carbohydrate not found in humans, galactose-alpha-1,3-galactose (alpha-gal), including allergy to mammalian red meat or red meat-derived products presenting 2–6 hours after consuming the product plus positive alpha-gal specific IgE testing that may indicate alpha-gal syndrome (AGS). Anaphylaxis in group O recipients of group B blood products in the absence of other risk factors for allergic transfusion reactions could be transfusion-related alpha-gal syndrome (TRAGS); galactose-alpha-1,3-galactose (Gal-alpha1-3 Galβ1-(3)4GlcNAc-R) is similar t

Sentinel trends in donor hemoglobin values and low-hemoglobin deferrals in a large US blood center.

BACKGROUND: Low hemoglobin (Hb) is the leading cause of blood donor deferral yet mean Hb values are seldom examined as sentinel indicators of changes in donor recruitment, assessment, or testing. Case in point, following unexplained improvement in these parameters, U.S. blood centers observed a sharp rise in low-Hb deferrals after the 2024 introduction of a new international calibrator for some fingerstick hemoglobinometers. METHODS: We retrospectively analyzed 15 years (2010-2024) of allogeneic donor data from a national blood center. Mean donor Hb and low-Hb deferral rates were trended over

Intra-and Inter-Analyzer Imprecision of Cell Population Data on Sysmex XN-10.

INTRODUCTION: Cell population data (CPD) derived from modern hematology analyzers provide morphological and functional insights into leukocytes beyond traditional cell counts. Nevertheless, their introduction into clinical practice requires proven analytical precision and consistency across instrumentation. METHOD: Two K2EDTA blood samples (one from a healthy blood donor and one from an intensive care unit patient) were analyzed in ten replicates on two Sysmex XN-10 analyzers. Intra- and inter-analyzer imprecision were calculated as coefficients of variation (CV%). RESULTS: Intra-analyzer CV

Forecasting blood supply and demand under population aging: implications and challenges for healthcare resource allocation.

BACKGROUND: Population aging poses major challenges to health-care systems by increasing the prevalence of chronic diseases and raising the demand for blood transfusion. Blood is essential in surgery, cancer therapy, and critical care, which are frequently used in older adults. However, declining birth rates and diminishing younger cohorts threaten the sustainability of the blood donor pool. METHODS: In this study, we integrated demographic data from the Taiwanese household registration system, annual blood supply and demand data from the Taiwan Blood Services Foundation, and national populati

Molecular identification of Vel-negative blood donors in a population from southern Brazil.

BACKGROUND AND OBJECTIVES: The Vel antigen is clinically significant and its alloantibody is involved in haemolytic transfusion reactions. This antigen has a high prevalence in the population. The Vel-negative phenotype is the result of a homozygous deletion in the SMIM1 gene (c.64_80del) and hinders the expression of the SMIM1 protein. MATERIALS AND METHODS: A total of 17,472 blood donor samples from the Center for Hematology and Hemotherapy of Santa Catarina State were genotyped targeting the 17-nucleotide (c.64_80del) deletion in the SMIM1 gene. The same method was applied to the siblings

Advancements in Blood Group Genotyping Technology and Clinical Applications.

BACKGROUND: Blood group typing is essential in transfusion medicine, transplantation, and prenatal care. With the elucidation of the molecular genetics underlying blood group antigens, primarily single-nucleotide variants (SNVs), DNA-based genotyping has emerged as a powerful alternative to serological methods, enabling more accurate and comprehensive antigen prediction. CONTENT: This review outlines the evolution of blood group genotyping technologies, structured around 2 main paradigms: targeted assays and comprehensive genomic approaches. Within targeted methods, throughput has progressed

Genome-wide association of pica within a cohort of volunteer blood donors potentially implicates the gene encoding neuropeptide VF.

BACKGROUND: Pica is an eating disorder characterized by the persistent craving and consumption of non-food substances such as ice, chalk, starch, or raw pasta. Pica symptoms are more common in people with iron deficiency and resolve upon treatment. Without iron supplementation, volunteer blood donors can become iron deficient after repeated donations, making them an ideal population to study pica. STUDY DESIGN AND METHODS: A genome-wide association study (GWAS) was conducted with 12,157 volunteer blood donors within the Recipient Epidemiology and Donor Evaluation Study-III (REDS-III) RBC-omic

De novo HLA antibodies after blood transfusion and/or kidney transplant in children with kidney disease.

