Molecular and Serological Evidence of Oropouche Virus Circulation in Asymptomatic Blood Donors During the 2023-2024 Outbreak in Manaus, Brazil.

BACKGROUND: Oropouche virus (OROV) re-emerged in Brazil in 2023, causing over 26 000 confirmed cases through December 2025. A reassortant lineage was linked to neurologic complications and congenital infections due to vertical transmission. Although transfusion transmission has not been reported, asymptomatic viremia in blood donors could represent an unrecognized route of infection. METHODS: We conducted nucleic acid testing (NAT), genomic, and serological surveillance among blood donors in Manaus, Brazil, during the 2023-2024 OROV outbreak. Minipools (MPs) of 18 donations from November 2023

Machine Learning for Donor Safety and Engagement: An Analytical Assessment.

Blood donor recruitment, retention, and safety face mounting challenges from an aging donor base and declining donation rates, and there is need for innovation in donor engagement. Machine learning (ML) offers tools to model donor behavior, physiology, and risk, but the maturity, methodological rigor, and translational readiness of this literature have not been systematically characterized. This study aimed to systematically identify and critically appraise studies applying ML methods to blood donation. We conducted structured searches of PubMed for articles published since January 1, 2018 (se

Detection of persistent viremia by a live attenuated vaccine strain of the chikungunya virus in a blood donor.

We report here the case of a blood donor who tested positive for chikungunya RNA by nucleic acid testing introduced in Guadeloupe (a French island in the Caribbean) to prevent the transmission of arboviral infections by transfusion. The strain was isolated, and further investigations revealed the presence of the IXCHIQ live attenuated vaccine strain in blood 1 month after vaccination.

Endemic progression of autochthonous West Nile virus infections in Europe.

Arthropod-borne West Nile virus (WNV) infections are highly prevalent under warm climate conditions. Our systematic review summarizes recent evidence on autochthonous human WNV infections in Europe considering increasing temperatures due to climate changes, and focuses on geographic expansion, reported drivers, and implications for One Health surveillance, diagnostics, and blood safety. The analysis of 27 included studies published since 2015 revealed progressive geographic expansion of autochthonous WNV infections including outbreaks, in several regions supported by phylogenetic data consiste

Pattern of Blood Donor Deferral and Donor Return Following Structured Telephonic Follow-up in a Tertiary Care Blood Centre in India.

OBJECTIVES: Blood donor deferral is an essential component of donor selection aimed at ensuring the safety of both donors and recipients. However, temporary deferral may negatively affect subsequent donor return and contribute to loss of potentially eligible donors. The present study aimed to evaluate the pattern of blood donor deferral and assess donor return following structured telephonic follow-up after temporary deferral in a tertiary care blood centre. METHODS: This prospective observational study was conducted in the Department of Immunohematology and Blood Transfusion at National Inst

Molecular and serologic analysis of linked blood donor and recipient samples to rule out transfusion-transmitted parvovirus B19 infection.

INTRODUCTION: Parvovirus B19 (B19V) has been found in asymptomatic blood donors. This prospective study tested linked blood donor-recipient samples from patients transfused at the National Institutes of Health (NIH) and affiliated hospitals for the presence of B19V DNA and anti-B19 immunoglobulin G (IgG) antibodies to assess the risk of B19V transmission by transfusion. MATERIALS AND METHODS: Post-transfusion recipient plasma samples collected during 2002-2020 were tested for B19V DNA at 1, 2, 4, and/or 8 weeks by nested polymerase chain reaction (PCR), and for anti-B19 IgG at 12 and ≥24 week

First-time blood donors experience less venipuncture pain than anticipated.

BACKGROUND: Little is known about the perceived pain associated with blood donation. This study aimed to investigate expected and perceived pain among first-time blood donors. STUDY DESIGN AND METHODS: In this prospective observational study, 140 consecutive first-time blood donors (70 men and 70 women) were included. The donors were asked to grade the expected pain prior to their first whole blood donation and to grade the perceived pain after the procedure. The pain was graded using an 11-point Numerical Rating Scale (NRS) from 0 (no pain) to 10 (worst imaginable pain). The association betw

First Usutu Virus Infection in an Asymptomatic Blood Donor in Greece.

Usutu virus (USUV) is a mosquito-borne flavivirus, widely distributed in Central Europe, where it causes avian outbreaks with large-scale mortality. Although most human infections are asymptomatic or mild, the reports of USUV neurologic infections are increasing, especially among immunocompromised patients. Cross-reactivity in serological and molecular assays is often seen between USUV and the genetically and antigenically related West Nile virus (WNV). Here, we report the first USUV infection in Greece in an asymptomatic blood donor who tested positive in the automated nucleic acid test durin

Prevalence and characterization of West Nile virus infections among people living with HIV in Northern Italy: a sero-repository analysis.

BACKGROUND: West Nile virus (WNV) is increasingly reported in Italy, yet its population prevalence remains uncertain. People living with HIV (PLWH) may be at increased risk of WNV severe manifestations, including neuroinvasive disease. This study assessed the seroprevalence and factors associated with WNV seropositivity among PLWH in a large Northern Italian cohort. METHODS: This retrospective single-center study included all PLWH followed at the Brescia HIV Outpatient Clinic with at least one serum sample collected in 2022 and stored in the institutional sero-repository. Samples were screene

Insights into parvovirus B19 clinical and virological dynamics among blood donors.

Coinciding with the exceptional European resurgence of human parvovirus B19 (B19V), between 2023 and 2024, twelve B19V DNA-positive blood donations out of 27.737 were detected by the plasma fractionating industry via nucleic acid testing (NAT) and notified to the blood collection centers of Trieste and Gorizia (Italy). In order to prevent potential B19V transmission, the implicated donors were recalled for clinical and virological follow-up for one year. None of the subjects had reported symptoms at the time of donation, though six subjects subsequently recalled either mild, transient symptoms

Structured Donor Re-engagement Following Temporary Deferral: A Prospective Implementation Study of Donor Re-entry.

BACKGROUND: Temporary blood donor deferral may interrupt subsequent donor participation. Structured follow-up may provide an opportunity for donor re-engagement and reassessment of eligibility. This study evaluated the implementation and donor re-entry outcomes of a structured donor re-engagement programme incorporating cause-specific recall and telephonic follow-up. METHODS: A prospective implementation study was conducted from January to June 2026 at a tertiary-care teaching hospital blood centre in India. Temporarily deferred donors who consented to follow-up received cause-specific counse

The neurodivergent blood donor: Fairness, discrimination, screening, and behavior.

BACKGROUND: We broaden the ongoing drive to diversify blood donor panels by proposing and testing a framework to explore neurodivergence and blood donation. Drawing on a range of theoretical perspectives, we predict that neurodivergent people will be underrepresented in donor panels, donate less frequently, perceive donor selection as less fair and more discriminatory, be more likely to guess when answering screening questions, and find these questions upsetting. STUDY DESIGN AND METHODS: 4376 members of the UK general-public and 8668 NHSBT blood donors indicated if they had a formal or self-

Leukemic stem cell subtypes determine venetoclax resistance and therapeutic vulnerabilities in AML.

The BCL-2 inhibitor venetoclax has transformed the treatment of acute myeloid leukemia (AML), but relapse due to resistance of leukemic stem cells (LSCs) remains a major challenge. By molecular and functional profiling of LSCs from >150 patients, we identify four LSC subtypes. These mirror distinct hematopoietic lineage stages, which determine the expression ratio between the venetoclax target BCL-2 and resistance-inducing proteins MCL-1 and BCL-xL (MAC-score). Longitudinal analyses reveal that venetoclax resistance mostly arises in LSCs through plasticity toward a megakaryocytic/erythroid-pro

Blood Donation Deferral at a Tertiary Centre in Riyadh: Epidemiology, Sex-Specific Patterns, and Donor Retention Outcomes.

Background: Blood donation deferral poses a significant challenge to transfusion services worldwide, threatening blood supply adequacy and donor retention. Population-specific data on deferral epidemiology and subsequent donor return behavior in Saudi Arabia remain limited. This study aimed to characterize the prevalence, reasons, and demographic patterns of blood donation deferral at King Fahad Medical City (KFMC)-a major quaternary referral center in Riyadh-and to quantify the rate at which temporarily deferred donors re-engage with the donation process at the encounter level. Methods: A ret

Combining sandwich and competitive immunoassay formats in a duplex lateral flow test for rapid iron-status assessment.

BACKGROUND: Reliable assessment of iron status benefits from the combined measurement of complementary biomarkers, but implementation in rapid lateral flow formats is complicated by large differences in biomarker concentration ranges and assay behavior. Soluble transferrin receptor (sTfR) occurs at substantially higher physiological concentrations than ferritin, creating a risk of high-dose hook effects in conventional sandwich lateral flow assays (LFAs). RESULTS: Here, we report a mixed-format duplex LFA for the simultaneous quantitative measurement of ferritin and sTfR in human plasma. The