BACKGROUND: This study assessed de novo human leukocyte antigen (HLA) antibodies associated with blood transfusions in a paediatric kidney transplantation cohort, associated risk factors and outcomes. METHODS: Patients with kidney disease at Royal Children's Hospital (Melbourne, Australia) who were transfused and/or received a kidney transplant between 2000 and 2018 were included. HLA antibody results from pre- and post-transfusion serum specimens, blood donor typing (where available) and clinical and laboratory data were collected. A control group comprised non-transfused patients from the s

Deep immune-phenotyping of HLA-homozygous iPS-cardiomyocytes by spectral flow cytometry.

INTRODUCTION: Immunogenicity of allogeneic human induced pluripotent stem cell (hiPSC)-derived transplants limits their applicability in regenerative medicine. Selecting human leukocyte antigen (HLA)-homozygous hiPSC lines could be a mitigation strategy and haplo-matching would profoundly expand the number of potential recipients. Here we show deep immune-phenotyping of hiPSC-derived cardiomyocytes (iPS-CM) differentiated from four independent iPSC lines in three centers under chemically defined conditions. METHODS AND RESULTS: Broad immunophenotyping with 354 antibodies revealed differential

Seroprevalence of Hepatitis E Virus Infection among Blood Donors in Lebanon: A National Comparative Evaluation of Two Serological Assays.

Hepatitis E virus (HEV) is an emerging transfusion-transmissible pathogen, yet epidemiological data from the Middle East remain limited, and differences in serological assay performance complicate interpretation of seroprevalence. Lebanon lacks national HEV surveillance data and has no blood donor screening policy. In this cross-sectional national study, 3000 blood donors from all Lebanese governorates (July 2023-August 2024) were screened for anti-HEV IgG and IgM using two commercial platforms: Euroimmun ELISA and Roche Elecsys electrochemiluminescence immunoassay (ECLIA). This study represen

Seroprevalence of Hepatitis A Virus (HAV) and Hepatitis E Virus (HEV) Among Blood Donor Candidates in Salvador, Bahia, Brazil.

Hepatitis A (HAV) and E (HEV) are clinically indistinguishable diseases. This study aimed to determine the seroprevalence of both viruses among 469 blood donor candidates at HEMOBA, Salvador, Brazil. The seroprevalence of anti-HAV IgG was determined by chemiluminescence (Abbott Diagnostics, Chicago, IL, USA) and that of anti-HEV (IgG/IgM) was determined by ELISA (Wantai-Biopharm, Beijing, China). The ineligibility rate was 16.8% (79/469), mostly temporary. There were no statistically significant demographic differences between ineligible and eligible donor candidates. The participants were pre

Estimating the incidence of SARS-CoV-2 infections by jointly modelling seroprevalence, hospitalization and mortality data, February 2020 - January 2021, Belgium.

BACKGROUND: Belgium experienced two SARS-CoV-2 epidemic waves in 2020, in spring and autumn. Due to limited testing capacity, restrictive case definitions, asymptomatic infections, and incomplete testing compliance, case counts represent only a lower bound of SARS-CoV-2 infection incidence. We estimated this incidence from February 2020 to January 2021 by jointly modelling seroprevalence and surveillance data. METHODS: We developed a hierarchical Bayesian model that jointly fits seroprevalence, hospitalization, and mortality data to a shared latent incidence curve, represented by a spline. Th

Rapid and Robust Multiplex Multicolour Melting Curve Analysis Enables Simultaneous Detection of HLA Cross-Reactive Groups and HPA Genotypes.

Platelet transfusion refractoriness (PTR) is a major clinical challenge that increases bleeding risk and wastes blood resources. Immune-mediated PTR is mainly caused by alloantibodies against HLA class I and human platelet antigens (HPA). To overcome this, transfusion of compatible platelets based on HLA cross-reactive groups (CREGs) and HPA genotypes is considered effective; however, current detection methods remain costly, complex and time-consuming, and no simple approach is available for direct CREGs detection. In this study, we developed a novel multiplex multicolour melting curve analysi

The role of social media, influencers, and online communities in blood donor recruitment and retention: A systematic scoping review.

BACKGROUND: Blood services worldwide continue to face persistent challenges in recruiting and retaining voluntary, non-remunerated blood donors, particularly among younger populations. Over the past decade, communication strategies have shifted from traditional mass media to digital environments, where social media platforms, influencers, and online communities increasingly shape social norms, identity formation, and prosocial behaviour. These digital tools are now widely used to mobilise blood donors; however, the existing evidence base remains fragmented across disciplines, regions, and stud
